What you will do:
As a PhysicalTherapist, you are responsible for direct patient care. Integrates family members into the treatment, educational and discharge planning process for a safe transition home, and to ensure continued care needs are met. Effectiveness of education measured by peer input, family verbal feedback and patient satisfaction surveys and quality metrics. Educational methods are adapted to meet patient and family education needs. Participates with multidisciplinary team, as needed, in all aspects of discharge planning including determination of appropriate next level of care. 7. Participating in continuing education/professional development activities. Attends/presents at departmental in‐service sessions as scheduled. Attends external professional growth activities as scheduled and communicates information to the team. 8. Adhere to policies and practices of applicable federal and state guidelines, professional organizations, entity facilities and entity specific policies. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city.

KEY ESSENTIAL FUNCTIONS:
Provide Child Welfare Preventive Services exclusively or primarily within the client's home and community
Learn and implement the TST model
Improve caregiver-child relationship as a way to restore the child's functioning
