What You'll Do:
Complete the Integrated Biopsychosocial Assessment and complete intake sessions with clients.
Formulate a mental health diagnosis by the current Diagnostic and Statistical Manual of Mental Disorders V (DSM V) standards.
Develop an initial treatment plan, incorporating standard practice guidelines for each diagnosis.
Review and update the treatment plan with the client and others involved in the individual's care as appropriate. Measures change and adjusts treatment as needed.
Document assessments and care provided in the client's Electronic Health Record (EHR) and complete all documentation as required.
Complete encounters and associated documentation according to YVFWC requirements and coding and billing rules.
Respond to crisis situations with assigned individuals, assesses urgency of individual's needs and help obtain appropriate services as needed.
Assist individuals and families in obtaining additional mental health services and other social resources as needed. Document all referrals made to YVFWC case manager or other resources.
Work with individuals to transition and maintain a presence in a community setting after psychiatric hospitalizations.
Present mental health findings in verbal or written format to appropriate professionals. Provides case presentations for peer review.
Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Perform other duties as assigned.
Who We're Looking For:
A licensed therapist who has direct experience treating children or adolescents
A team-focused individual who can engage with all levels of an interdisciplinary team
A therapist who can handle complex, multi-service treatment plans that may require expertise in evidence-based practices
Bilingual (English/Spanish) is strongly preferred but not required
Qualifications:
Master's Degree in Social Work, Psychology, Counseling, or related field.
Basic Life Support (BLS) certification within 45 days of hire.

Position Summary:
Full-time Community Health Worker position available for the Maternity Support Services (MSS) Program in University Place, WA. This position requires flexibility, self-initiative, and the ability to manage time, resources, and client caseload efficiently. Applicant must have access to an insured vehicle, and be able to make home visits. Computer experience needed and database entry skills preferred. Under the direction of the program manager, works closely with all MSS team members scheduling appointments, maintaining client's charts, enrolling clients into the program, dealing with phone calls and questions about the program. May follow up on clients in their homes and deliver basic health messages, and ensure that utilization of medical care and other community resources are met.
Duties and Responsibilities:
Teaches basic principles of self-care including appropriate utilization of available pre- and post-natal care, thus reducing negative birth outcomes.
Connect clients to Sea Mar and non-Sea Mar resources as appropriate and track all resources
available to clients. These services may include but are not limited to insurance enrollment, preventive health services, behavioral health, dental and care management.
Provide appropriate provider and/or resource referrals to the client to help promote well-being
within the family.
Enrolls clients in the First Steps program and enters client information into database.
Monitor and document clients' ability to keep regularly scheduled medical and social service
appointments, thus ensuring greater client use of appropriate services.
Qualifications and/or Education:
High School diploma or GED required.
One year of health and/or social services experience working with the childbearing family required.
This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.

The Connected Care Community Health Worker provides information, resources, and care coordination to help older adult patients and their families access the care they want and need. They work closely with patients primary care and behavioral health providers, communicating key information about what matters most to patients. Responsibilities include assisting patients with completing Advance Directives, reconciling medications, identifying and removing fall hazards in the home, providing self-management support and education for chronic conditions, and connecting patients with community resources. This is a pilot program, grant-funded through 2026 and a full-time position based in White Salmon, WA, with the possibility of working in Stevenson, WA, depending on patient needs. Paid on-the-job training is provided for candidates not yet certified as a Community Health Worker (CHW). Certification can take up to six months, after which employees will receive CHW certification and an increase in hourly pay.
Minimum Qualifications:
High School Diploma or GED.
2 years experience in a healthcare or social service-related field.
Valid driver's license and clean driving record.
Willingness to travel for events, paitent care, and between clinic locations, as needed
Experience giving presentations and provide instruction to patients and groups.
Familiar with medical terminology and the healthcare system including the Oregon Health Plan and Medicare.

PRIMARY ACCOUNTABILITY: Integral part of the behavioral health / primary care team providing quality, comprehensive care, by providing accurate, effective interpretation between the patient and behavioral health specialist or primary care provider. Assists with other patient care coordination activities as directed by the supervisor.
PRIMARY DUTIES AND RESPONSIBILITIES
Interprets for patients who prefer communication in Spanish and assists the patient to communicate with the behavioral health specialists, primary care providers, and other FHC staff during the patient visit
Coordinates and assures access to identified areas of care necessary for improved patient health
Assists patients by scheduling appointments, assists in generating referrals and obtaining prior authorizations for urgent needs
Assists patients with coverage by providing access to insurance enrollment assistance
Communicates health information to patients, support staff, and team members
Answers patient phone calls, documents and communicates processes in Electronic Health Records (EHR)
Relays appropriate information to the patient and medical team via face to face, phone, mail or documentation in EHR
Assists care team with patient education and support groups
Participates in co-visits with providers under provider direction and conforming to approved patient education curriculum
Monitors and processes voicemail, EHR messaging and email in a timely manner
Assists PCPs/ BH Specialists with patient visits
Escorts patients to consult rooms
Obtains and documents all vital signs
Administers SBIRT and other relevant screeners
Collects and enters screener results and meaningful use data into HER
Conducts “Fond Farewell” visits with patient to assure that all possible tasks are completed the day of visit as the patient exits
Schedules “Next step” encounters or ticklers

PROFESSIONAL & TECHNICAL KNOWLEDGE
Possesses a basic level of written and verbal communications skills, computational and computer skills and mathematical knowledge typically acquired through completion of a high school program. Bilingual English/Spanish communication, written and verbal required.
TECHNICAL SKILLS
Current MA-R (Internal transfer) or obtained within 60 days on the job
Experience interpreting for patients who have co-occurring mental health, substance abuse, and physical health problems.
Experience with interpreting for behavioral health workers during brief, structured counseling techniques such as motivational interviewing.
Requires a high level of professionalism and patient confidentiality.
Experience working with underserved, transient populations
Fully functional in use of the Electronic Health Records (EHR) program
Ability to access and complete data entry in the Electronic Practice Management (EPM) program
Ability to prepare basic correspondence and simple reports using computer
Ability to create, send and manage email
Ability to access web-based applications and other computer programs including telehealth
WORK ENVIRONMENT
Work is performed in an ambulatory care setting, which may include the requirement of working at other sites. Must have reliable transportation and provide documentation of auto insurance and valid driver's license as required.

Day to Day:
Assist clients in obtaining health services, addressing barriers that may prevent access to care. This includes coaching in the use of technology to access patient portals and telehealth for medical, behavioral health, and social service care.
Assist clients in obtaining Smart Phones or other devices available through state/federal programs, including Managed Care Organizations, so they can utilize telehealth for health and social services.
Assist clients in scheduling virtual visits for medical or behavioral health, setting up their smart device for virtual visits, and coaching them to confidently be able to conduct virtual visits independently. This may involve the CHW accompanying the patient/client at the beginning of several visits to successfully start each visit in a confidential area and then leave the patient in a confidential space for the visit, still being available for technical help if needed.
Assist clients in accessing health care information via patient portals to obtain their own health information, request medication refills, and communicate with their health care team using technology from their home or dwelling unit.
Provide assessments and screening for Social Determinants using approved tools.
Assist clients address other barriers to care i.e. Language, transportation, insurance needs, etc.
Assist clients address emergent needs i.e. food, shelter, clothing, etc.
Provide health education, as directed, using appropriate health aids.
Attend case conferences or morning huddles as needed to address the comprehensive needs of the client.
Client contacts may be conducted in the clinic, client dwellings, streets, or other outdoor settings
Document contacts and interactions in Electronic Health Records.
Participate in quality assurance activities and peer review as requested.
Maintain organized, clean, efficient and confidential work area.
Represent YNHS in local meetings as requested.

Community Health Worker (CHW)- Connected Care

The Connected Care Community Health Worker provides information, resources, and care coordination to help older adult patients and their families access the care they want and need. They work closely with patients primary care and behavioral health providers, communicating key information about what matters most to patients. Responsibilities include assisting patients with completing Advance Directives, reconciling medications, identifying and removing fall hazards in the home, providing self-management support and education for chronic conditions, and connecting patients with community resources. The role also includes traveling to patients homes to provide care and support.
This is a pilot program, grant-funded through 2026 and a full-time position based in White Salmon, WA, with the possibility of working in Stevenson, WA, depending on patient needs. Paid on-the-job training is provided for candidates not yet certified as a Community Health Worker (CHW). Certification can take up to six months, after which employees will receive CHW certification and an increase in hourly pay.
Minimum Qualifications:
* High School Diploma or GED.
* 2 years experience in a healthcare or social service-related field.
* Valid driver's license and clean driving record.
* Willingness to travel for events, paitent care, and between clinic locations, as needed
* Experience giving presentations and provide instruction to patients and groups.
* Familiar with medical terminology and the healthcare system including the Oregon Health Plan and Medicare.
* Knowledge of community resources in OCH services areas, especially those available to older adults.
* High-level computer skills with Microsoft Office applications, Word, Excel, Outlook, Epic, and other systems.
* Knowledge of and compliance with HIPAA regulations.
* Work independently, work with multiple projects and maintain a high degree of professionalism and confidentiality.
Preferred Qualifications:
* Fluent Bilingual in English/Spanish.
* Advanced education such as graduate of an accredited Medical Assisting program.
* 90-hour Community Health Worker certification or Certified Community Health Worker
