GENERAL DESCRIPTION: Lutheran Social Services of New York's (“LSSNY”) Safe Haven Shelter and Transitional Foster Care Program is a 24-hour program that serves unaccompanied children (UACs) in immigration custody who are awaiting release to their sponsors. The Medical Coordinator is responsible for the oversight, coordination and provision of medical and mental health care of the children in the program including initial medical examinations, screening for infectious diseases, dental screening when applicable, and immunizations.
The Medical Coordinator is classified as an essential worker, and as such accepts responsibility for the completion of their critical services to clients, on site, even during times of emergency. These include, but are not limited to, emergencies caused by weather, nationwide pandemics, civil unrest, and domestic threats.
MAJOR DUTIES AND RESPONSIBILITIES:
• Arrange and coordinate all health services, including urgent, emergent and preventative health services for children in care.
• Coordinate the provision of a complete medical examination, including a screening for infectious diseases, as required by ORR, within 48 hours of arrival.
• Maintain complete, confidential medical records for children, ensuring appropriate communication of health needs to external providers and LSSNY's staff (case managers, clinicians, education team, and child care workers).
• Schedule health care appointments, ensuring qualified interpretation and translation services are in place and coordinate with Child Care Worker teams to ensure transport of minors to providers.
• Serve as a liaison with medical, dental, mental health, and pharmacy providers and document interactions for appropriate reporting to ORR.
• Monitor the provision of medical care for children in UAC program as well as document all follow-up recommendations and services.
• Ensure that all entering female UAC will undergo pregnancy screening and family planning services in accordance with ORR policy.
• Communicate all special diets and medication regimens for UAC to all program staff and foster parents, as applicable.
• Monitor referrals for specialty care, dental care, and mental health services, including ensuring immunization provision according to CDC catch-up schedule.
• Submit all recommended Treatment Authorization Requests to insurance and medical providers in a timely fashion and monitor the system for approval and coordinate any approved follow-up services.
• Obtain and maintain up to date medical, dental and mental health appointments and communications, and prescriptions on all medical records.
• Administer or oversee the self-administration of medication to children.
• Provide billing support to new medical, dental and mental health providers to ensure timely submission of billing and receipt of payment.

Manage educational department activities within rules of the long-term care facility to ensure facility compliance.
Plan, implement, direct, and evaluate the education program based on identified resident needs, staff needs, federal, state, and local regulations.
Coordinate the orientation program for new staff and other programs.
Plan and publish the in-service calendar for all mandatory in-services.
Oversee preceptors of new hires.
Provide regular continuing education programs for nurses.
Conductsan annual competency skills evaluation of nurses in preparation for their performance evaluation.
Act as the liaison between the nursing instructors and affiliated students.
Maintain a resource center with current geriatric literature and other media.
Maintain and submits to HR all training attendance documentation.
Compile necessary reports for DPH and other regulatory agencies.
Supervise student instructors during any resident contact and care.

GENERAL DESCRIPTION:
The Clinician promotes client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation.
The schedule for this position is Sunday-Thursday, 9am-5pm. This schedule is subject to change based on program needs.
The Clinician is classified as an essential worker, and as such, accepts responsibility for the completion of their critical services to clients on site, even during times of emergency. MAJOR DUTIES AND RESPONSIBLITIES:
· Supports and advocates for residents on client caseload (total number to be determined).
· Establishes and maintains therapeutic relationships with clients, maintaining professional boundaries and a non-judgmental, unbiased approach.
· Conducts psychosocial assessments of clients.
· In conjunction with clients, develops individualized service plans to promote client health, well-being and independence.
· Links high risk & geriatric residents (65+) with appropriate providers and resources throughout the continuum of health and human services and care settings, in accordance with residents needs and values.
· Provides preventative monitoring of all high risk & geriatric residents in crisis and decompensation to reduce hospitalizations.
· Serves as clinical liaison for high risk psychiatric hospitalizations via communication and visits.
· Maintains weekly contacts with medical & mental health providers for high risk & geriatric residents.
· Monitors medical appointments and escorts residents to scheduled and unscheduled psychiatric/medical care.
· Provides medication education and monitoring to high risk & geriatric residents in need of additional support.
· Assists residents and their families/caregivers in navigating the elder care system to meet their needs.
· Ensures Advance Directives, Healthcare Proxies, and other planning documents are in place to help aging adults maintain their independence and age in place within a supportive setting.
· Advocates for safe, effective, client-centered, timely, efficient, and equitable geriatric care to promote aging in place.
· Organizes, facilitates, and escorts residents as needed for on/off-site activities.
· Manages two or more ongoing groups; tracks client participation.
· Conducts monthly unit inspections and ensures proper follow-up to findings, including assisting residents as needed.
Maintains the list of high-risk residents, updating the list weekly.
Conducts intake interviews for new residents, completing assessments and paperwork in line with program and funder standards.
· Maintains client records as audit-ready at all times as required by program funding sources. This includes, but is not limited to, recording progress notes, constructing service plans, updating medical and mental health information, and completing psychosocial assessments.
· Handles other job-related duties as assigned by supervisor.

GENERAL DESCRIPTION:
The Resident Aide/Intake Coordinator supports client wellbeing by welcoming clients and providing a detailed assessment for any economic, social, vocational and/or mental or medical health barriers, and other appropriate services. The Resident Aide/Intake Coordinator assists with clients' basic needs while they are at the Emergency Shelter facility.
This is a 24-hour facility. The Resident Aide/Intake Coordinator is classified as an essential worker, and as such, they accept responsibility for the completion of their critical services to clients, on site, even during times of emergency. These include, but are not limited to, emergencies caused by weather, nationwide pandemics, civil unrest, and domestic threat.
MAJOR DUTIES AND RESPONSIBLITIES:
• Interviews and conducts assessments for incoming clients.
• Provides a facility tour and welcome packet.
• Overviews expectations for clients' code of conduct during their stay at the Emergency Shelter facility.
• Assists in the provision of diligent case management.
• Encourages self-sufficiency and independence while simultaneously providing support, as needed.
• Documents assessments and other service contacts in client record, according to funder requirements.
• Schedules meetings and has ongoing communication with members of the social service team.
