Essential Position Functions:
Employed and compensated by The Bridge for up to 14 hours per week, which includes time allocated for client sessions, team meetings, required trainings, and individual supervision.
Provide client sessions (in person at the MHOTRS or remotely) totaling 14 hours per week, including time for weekly team meetings, required training, and individual supervision.
Conduct screenings and assessments using validated tools to identify depression, trauma, substance use, and risks/needs associated with criminal behavior.
Develop recovery-oriented treatment plans and reviews in collaboration with clients, collateral contacts, and consented stakeholders (e.g., significant others, criminal justice personnel, primary care providers, care coordinators, and schools).
Coordinate care with Bridge psychiatrists, nurse practitioners, administrative staff, and residential staff involved in the client's services.
Utilize evidence-based, sequential screening to assess risk of violence, and apply appropriate risk management protocols to effectively serve high-risk clients.
Document all treatment contacts using collaborative documentation practices.
Apply best and evidence-based treatment approaches consistent with The Bridge’s organizational culture, including integration of health and mental health services, rehabilitation and recovery models, and trauma-informed care.
Requirements:
LMSW, LMHC-LP, LCSW, or LMHC required.
At least 2 years of experience providing direct services to individuals with serious mental illness, co-occurring substance use disorders, and comorbid medical conditions in a community-based setting, with demonstrated knowledge and experience in assessment, treatment planning, documentation, and person-centered care.
High degree of computer literacy, including proficiency with Electronic Health Records (EHR).
Demonstrated expertise in recovery-oriented, evidence-based practices.

About the Role
The FFT Therapist provides FFT (Functional Family Therapy) virtually, to a maximum of 10 voluntarily participating families at a time who reside in NYC. The target population of young adults (16-25 years old), who are referred to us after an arrest. Rising Ground is participating in a pilot program to offer FFT to families and young people who might otherwise find themselves incarcerated without this alternative. FFT is an evidence-based model which is home-based, intensive, family therapy. The FFT Therapist participates in all FFT training and consultation activities, and program reviews. In addition to providing Family Therapy, the FFT Therapist will collaborate with other service providers, including but not limited to the Mayor’s Office of Criminal Justice (MOCJ) and New York City Criminal Justice Agency (CJA). The FFT Therapist is required to complete all FFT documentation requirements, including meeting appropriate therapeutic and engagement requirements and assessing for family safety and risk.
What You’ll Do

About the Role
Functional Family Therapist (FFT)
provides evidence-based intervention for youth and their families. This high-quality, strength-focused family counseling model is designed primarily for at-risk youth who have been referred by ACS. Services are conducted in home settings and can also be provided in schools and child welfare facilities. Flexible schedule including some nights and weekends, depending on needs of the client.
What You’ll Do
Conduct assessment including review of referral information, identifying, and engaging key participants, identifying systemic strengths and weaknesses, and developing an analysis of the fit problem behaviors within the ecological context.
Engage primary caregivers and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement.
Implement problem conceptualization, treatment planning, intervention implementation, outcome review and strategy revision procedure using the models Analytic Process.
Maintain clear and concise documentation of treatment efforts that promote peer and supervisory review and feedback, and that demonstrate compliance with model Analytic Process.
Collaborate with all relevant systems and key participants within each system to ensure their buy-in and cooperation throughout treatment.
Participate in all model program training, supervision, and consultation activities.
Provide home-based clinical treatment.
Travel requirements may include travel from Rising Ground sites and various locations, including home-based clinical treatments and school visits. Work-related travel expenses will be reimbursed by agency.
Your Qualifications
Master’s degree from an accredited university in a field that has professional licensure. The field includes, but is not limited to, the following disciplines: Social Work, Mental Health Counseling, and Psychology.
Clinical experience and experience with evidence-based models preferred.
Good written and oral communication skills; computer literate.
Experience in child welfare field strongly preferred.
Excellent time management skills with the ability to meet deadlines in a timely manner while managing competing priorities.
Prefer bilingual, (preferably Spanish) to conduct case management for our families.
Proficient use of Microsoft applications; knowledge of PROMIS/CNNX desired. Cultural awareness and sensitivity when serving clients that come from different backgrounds and walks of life.
Supervisory Responsibility: No
Work Environment: Hybrid (2) Remote (3) On-site.
Position Type/Expected Hours of Work: This is a full-time position. Days and hours of work are Monday through Friday 9AM - 5PM. Schedule may vary from Director or Supervisor discretion on program need.

Ability to work in-person in New York City (NY) for at least 3 days or more per week.
