Job Summary
Plans, organizes and delivers physical therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions clients/patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. Demonstrate and incorporate knowledge of current research into daily treatment of clients/patients/residents.
May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization.
Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Will be interacting with individuals outside of the department including but not limited to: providers, case managers, third party payers, public organizations, companies/contractual partners, etc. May be asked to supervise assistants, aides, and students as they provide client/patient/resident care, treatment, or other services. Qualifications
Appropriate education level required in accordance with state licensure.
Must be licensed in the state(s) of practice as a Physical Therapist.

Obtains and subsequently maintains required department specific competencies and certifications. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

The Mental Health Therapist will provide effective counseling skills that facilitate client growth in ways that support their goals and interests, promote client's welfare and demonstrate valuing and respect for client's diversity and cultural contexts.
Essential Job Responsibilities:
To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily. The essential functions include the following:
Responsible to ensure and/or deliver and properly document the provision of mental health services including standardized mental health screening, diagnostic assessment, individualized treatment plan, individual, group and family therapy, mental health case management, progress notes, discharge summary at termination of services, and the maintenance of client records in accordance with Minnesota Statutes for Mental Health (245I MN Statutes) and MnTC policy.
Mental Health Services:
With the treatment director (s), ensure that clients receive a standardized mental health screening;
If clients score positive on this screening, complete a diagnostic assessment (DA) within ten days of admission or immediately after 1st session whichever comes first; complete other diagnostics as indicated by the DA or required by third party payers.
Complete multidisciplinary case reviews that include a monthly review for each client; document monthly reviews for each client in a progress note and per documentation requirements.
Provide case consultation and clinical supervision to licensing candidates and interns.
With the treatment director (s) ensure that family education is offered that addresses mental health and substance use disorders and the interaction between the two.
Integrate diagnostic assessment into the Individual Treatment Plan which is subject to amendment until services to the client are terminated. The individual treatment plan must include treatment goals, specific methods to be used to address identified problems, resources to which the client is being referred, and client goals. Complete update of treatment plan before 2nd session.
Enter clinical charting after each treatment service. The progress notes must reference the individual treatment plan and link the specific service provided with a problem identified in the treatment plan.
Assist the completion of a discharge summary within five days of the client's termination of services.
Ensure the content and format of client records be uniform and entries in each case are signed and dated.
Provide individual and group counseling, client education strategies to avoid inappropriate chemical use and health problems related to chemical use, transition services, and services to address issues related to co-occurring mental illness.
Lead client education lecture classes as required.
Participate in the weekly treatment team meetings to review and update client progress notes.
Coordinate with the case manager regarding the treatment plan
Coordinate mental health services with other service providers as necessary to the treatment plan.
Continuing Education and Training:
Receive 8 hours of co-occurring disorder training annually.
Complete continuing education courses in current treatment concepts and methods as required by the State of Minnesota, and the licensing board, to maintain mental health license.
Attend required in-house training and staff meetings.
Other:
Maintain credentialing with the State of Minnesota and health plans
Meet billable hours requirements as required by Supervisor; follow billing procedures
Insure that all necessary releases, Physician Communication Form, Notice of Privacy Practices form and the Financial Agreement are current (re-signed each year).
Required Skills & Experience (Minimum Qualifications):
Meet the definition of a mental health professional as defined in Minnesota Statutes
At least 21 years of age.
Must have a master's degree in a mental health related field.
Must be licensed within the State of Minnesota as a psychologist (LP), marriage and family therapist (LMFT), professional clinical counselor (LPCC), or clinical social worker (LCSW).
At least 1 year of freedom from problematic substances.
Must be comfortable working with people with life-controlling challenges, including mental health and addiction.
Must have or obtain 12 hours of training in co-occurring mental health problems training within six months of hire.
Ability to provide individual and group counseling including crisis intervention.
Must document competence in screening for and working with clients with mental health problems, through education, training, and experience.

We are currently hiring for the Behavioral Health Clinician (BHC) position, a key role within a multi-disciplinary team in the Integrated Behavioral Health model. A Behavioral Health Clinician (BHC) is a fully integrated Mental Health Professional and core member of the primary care team that works under the direction of the primary care provider(s). The role of the BHC is to support the primary care team in the early identification and treatment of behavioral health conditions within the defined clinic population. The primary functions of the BHC are to:

POSITION: SBCC currently has a need for a 0.8 FTE or 1.0 FTE outpatient therapist in our Northfield office, approximately 24-28 client sessions per week.
POSITION MINIMUM QUALIFICATIONS: A master's degree in a field of counseling, psychology or social work and fully licensed or on track for licensure as an LMFT, LPCC or LICSW. Supervision would be provided to pre-licensed therapists.
LOCATION: Northfield, Minnesota with the potential of some telehealth work as well.
SCHEDULE: Schedule is flexible, but an ideal candidate would be able to work 2-3 evenings per week.
COMPENSATION: Clinicians at SBCC are W2 employees. One way that SBCC offers flexibility and autonomy for our clinicians is through the choice of two pay methods. We offer a salaried compensation method, with stability in pay but more set hours, and a pay-per-code method with significant flexibility in hours but some variation in pay from week to week. While our salaried compensation method does have billable hour expectations, we provide you significant support to reach these goals using a non-punitive model.
Licensed Mental Health Professionals (LPCC, LMFT, LICSW): 1.0 FTE salary starts at $70,000-$80,000 for independently licensed clinicians. Licensed clinicians who prefer a pay-per-code option start at $45-$60 per billable hour. Compensation is based on experience and other factors.
Pre-licensed therapists: 1.0 FTE salary starts at $55,000-$60,000 for pre-licensed clinicians needing supervision. Pre-licensed therapists who prefer a pay-per-code option start at $35-$40 per billable hour. Compensation is based on experience and other factors.

TITLE: Outpatient Mental Health Therapist
PROGRAM: Outpatient Mental Health
Job Summary: To provide outpatient mental health services to the citizens of Winona, Houston, Fillmore, Wabasha, Goodhue and other participating County partners, in a non-profit community mental health clinic setting with other licensed professionals and Center staff including working with individuals, couples, families and the community at large.
JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
Assess and document client's current life situation, including reasons for referral, history of the current mental health problem including important developmental incidents, strengths, and vulnerabilities, current functioning and symptoms, diagnosis using the DSM 5 or other updated tools, and providing needed mental health services.
Develop a written plan of intervention, treatment and services identifying goals and objectives of treatment, treatment strategy, a schedule for accomplishing treatment goals and objectives, and the individual or individuals responsible for providing treatment to the person.
Perform planned and structured treatment (individual, family and/or group psychotherapy) based on client's diagnosis.
Assist in the coordination of therapeutic services with other Center staff, including consulting with on-staff psychiatric prescribers.
Consult in a timely manner with other professionals or agencies concerning clients referred to or from the Center as well as mutual clients.
Participate in the required multi-disciplinary team meetings and clinical supervision.
Provide case or program consultation to health care and human service professionals and agencies.
Participate in efforts to coordinate agency and community resources.
Participate in conferences, seminars, or other educational activities based on the annual staff development plan and as needed to be in good standing with state licensure requirements.
Provide training to in-house staff, share approved and legal video/audio tapes of sessions with clients for intra-agency training or as requested by the Clinical Director.
Perform other outpatient mental health activities such as giving presentations at all-agency meetings or to area businesses, schools, and other organizations as directed by the Director or staffing information booths at events or fairs and participating in activities outside of the agency as directed.
Maintain accurate and current documentation of all client treatment activities in the agency's ECR. Progress Notes are to be completed within 48 hours of seeing a client; treatment plans completed at the first therapy session; diagnostic assessment completed within insurance compliance guidelines or sooner in time sensitive situations and as requested by county or other partner staff.
Maintain accurate and up-to-date case closings and terminations, typically within three months of the most recent session with a client.
Maintain a thorough and accurate schedule in the electronic health record, including all productive and non-productive time and activities.
Maintain productivity time standards - billable and non-billable at the required minimal level, currently 65% of total paid time.
Complete all information/forms needed by the business office (Enrollment form, prior authorizations, requests for information or letters sent to doctors etc.) in a timely manner.
Develop and maintain good working relationships with all referral sources and intra-agency staff.
Possess a vehicle, valid driver license, and valid auto insurance.
