Job Summary:
Provide care services and administer specific nursing procedures, supportive therapy services and emotional support to patients, caregivers and family. The primary nurse or therapist on a case provides the direct supervision of the patient care provided by the HHA. Home health aide services are provided according to the home health or hospice plan of care, according to department policy and scope of practice guidelines as set by applicable regulations or state law. Primary Duties:
Aide performs personal care services, provides assistance with routine household tasks, and respite or companion visits for patients and their families.
Assists patients with activities of daily living, i.e., bathing, dressing, feeding, transfers, toileting and ambulation as directed on home health aide plan of care.
Observes, assesses and reports changes in patients within scope of practice.

Job Summary:

2. Analyzes patient's general body configuration to analyze the most acceptable examination approach
3. Assists with introduction or placement and the application of various ultrasound transducers.
4. Assists with introduction or placement of intra cavity devices, as well as needles designed for biopsy or amniocentesis.
5. Provides portable examinations at bedside as requested by attending physicians and processes the documenting video film.
6. Performs high-level disinfection on transducers as appropriate.7. Performs other duties as assigned. License, Certification, Education or Experience:

Job Summary:
Responsible for the total direct nursing care of all patients within established standards of care, hospice, hospital and unit policies, procedures, and guidelines.

Job Summary:
Responsible for applying prescribed ionizing radiation in performing CT procedures at a technical level for diagnostic purpose under the prescription of a physician.
Primary Duties
Performs complex CT scan examinations and procedures, including injecting contrast material under the direct supervision of a Radiologist.
Sets, adjusts and controls the CT scanner equipment to apply ionizing radiation for CT procedures. Job Summary: EvergreenHealth's Heart Care Clinic is looking for an experienced RN to join our well-established team. This position will provide nursing care through face-to-face or phone, by process of analyzing information delivered by the patient to sort out the need of appropriate care related to the severity of the concerns and symptoms represented. Provides patient education as needed for better care and compliance.Ensures the direct nursing care of patients is conducted within established standards of care, hospital and unit policies, procedures, and guidelines. May assist in the developing of case management protocols. Primary Duties:
Manages the clinic flow.
Provides continuity of care between the clinic, hospital, ER, specialists, Home Health, Hospice and LTC facilities.
Triages patients in person and via phone
Assesses the comprehensive nursing needs and develops a nursing diagnosis
Participates with the providers to develop and implement the plan of care
Analyzes diagnostic / lab test results and relays that information to the providers
Coordinates critical results in a timely manner
Provides oversight of medical assistants to ensure patient care is provided within established guidelines and evaluates the outcomes of care.
May assist with the Medical Home pilot by attending collaborative meetings, preparing month end reports, and working toward case management of patients in conjunction with the nurse case manager.
Performs nursing visits within scope of practice and assists with developing group visits with manager and providers.
Provides education and training for medical assistants in collaboration with the manager and/or RN Quality Manager.
Balances workloads and adjusts staffing in various pods to adequately cover providers.
Provides safe, family centered nursing patient care and serves as a consultant to the patients and caregivers
Performs other duties as assigned.
REQUIRED for the position:
Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required)
Licensed as a Registered Nurse in the State of Washington
3 years of previous RN experience
Chronic care management and patient education skills
Ability to manage a variety of concurrent programs and projects effectively while Utilizing time, equipment and supplies appropriately
Current Healthcare Provider BLS certification by start date
Current Healthcare Provider ACLS certification by start date
DESIRED for the position:

Job Summary:
Responsible for applying prescribed ionizing radiation in performing CT procedures at a technical level for diagnostic purpose under the prescription of a physician.Primary Duties
Performs complex CT scan examinations and procedures, including injecting contrast material under the direct supervision of a Radiologist.
Sets, adjusts and controls the CT scanner equipment to apply ionizing radiation for CT procedures. Measures and selects proper electronics and technical factors such as kilovolts, milliamperes, slices and exposure time. Consistently produces high quality CT scans, develops images and interprets technical quality of images as related to quality assurance standards.
Analyzes patient's general body size, physical condition and age to accurately perform CT examinations.
Communicates to patient constantly, in lay language, describing the procedure, what the patient can expect from the procedure, and the status of the procedure, always maintaining patient's confidence. Demonstrates respect for patient's dignity and privacy.
Handles incoming and interoffice telephone calls and inquiries.
Greets patients, obtains history from patient, starts IV when appropriate and readies the patient for scan.
Obtains lab work and other necessary patient information from Cerner.
Schedules CT Scan exams from the Cerner imaging queue, ensuring all paperwork is complete and accurate.
Updates patient status in the RIS to indicate exam completion.
Performs other duties as assigned
REQUIRED for the position:
• ARRT, or registry eligible, must take registry within first 45 days of employment.
• Current Healthcare Provider BLS certification by date of hire
• Training specific to the operation and performance of CT scanners.
• Current WA State Radiologic Technologist licensure
• Registered in CT Technology with ARRT within one year of employment or if hired on or before December 31, 2017 must obtain by January 1, 2019

Job Summary:

Primary Duties:

b) OB Ultrasound (First and 2nd Trimester OB, D & E Procedures, and amniocentesis)
c) Vascular Ultrasound (Abdominal Vasculature, Venous Extremities, Arterial Extremities, Carotid Doppler, Renal Arterial Doppler, TIPS, Grafts, and ABI/Toe Pressures).
2. Analyzes patient's general body configuration to analyze the most acceptable examination approach
3. Assists with introduction or placement and the application of various ultrasound transducers.
4. Assists with introduction or placement of intra cavity devices, as well as needles designed for biopsy or amniocentesis.
5. Provides portable examinations at bedside as requested by attending physicians and processes the documenting video film.
6. Performs high-level disinfection on transducers as appropriate.7. Performs other duties as assigned. License, Certification, Education or Experience:
REQUIRED for the position:
• ARDMS-eligible by date of hire. ARDMS-certified in Abdomen and Vascular Certification (RVT) within one year of employment.
• OB/GYN at a minimum by two years of employment.
• Graduate of an Accredited Ultrasound Program.
