You will:
Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.
Be responsible for handling patient appointments and performing routing examinations to ensure positive patient health outcomes

Required Qualifications:
A minimum of one year of experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.
Active Family Nurse practitioner license in good standing
Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.
Order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.
Minimum of two years of medically relevant experience or equivalent
Complete the DOT certification exam within 30 days of employment
Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish
Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).
Pass a FIT test and wear the provided respirator mask when protocol requires.
Education:
Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role
Or in select states, Master of Science in Physician Assistant Studies (or other health-related master's degree or equivalent experience together with Bachelor of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role.

The typical pay range for this role is:
$50.22 - $224,952.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

YOUR ROLE
As a Thyme Care Palliative Care Nurse Practitioner, you will be a critical member of the clinical team caring for our members with serious illness. You will have three primary responsibilities:
First, you will see members for specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team consisting of our Community Health Workers, Oncology Nurses, Social Worker, and Palliative Medicine Physician. As one of the first Nurse Practitioners to join the palliative care team, you will be an important part of bringing our model to scale.
Second, you will be responsible for the oversight and direction of interdisciplinary case conferences for our members with advanced serious illness, in which individualized care plans for our members are created and monitored by our team of Community Health Workers and Oncology Nurses. You will provide clinical oversight to the case management team and direct clinical care, as needed.
Third, you will provide education about palliative care to our team of Community Health Workers and Oncology Nurses. In particular, you will focus your teaching efforts on the outpatient management of cancer-related symptoms, serious illness communication skills, and advance care planning. In addition, you will be integral to the development and implementation of our advance care planning program.
This role reports into our Senior Medical Director and can be remote or hybrid based in our New York City or Nashville offices. All patient interactions will be virtual via telephone, video, text, or our proprietary virtual care platform.
Within your first three months, you will:
Complete training and are up to speed on Thyme Care's systems, tools, technology, partners, and clinical expectations.
Complete virtual palliative care outpatient visits for our members with the highest degree of clinical rigor. You will work fluidly with our in-house care team and providers to ensure any identified needs are met in follow-up to patient visits.
Be adept at leading interdisciplinary case conferences for members with advanced serious illness, guiding our team of community health workers and oncology nurse navigators in the creation of evidence-based, member-centered care plans that focus on interventions that improve member outcomes.
Become the go-to person for our oncology nurse navigators and community health workers when they are working with a member who has advanced serious illness.
Be comfortable with and correctly follow policies and procedures, escalation pathways, communications best practices, and documentation standards. Your ability to effectively engage and support our members is reflected in our metrics and quality standards.

Experience. You have at least 3 years of nurse practitioner (NP) experience with at least 2 years in palliative care, preferably caring for patients in the outpatient oncology setting. You are an advanced certified hospice and palliative nurse practitioner (ACHPN). You have an unrestricted nurse practitioner license and a willingness to obtain additional state licenses, as needed. It would be exceptional if you have worked at a startup or tech-forward company.

Community of Hope is seeking a Family Nurse Practitioner to provide comprehensive, high-quality primary and preventive healthcare services to individuals and families across the lifespan. You'll work closely with our physicians, nurses and other care team members, providing team-based clinical care to patients, emphasizing health promotion, disease prevention, and management of chronic and acute conditions. This position is located at our Family Health and Birth Center in NE, Washington, DC.

What You'll Do
Provides direct, team based clinical care to patients, meeting clinical standards, within the appropriate scope of practice.
Provides virtual medical care to patients, following approved guidelines and refers them for in-person visits as needed.
Refers patients to specialists as needed. Refers patients for care coordination as needed.
Cooperates with clinical and office policies and protocols.
Maintains the required productivity numbers expected by Community of Hope, as set by the board.
Precepts residents, medical students, nurse provider students, midwifery students, and physician assistant students during their community medicine rotations as agreed upon with supervisor.
Maintains safe, secure, and healthy work environment by following standards, procedures, and legal guidelines.
Signs off on all laboratory results, diagnostic imaging results, and referral reports in a timely fashion; sharing this information with patients as appropriate.
Requirements
Must-Haves
Completion of a Family Nurse Practitioner program of study.
National Certification as Family Nurse Practitioner.
Licensed to practice in the District of Columbia required. Any providers who are licensed in Maryland who provide a copy of their license and MD CDS to Talent Management are also eligible for MD licensure reimbursement.
BLS certification. (ACLS or BLS for the Healthcare Provider or equivalent).
Able to work across clinical sites as needed.
Shares Saturday clinic coverage with other providers by completing at least one Saturday shift per month.
Ability to work evening hours as needed.
Proof of vaccinations is required. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
Nice-to-Haves
Bilingual in Spanish and/or Amharic.
Familiarity with eClinicalWorks.
Commitment to work with underserved populations in an urban setting.
