We are seeking to hire a Human Resources Coordinator.
$23 to $26 per hour based on skills and experience.
9am to 5pm; Monday - Friday.
Human Resources Coordinator Introduction:
The Human Resources Coordinator functions under the direction of the Director of Human Resources to provide support in administration of HR functions such as employee/labor relations, payroll, timekeeping, onboarding, recruitment, documents update, benefits, workers compensation, and other leaves.
Human Resources Coordinator Qualifications:
Associates degree in human resources or related field preferred.
Minimum of 2 years of experience in Human Resources in a Skilled Nursing Facility required.
Demonstrated intermediate skill with MS Office products (Excel, World and PowerPoint), web-based database and recruiting software.
Human Resources Coordinator Job Summary:
Ensure that all employee files are updated regularly and contain copies of all Federal, State, Local, and internal forms.
Conduct periodic employee files and other data audits.
Assist in payroll preparation by ensuring employee data is updated correctly.
Review exceptions report for lateness and missed timeclock punches and prepare documentation for disciplinary actions.
Be the front-line face for daily employee support in answering general HR questions.
Ensure that all New Hires have the necessary computer access, work email, etc.
In cooperation with the HR Department, manage the recruiting and onboarding function.
Assist with the Worker Compensation claims and other leaves filling process.

OPERATIONAL READINESS
· Knowledge of and remain current with federal, state, local laws and regulations, including health care professionals practice act requirements. · Assure that the clinic is in compliance with all applicable federal, state, and local laws and regulations and receives continuing certification from all statutory and regulatory agencies. · Develop, implement and follow up necessary Corrective Action Plans for internal and external surveys. · May assume Charge Nurse's responsibilities as needed. · Responsible for duties listed in Registered Supervising Nurse job description and nursing services policy # C-AD-0110 or must designate Registered Nurse meeting these qualifications. · May fulfill responsibility of facility CEO as delegated by Governing Body. · Conduct and/or participate in both formal and informal meetings with the governing body, Regional Director, Medical Director and the staff. Assure that appropriate staff meetings, in-service education meetings, and team patient care planning meetings are held monthly. OPERATIONAL READINESS (cont.) · Continuously monitor to ensure that a safe and sanitary physical environment is maintained throughout the facility; that all equipment is maintained and functions properly; and that adequate and appropriate inventory levels of all supplies are available and used correctly. · Supervise the maintenance of equipment, building areas occupied by the center and other property belonging to the center. · Know and understand the water treatment and mechanisms of the equipment of the facility. · Assist, when necessary, with disinfection of equipment and supplies including bicarbonate delivery systems and dialysis machines.
PARTNERSHIPS · Monitor all contractual agreements; update as needed with corporate oversight. · Maintain collaborative working relationship with Medical Director and physicians. · Establish and maintain a positive relationship with area hospitals, agencies, vendors and the community. · Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization. · Respond effectively to inquiries or complaints.
STAFF DEVELOPMENT/ RETENTION · Ensure all staff meet required qualifications for position held and perform duties within limitations established by and in accordance with company policy/procedures, health care professionals practice acts, applicable state and federal laws and regulations. · Recruit, train, develop, and supervise all personnel. · Maintain effective personnel management and employee relations, including evaluating the performance of all personnel; approving and submitting all hours worked and counseling and disciplining employees. · Uphold management goals of corporation by leading staff in team concepts and promoting a team effort. · Effectively communicates expectations; accepts accountability and holds others accountable for performance.
Qualifications/Requirements:

1 year previous dialysis management experience preferred.

• Demonstrates and reviews specific procedures with RNs and LPNs, individually and as a group, as needed. What You Will Do
Provides input to Education team regarding gaps in educational materials used by the Assessment Unit to educate internal staff and external vendors regarding the types of customers appropriate for MLTC/MLTC MAP.

Reviews and monitors processing of new referrals, development of treatment plans for new enrollees and the documentation of prescribed services to ensure all pre and post enrollment activities are in accordance with VNS Health Plans policy, regulatory requirements and quality standards.
Participates in the preparation of summary reports of enrollments, deletions, withdrawals, deferrals, CASA conversions, etc. required by HRA and DOH. Acts as a liaison between MLTC/MLTC MAP Plans, HRA and responds to requests for clarification or information regarding assessment documentation.
Participates in case conferences with Medicaid Eligibility Unit, Assessment Unit, Utilization Management, Grievance and Appeals, Vendors and Care Management to resolve concerns that may hinder the assessment from being promoted to enrollment.
Conducts marketing activities to potential new members, physicians and community resources designed to increase member participation.
Participates in the review of clinically complex cases to ensure potential new members receive appropriate services based on clinical guidelines, evidenced base practices and medical necessity. Reviews and submits required documents on clinically complex cases to Medical Director so that a review and timely decision is made.
Works with Medicaid Eligibility Unit to ensure all viable cases are scheduled timely.

Strategic & Operational Leadership

What you'll do:
Serve as the strategic lead and executive counterpart for enterprise deployments, working side-by-side with CHROs and business leaders.

Role Description:
Oak Street Health's graduate Behavioral Health Specialist Fellowship is a 12-month paid training program in the revolutionary collaborative care model in a value-based setting, which rewards healthcare companies for the quality of their care not the quantity.
This program will include time spent building your knowledge of evidence-based interventions and psychotropic medications, earning clinical supervision hours from a licensed clinical social worker, and engaging in didactic content that aims to build on, not repeat, graduate training. All fellows will work at Oak Street for 24 months following the completion of this program, but we hope will continue to spend their careers at Oak Street improving the health of communities across the country.

Selection Timeline
First-round interviews will be conducted as applications are received.
