Clinical Pharmacist (Clinical Programs) - Remote EST Position Purpose: Support Pharmacy Quality & Population Health Clinical programs with a focus on improving Medicaid QARR scores and collaborative Case Management programs to improve member outcomes. Conduct Quality Measure outreach campaigns for HEDIS measures such as ADD, adherence outreaches for MAT medications, Antidepressant, Antipsychotic medications, or confirmation of lab work. Perform comprehensive Medication Reconciliation reviews to identify and recommend targeted clinical interventions in collaboration with various Case Management team for Populations Health Measures such as High-Risk Diabetes, Hypertension or cardiovascular disease states. Collaborate with case managers during clinical integrated rounds workups to identify and communicate clinical recommendations to providers for members within various lines of business including Children's, Foster Care, and High-Risk members to assist with management and improve quality of life. Desired Skills (Combination of and/or): Managed Care - Medicaid Case Management Quality medicine reconciliation Specialty Pharmacy Excel - Data Analysis Experience communicating clinical recommendations with health care providers, including specialists. Working shift: Starting would be Monday-Friday 9am -5:30pm (EST) with possibility to shift into a 4-day work week. Education/Experience: Bachelor's degree or advanced degree (Pharm.D., M.S.) in pharmacy. 2+ years of mail order, retail, hospital or managed care pharmacy experience or 1+ years of pharmacy residency program experience. Qualifications This role is located onsite and requires availability for our 6:00am - 2:30pm CST shift. Under the direct supervision of the Pharmacy Manager the Staff Pharmacist is responsible and accountable for the provision of safe effective and prompt medication therapy. The staff Pharmacist participates in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas.
