Summary:

The Occupational Therapist, School Based serves students with various physical, behavioral, and developmental challenges ranging in age from 3-21 years old. Age specific experience and/or special training and/or expertise is required to serve this population.
RESPONSIBILITIES/ACCOUNTABILITIES:
Implements treatment plans and programs through consistent daily and weekly schedules that focus on functional and measurable outcomes to be shared with the school and/or supervisors.
Initiates new program development with a focus on needed functional outcomes.
Delivers specialized individual and group occupational therapy instruction as mandated in students' Individualized Education Plans (IEPs).
Collects data and completes reports regarding the student's goals and progress.
Consults with other professionals (Teachers, SLP, PT, PCP, etc. ) on the student's treatment team to ensure a cohesive and comprehensive therapy team.
Assesses and documents each student's progress through formal and informal tests and measurements.
Writes and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements in order to remain in compliance with state guidelines.
Provides occupational therapy expertise when developing FBAs/BIPs.
Provides parent and caregiver support and education to discuss the student's progress and strategies for continued support outside of school.
Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domain, Referrals, IEP, and pre-IEP meetings for each individual student.
Ensures a suitable learning environment by setting up and preparing materials and supplies as needed.
Provides screenings and evaluations to determine requirements for skilled occupational therapy intervention relative to the client's level of function. This includes, but is not limited to the following areas: ADL (activities of daily living, including dressing, bathing, grooming, bed mobility, functional transfers, hygiene issues, feeding, school function, home management tasks, etc.), range of motion, strength, positioning, contracture management, conducting home assessments, discharge planning, development of maintenance programs and other related client care issues.

How you'll contribute

Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
Formulates and updates patient specific plan of care for physical therapy
Reports patient progress and barriers to discharge in each patient team conference
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. Performs appropriate documentation.
Demonstrates knowledge, competency and proficiency of physical therapy modalities.

What You Will Do

We have an exciting opportunity for full-time PhysicalTherapists to join our Outpatient Rehab at Christian Health. Our Outpatient Rehab offers expert physical, occupational, and speech therapy services conveniently located on our Wyckoff campus. We offer personalized treatment plans to help the patient move forward safely, confidently, and at their own pace. Our highly skilled interdisciplinary team of professionals are dedicated to a successful recovery and uses state-of-the-art, innovative approaches to help the patient achieve the best outcome. The PhysicalTherapist is responsible to manage the care of the patient, managing quality of all related outcomes. The Therapist participates as an integral part of the interdisciplinary team, manages resources to meet the needs of the patient balancing outcomes to provide effective and efficient treatment. Supervises clinically all assistants and aides of patients to which the Therapist is directing or overseeing the delivery of services according to an established plan of care. Supervision follows all accepted state and federal guidelines for the respective discipline. Demonstrates an understanding of, and embraces, the mission statement of Christian Health.
Competencies:
Provides and directs patient rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
Evaluates patient's needs and establishes effective and efficient treatment plans for the patient assigned.
Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated.
Participates in the screening of all patients according to directed schedules and practices.
Provides feedback to the designated supervisor on all clinical and rehabilitation issues.
Participates in all center directed processes and services.
Shows effective management of treatment and non-treatment related activities.
Participates in in-service training programs.
May travel or float to other CH communities to provide services.
Completes all necessary paperwork for clinical and billing documentation.
Treats patients in accordance with assignment and plan of care.
Participates in center and patient specific meetings as needed.
Assists in the maintenance and cleanliness of the department and equipment.
Provides supervision to assistants and aides/techs.
Completes weekly reports and patient summaries.
Participates in weekly meetings.
Updates and renews patient orders and plan of care as indicated.
Participates in monthly meetings.
Assures complete monthly billing.
Renews patient orders and updates plan of care as indicated.
Participates in quarterly quality improvement initiatives as Directed by DOR.
Participates in quarterly reviews of long-term residents as directed by DOR.
Participates in annual review.
Assists DOR in annual review of other staff as appropriate.
Performs other duties as required.
Qualifications:
Must possess a current, unencumbered license to practice as a licensed PhysicalTherapist in the State of New Jersey.
CPR certification preferred.
Must be able to lift a minimum of 50 pounds without assistance.
Schedule: 8am-4pm, Monday - Friday.
Education:
Graduate of an accredited college or university with a bachelor's degree in physical therapy.
Must possess a current, unencumbered license to practice as a licensed PhysicalTherapist in the State of New Jersey.
CPR certification preferred.
Previous Experience: No previous experience required.

LiveWell Counseling offers evidence-based outpatient mental health services for individuals, couples, and families.
