As an Outreach Counselor, you will:

You’re someone who has:

Pay & Benefits


We offer a wage of $15.50/hour—**
plus a bonus of $1.50/hr for every hour worked until 12/31/22
**- $0.75 wage differential for Medication Administration Program (MAP) certification. We provide paid orientation and training for certification in Medication Administration Program (MAP), CPR & First Aid, and Proactive Approaches to Behavioral Challenges (PABC) if applicable. Description and Responsibilities

Adult Community Clinical Services (ACCS) is a Department of Mental Health (DMH) funded service that is designed to deliver evidence-based interventions within the context of a clinically focused model. Individuals served reside in the community, both in their own homes and in group living environments (GLEs). ACCS Teams are multidisciplinary, and include clinicians, substance abuse counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff.

Outreach Counselors encourage and empower adults with mental health needs to see beyond and live beyond perceived limitations. Outreach Counselors provide individuals served with tools required for success as well as:

* Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with Persons served and family members.
* Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
* Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
* Provide side-by-side support and coaching to help Persons socialize.
* Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation.
* Act as a liaison between Persons served, community agencies, and families to coordinate treatment, including: employment and education entities, Care Coordination Entities, Emergency Service Providers, and outpatient clinics.
* Ensure Persons have adequate financial support through assisting with finding gainful employment or applying for entitlements.
* Implement all prescribed medical and medication protocols in accordance with agency and MAP policies.
* Participate in the On-Call rotation.

Qualifications

* High School Diploma or equivalent. Alternative education/experience in lieu of formal education will be considered.
* Valid Driver's License and acceptable driving record.
* Reliability, willingness to learn, and being open to new opportunities.
