Evolve Treatment Centers is looking for a Primary Therapist to join our team at our PHP/IOP facility in San Jose, CA!
We are looking for a professional who is passionate about working with adolescents to join our team and grow with us. The ideal candidate is flexible, dedicated, knows how to take initiative, maintains appropriate boundaries, and stays calm in stressful situations. Payrate: 85,000 - 95,000 for unlicensed and $100,000 - $105,000 for licensed.
Hours: We ask that candidates be available Monday-Thursday between 9am and 7pm. Your actual workday will be 8 hours within that time frame, and at least two days each week will be scheduled 11am-7pm. Fridays will be 9am - 5pm
Responsibilities and Duties:
Maintain a reasonable caseload (including individual and family sessions)
Complete all necessary documentation on time in EMR
Facilitate weekly therapy groups, including DBT Groups
Attend weekly Treatment Team meetings and present cases to clinical team
Complete Biopsychosocial Assessments upon admission
Coordinate after-care with parents and Discharge Coordinator
Requirements:
LMFT/LCSW/LPCC or associates ACSW/AMFT/APCC
Must be able to pass a background check
Exceptional clinical judgment
Ability to work in a fast-paced, challenging, and dynamic environment

Under the leadership of the department director and/or assistant director the staff physicaltherapist is responsible for supporting the mission of the Hospital by providing quality physical therapy services to patients of all ages who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes. The physicaltherapist interacts and practices in collaboration with a variety of professionals involved in the care of the patient. The physicaltherapist educates, engages in critical inquiry, directs and supervises physical therapy services including support personnel. Physical therapy services include examination, evaluation, diagnosis, prognosis, and interventions in order to reduce the patient's disability. The physicaltherapist also provides prevention and wellness services.
Education
Bachelor of Science in Physical Therapy required. Masters or doctorate degree preferred.
Must pass orientation and initial competency assessment prior to independent assignment of patients requiring the applicable skill, and must complete and pass all annual competencies.
Licensure/Certifications
Physical Therapy Board of California license required.
American Heart Association Healthcare Provider BLS certification required.

Under the leadership of the Director of Physical Therapy and Rehabilitation, the PhysicalTherapist is responsible for delivering high-quality, patient-centered care to individuals referred to the medical group. This role involves collaborating with referring providers to assess patient needs, develop personalized treatment plans, and deliver evidence-based therapy to promote optimal outcomes. The PhysicalTherapist plays a key role in maintaining clinical excellence and community engagement through active participation in outreach and partnerships with local healthcare providers.
The position requires evaluating referrals and patient records, conducting assessments, and documenting progress in accordance with clinical best practices. The therapist will administer appropriate treatments, supervise support staff including PhysicalTherapist Assistants (PTAs), and ensure timely and accurate completion of documentation and billing. Additional responsibilities include contributing to the development of the rehabilitation program, staying current with professional education, and fostering strong relationships with patients, colleagues, and external providers. The PhysicalTherapist is expected to uphold safety standards, maintain confidentiality, and consistently reflect the organization's values in all professional interactions.
Experience
The ideal candidate will have a minimum of two (2) years of experience in an outpatient orthopedic clinic and must be computer literate, with prior experience using Electronic Medical Record (EMR) systems. Familiarity with the Microsoft Office Suite is preferred. Candidates must be highly organized, detail-oriented, and possess strong telephone communication skills. A working knowledge of medical terminology, efficient filing systems, and standard office equipment is essential. The role requires the ability to manage shifting priorities, handle emergency situations, and meet deadlines effectively. Strong language skills are necessary to read and write simple correspondence, document treatment plans and billing codes accurately, and communicate clearly with physicians, patients, vendors, and staff. Additionally, the candidate should possess basic mathematical skills and the ability to follow straightforward written or verbal instructions while applying sound reasoning in routine clinical and administrative situations.
Education
Graduate of accredited physical therapy school.
Licenses/Certifications
CA State Board of Physical Therapy licensure or has applied for CA state licensure. Current CPR/First Aid Certification. ATC, CSCS, SCS, and OCS or like certifications are preferred, but not required.
