Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
Establishes a plan of care and implements individual treatment based on evaluation results and functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. As a Therapist with FCT, you will:
Work a flexible weekly schedule that includes some evening hours for client appointments, bases on client/family availability
Learn the evidenced-based framework of FCT and the four phases of treatment-including completing Level 1 Certification within 12 months of hire, supported by the supervisor and team trainer.
Provides direct clinical treatment using methods compatible with FCT principles and practices for assigned cases and meets fidelity requirements for treatment intensity (5 hours per week per family). Caseloads vary from 3-5 families, depending on travel.
Maintain clear, concise, and timely documentation records in an Electronic Health Record system to meet state, county and EBP standards.
Collaborate with all relevant systems and key participants to ensure buy-in and cooperation throughout FCT treatment
Participate in weekly individual and team supervision and provide kind, direct, and honest feedback to team members, including participation in peer reviews.
Participate in a rotating 24/7 caregiver coaching support system that has been established by the team (1-2 weekends in a quarter).

Major Responsibilities:
Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
Completes all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate, to record patient care and progress of each patient assigned as well as daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
Pursues professional growth through participation in continuing education courses, department inservices, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:

We're Hiring!
We are seeking a licensed Co-Occurring Substance Abuse Disorder Therapist or Co-Occurring Therapist-in-Training to join our team. This full-time position will serve clients in our Menasha Counseling Clinic by providing therapeutic counseling.
Key Responsibilities
Provide a comprehensive assessment by collecting all information outlined in the program's Biopsychosocial Assessment Form.
Provide and updates diagnostic impressions based on information collected from the assessment.
Develop a treatment plan by establishing treatment goals with the client based on the assessment and diagnosis(es) and obtaining client/parent's signature on treatment plan.
Implement treatment by providing psychotherapy in accordance with diagnosis and treatment plan, applying a variety of psychotherapeutic methods with individuals, couples, families, and groups.
Monitor progress toward treatment goals by evaluating and adjusting services provided, completing initial and ongoing clinical case review requirements as necessary.
Qualifications
Education Requirement:
Master's degree in social work, counseling, psychology or other closely related mental health discipline.
Licensure Requirements:
Wisconsin state certification as a Licensed Clinical Social Worker or Professional Counselor, or In-Training Licensure.
CSAC (Clinical Substance Abuse Counselor), SAC (Substance Abuse Counselor), or SAC-IT (Substance Abuse Counselor In-Training
Must be eligible for Medical Assistance, Medicare (if applicable) and other third-party reimbursement.
Other Requirements:
Family Services is able to obtain malpractice insurance on your behalf.
Maintain eligibility to practice and active status with your licensing board through the State of Wisconsin Department of Regulation and Licensing.
Maintain eligibility for reimbursement from Medicaid, Medicare (if applicable) or other insurance panels.

Compensation range $55,000 - $70,000 annually.
