Key Responsibilities

The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes on an intermittent visit basis. Document all patient visits, assessments, and interventions in accordance with Medicare, Title 22, ACHC, and agency standards for accuracy, timeliness, and completeness.
Educate patients and caregivers regarding safe and effective use of equipment, exercise programs, mobility training, and strategies to maximize independence.
Monitor patient progress and communicate changes in condition or plan of care to the physician and interdisciplinary team promptly.
Adhere to all agency policies, procedures, and safety protocols while promoting quality patient care and regulatory compliance.
Participate in case conferences, staff meetings, and ongoing training as required.
Submit all clinical documentation within 48 hours of each patient visit.

We are currently hiring for a Primary Therapist at our 6-bed residential treatment center in La Mesa, CA
Requirements:
Act as the point-person for each assigned client and their family during treatment
Provide individual and family therapy to each client at a weekly frequency commensurate with the current level of care
Facilitate 1-2 groups per week and complete group notes in the medical record
Orient new clients and their caregivers to treatment and the program structure
Facilitate Biopsychosocial Assessments and Risk Assessments for clients upon admission
Create comprehensive treatment plans for clients, monitor progress and adjust, as needed

Job Overview

The work environment is dynamic, and the caseload is manageable, encompassing a diverse population, including low-income and geriatric individuals. The role involves working with adolescents and adults, with a focus on adults.
FQHC level, the focus typically revolves around addressing mild to moderate concerns. This involves conducting 1-hour intakes, with 30-minute sessions allocated for adults and 45-minute sessions for children. Patients are typically seen monthly, not weekly, averaging 12 patients per day. There is a particular need for FQHC therapists with expertise in providing therapy to children aged 5 to 10 years of age.
CCBHC level is designed to address moderate to severe concerns, offering weekly therapy sessions lasting 45 minutes each. The average daily workload for therapy is set at 9 sessions. Specialized experience is sought for CCBHC therapists who have a background in working with pregnant and post-partum patients.

What you will do:
Perform evaluations and develop treatment plans

What you'll be doing:
The Milieu Therapist is a licensed therapist who facilitates skills groups and psychotherapy groups based on core curriculum, provides meal monitoring and meal support, and offers milieu support, de-escalation and behavior coaching to patients in inpatient, residential, partial hospitalization and intensive outpatient levels of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Facilitates curriculum-based groups
Provides meal monitoring and support.
Documentation of groups, meals and patient check-ins and observations 15% Provides support to Milieu including the following:
De-escalation and crisis management
Bathroom monitoring and support for eating disorder patients
Monitoring outside breaks Conducts self-harm and suicide assessment
Support morning routine in IP/Res programs
Collects and reports weights, vitals and urine in some settings
Implements individual behavior management plans for some patients and serves as clinical lead for the program in standalone programs in evenings and weekends. Will lead some psychotherapy groups.
Coverage for Primary Therapists (for individual, family and group therapies as well as for patient assessments) with support from Clinical Manager or Clinical Director
Must Haves:
Graduate degree in mental health or related degree program- required
Bachelors degree- required
Intern or associate license from appropriate state licensing board- required
Strong knowledge of therapeutic principles and techniques- required
Ability to manage group dynamics effectively Proficient in keeping timely, detailed and accurate records- required
Demonstrated ability to handle crisis situations effectively- required
Excellent written and verbal communication skills- Required
Bonus Qualities:
1+ year facilitating group-based treatment interventions- preferred
1+ year providing crisis management support and interventions- preferred
1+ year performing safety assessments and implementing safety management plans to patients in treatment at higher levels of care- preferred
