Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes. Provide a written evaluation and plan of care defining occupational therapy needs, goals, progress and expected outcomes. Delivery of occupational therapy services to comply with prescribed physician orders. Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate. Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge. Participates in coordination of services with health care team, physicians, parent/guardians, nurses, etc. Documents according to policy and procedure and requires minimal supervision or instruction. Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payers Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff. Supervises other personnel (OTA, HHA) as applicable. Completes OASIS assessments where allowed by state professional practice and regulation. This position works closely with youth, family, foster parents, and community partners, providing counseling or therapy, advocacy, and support toward the ultimate goal of living safely and successfully in the community. Responsibilities * Actively supports, represents, and extends the mission, vision, and values of the organization. * Provides appropriate individual, group, and family mental health services, case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds on behalf of persons served. * Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements. * Completes all required documentation in a timely manner. * Completes all required training in a timely manner. * Consistently achieves established productivity thresholds. * Coordinate and monitor services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services. * Regularly reviews and modifies youth based plans to assure consistent progress and success on written plan goals. * Establishes and maintains strong relationship with assigned foster parents and families of origin. * Assists in family reunification and/or stabilization efforts with the youth's primary family or other permanent living arrangement consistent with the youth's case plan. * Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served. * Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis. * Actively participates in the weekly supervision process. Minimum Qualifications * Master's Degree in Social Work or comparable Human Services field from an accredited institution. * 2 years of work experience working in direct service with youth and families strongly preferred. * Must have an intern license, or working on licensure. * Must have a valid driver's license, reliable transportation, automotive insurance, and a good driving record. * A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served. * Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers. * Capacity to remain objective and professional in all areas of job function. Demonstrates tolerance and respect for the ideas and actions of others. * Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds. Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs. * Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability.     The Crisis Center of Tampa Bay brings help, hope and healing to people facing serious life challenges or trauma resulting from sexual assault or abuse, domestic violence, financial distress, substance abuse, medical emergency, suicidal thoughts, emotional or situational problems. The position of Therapist I provides ongoing counseling for individuals who have been victims of trauma and their families. This position provides services under the direct supervision of and reports to the Manager of Clinical Services and in their absence, the Director of Clinical Services. Strategic/Transformational Duties and Responsibilities * To provide help, hope and healing to victims of trauma by providing ongoing counseling. Also assists with raising awareness in the community through events. * Involves staff and other stakeholders in the performance quality improvement (PQI) process and uses data to improve services and outcomes. Transactional/Administrative Duties and Responsibilities * Provide therapy for survivors of trauma, including but not exclusively to, domestic violence survivors, sexually abused juveniles and adult rape victims, adult incest survivors and their families, individually, in group sessions, and/or family sessions. * Provide case management services for clients. * Consult with and make recommendations regarding treatment and treatment needs to Child Welfare and the courts as requested. * Conduct assessment interviews with potential clients. * Document client cases within one week of seeing the client and complete psychosocial histories, treatment plans, other documentation, and diagnosis using the DSM following protocol. * Participate in community awareness and advocacy activities. * Participate in regular clinical supervision, as required by Department of Professional Regulations. * Complete State required outcome measures; receive State Rating Certification as required by funding source. * Complete ACT Service Training as required by FCASV. * Assist in coordination of clinical services as assigned by Manager of Clinical Services. * Represent the Agency at appropriate meetings or task forces. * Assist with supervision of interns. * Assist with training programs related to trauma. * Perform such other duties as may be assigned by the Manager of Clinical Services or Director of Clinical Services.
