What You'll Do:
Provide on-site behavioral health services in primary care clinic.
Provide consultation with Providers to aid or assist in the primary care of patients.
Serve as primary mental health Provider or ancillary health Provider as needed.
Assist care team with ICD-11 diagnoses, adding mental health differential diagnoses and contextual and relational perspectives.
Work across disciplines to provide equitable biopsychosocial-spiritual care, intervening in support of team and patient goals.
Use culturally informed and evidence-based literature for shared decision-making around health behaviors and assist with navigating the healthcare system.
Work in a closely organized, interdisciplinary team on treatment plans, patient advocacy, and clinic processes.
Conduct individual and group training for staff in areas of expertise and to community organizations as requested by supervisor.
Develop research-related funding proposals.
Partner with other Providers to triage referrals.
Provide coverage and backup for other Providers.
May assign patients to team clinicians.
Perform other duties as assigned.
Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), American Psychological Association (APA) and YVFWC requirements.
Represent the program at meetings as requested by Behavioral Health or clinic leadership.
Participate in the development of new programming and projects related to Behavioral Health
Qualifications
Clinical Psychologist Requirements:
Education: Doctor of Philosophy (Ph.D.) or Doctor of Psychology (Psy.D.) in Clinical Psychology.
Experience Preferred: One year's clinical experience working within a multidisciplinary team in primary care or other medical settings, providing diagnostic and psychological assessments for primary care-based positions. One year's clinical experience working with children, adolescents, and families in specialty behavioral health settings.
Licenses/Registration: Licensed Psychologist within the state of practice. Basic Life Support (BLS) certification within 45 days of hire.
Master's Level Therapist Requirements
Education: Master's Degree in Social Work, Psychology, Counseling, or related field.
Experience Required: Two years of postgraduate/master's experience in the direct treatment of persons with mental illness or emotional disturbance; such experience must have been gained under the supervision of a mental health professional. Washington State designation as a Mental Health Professional (MHP) may be substituted for this experience requirement.
Licenses/Certificates/Registration: Basic Life Support (BLS) certification within 45 days of hire. One of the following licenses is required for this position (must apply for within one week of hire if the license is not current):
Licensed Independent Clinical Social Worker (LICSW)
Licensed Mental Health Counselor (LMHC)

Position Summary:
NeighborHealth South End Primary Care is seeking a dedicated CommunityHealthWorker (CHW) to join our Primary Care Department. As an integral member of our care team, the CHW will initiate, build, and sustain relationships that promote the health and well-being of individuals and families in underserved communities. Utilizing various outreach methods, the CHW will connect healthcare, social services, and community resources, providing culturally appropriate health education and support. This role requires flexibility, as outreach methods may include in-person, phone, or telehealth visits in various settings, including the community, homes, facilities, or health centers.
Key Responsibilities:
Conduct outreach calls to engage enrollees and caregivers in care management programs.
Collaborate with team members to develop and implement tailored outreach plans.
Identify and address barriers to healthcare access in homes and communities.
Provide information, referrals, and resources to meet clients' needs.
Schedule and accompany enrollees to provider visits and community services.
Build trusting relationships with clients, families, and community agencies.
Act as a liaison between enrollees and community service organizations.
Conduct follow-ups, motivational interviewing, and goal setting with clients.
Ensure culturally competent services and support diverse populations.
Assist with access to community resources, Medicaid applications, and benefits.
Qualifications:
Education: Bachelor's Degree preferred or HS Diploma with equivalent experience (3+ years) in public health or community engagement.
Experience: Clinical or special services setting experience with Medicare, Medicaid, or Special Needs populations. Certifications (CHW, Engagement Specialist, Medical Assistant) preferred.
Transportation: Valid Driver's license and reliable transportation.

Central to this responsibility is the engagement of members in all aspects of clubhouse operation. This engagement is performed by helping members to experience the sense of belonging, with a focus on their strengths, talents and interests.
This position exists to assist King County adults with serious mental illness to maximize their optimal level of functioning within the community.
The Clubhouse Rehabilitation Associate is responsible to and is supervised by the Program Director of Seattle Clubhouse.
Primary Job Functions
Offer a positive supportive relationship that helps promote hope, choice, wellness and recovery for participants while working side by side on the work of the clubhouse.
Work side by side with the members of Seattle Clubhouse in program areas of work ordered day (
within assigned unit such as Hospitality (kitchen) or Business
), social/recreational, education, employment (
Transitional and Supported
), advocacy, and member supports
Consistently fulfills the "primary responsibility" of helping members experience being needed and actively reaches out to each member to establish productive partnership
Facilitate assigned tasks and projects in a timely manner maximizing members skills, talents, engagement and leadership
Ensure that performance of job duties actively reflects and promotes the Standards for Clubhouse Model of Psychiatric Rehabilitation as developed by the Clubhouse International
Work in an assigned work unit (Hospitality/Kitchen or Business/Clerical)
Train, job coach and work with members at their place of employment
Work evenings and weekends
Provide education and resources that support illness self-management, self-determination, self-advocacy, and shared-health-care decision making
Ensure all documentation is complete, accurate and performed within time frames as specified in Seattle Clubhouse procedures
Provide assistance in identifying and linking to community-based services and supports (primary care, mental health and or substance abuse treatment, housing, financial benefits, etc.)
Provide and model advocacy to ensure access to service for the member where/when needed.
Help with identifying and/or re-engaging with family, friends, and other natural supports
Assistance with transportation to appointments, employment, social outings, etc.
Keep supervisor informed on a timely basis of member routine/emergency/crisis needs developing, the proposed or actual response to these needs and/or barriers to, or gaps in needed services

Education/ Training/ Experience
Must have Washington State Drivers License
Must pass a criminal history background check
Must have a Bachelors Degree
Must have basic computer skills including working knowledge of Microsoft Word
Prefer prior training in Trauma Informed Care
Must be (or able to become) a WA State Registered Counselor
Associate I Requirements
0-2 years of Clubhouse experience required
Associate II Requirements
2-5 years of Clubhouse experience required
Successful completion of Colleague Training
