The contractor is responsible for providing appropriate supervision, mentoring and teaching opportunities so that adults may gain greater independence, increase their personal repertoire of leisure skills, and build meaningful relationships with community members. The contractor helps to ensure activities are provided in a healthy and safe environment. Responsibilities Program Operations Provide direct support and supervision during travel events. Assist and support individuals to ensure participation in the activities. Break down tasks or parts of an activity into manageable components so that people of all abilities can participate to the best of their ability. Assist with daily living skills including meal preparation and eating. Assist adults with a variety of aspects of personal care, include toileting, grooming and ambulating. Ensure the activities and the health and safety needs of the individuals are consistently met. Administer medications as prescribed and document administration. Display an awareness of individuals needing physical assistance with ambulation. Recognize the needs and desires of the individual and provide appropriate support. We are looking for a PRN Van Driver Job Duties and Responsibilities: Transport Driver Reviews patient information prior to appointment in order to determine patient needs. Verifies all information with nursing staff as necessary. Implements customer-service standards, resolves problems, and determines & completes patient transportation system improvements. Transport Driver Stays with patient at appointments to provide assistance upon request. Enters orders for appointments and transportation and retrieves/sends diagnostic labs needed for appointment. Determines best route to and from appointments prior to the departing time, along with equipment required to load and unload patient.
