The Physical Therapist will provide ongoing therapy evaluations and treatments and education to clients in a timely manner per physician's orders. Adult Intensive Services (AIS) serves clients experiencing Severe and Persistent Mental Illnesses (SPMI) who need a higher level of care than traditional outpatient services may provide. AIS provides responsive, “wrap around” support including individual therapy, case management, psychoeducation, group therapy, and crisis intervention services in virtual, office, and community settings. In addition, AIS provides coordination of care, consultation, and advocacy with internal programs, external agencies, community resources, family members, and other client supports. This is a dynamic and exciting position which requires strong clinical, communication, and problem-solving skills along with a creative, responsive, and trauma informed approach to care. AIS offers many opportunities for growth and learning as well as a flexible schedule that combines time working from home with time working in the office and in the community. Schedule: Monday - Friday 8am - 5pm; this position will require some travel between patient's homes. Milage paid at Federal Government Rate.
The Physical Therapist will provide ongoing therapy evaluations and treatments and education to clients in a timely manner per physician's orders. To provide leadership and supervision of physical therapy services to other physical therapists, assistants and aides. The Physical Therapist evaluates and treats clients, communicates with families, physicians and other health team members and maintains appropriate documentation of services and complying with State and Federal regulations. Participates in on-going agency meetings and conducts in-service and training for agency staff on ongoing basis.
QUALIFICATIONS:
