You will work with autonomy, using Total Care Connect's electronic patient care record system, following physician-led protocols. Key Responsibilities Perform patient assessments in the home environment: vital signs, health history, home safety, risk factors, etc. Provide in-home follow-up care post-hospitalization or after urgent care visits to prevent complications and readmissions. Deliver patient education on disease management, medication adherence, lifestyle improvements, and social determinants of health (e.g. nutrition, housing, etc.). Work under clinical protocols to treat/manage health issues, escalate when necessary, consult virtually with advanced providers (NPs, RNs, physicians). Coordinate care: schedule or facilitate follow-ups, referrals, community resources, and linkages with other health or social services. Document all care, assessments, and patient encounters thoroughly, using the organization's mobile electronic patient care record (ePCR) system. Participate in internal meetings, trainings, and continuous quality improvement activities. Maintain all required licenses, certifications, and compliance with relevant regulations. Qualifications Valid Paramedic certification/license in the applicable state, in good standing. Minimum of 2-5 years paramedic / prehospital experience; experience in community paramedicine or similar mobile health / MIH models is strongly preferred. BLS certification required; ACLS and PALS preferred where applicable. Excellent clinical assessment, problem-solving, and decision-making abilities. Strong verbal and written communication skills. Ability to work independently in the field, manage time well, adapt to changing schedules, and problem-solve in diverse home settings. Valid driver's license, reliable transportation, required insurance, ability to drive to patient homes. Job DescriptionDescription: A Paramedic working for Donald Martens and Sons Ambulance Service is responsible for responding to emergent and non-emergent calls that require a higher level of medical care. This could include intravenous administration, administration of medications, advanced oxygen treatment and advanced cardiac monitoring and procedures. Works closely with other Paramedics and/or EMTS to deliver excellent patient care and customer service. Requirements: RESPONSIBILITIES: · Ability to critically think to assess the extent of illness or injury accurately and obtain the patient's medical history when possible. · Providing appropriate care for patient condition and acuity, such as stabilization of a patient, assessing/ managing airway and administering oxygen, CPR and cardiac monitoring, placing catheters for venous access, administering IV fluids and medication, following established guidelines and protocols. · Administering invasive and non-invasive therapies, including intubation and pulmonary ventilation, application of bandages, immobilization of fractures, bleeding control and various other tasks. · Able to lead a patient care team and manage a change in patient condition effectively. · Ability to stay calm and make the right decisions in adverse conditions · Dealing with physical as well as emotional needs of a patient. · Handling radio communications and coordinating with dispatch center. · Safe transportation of the patient to the appropriate medical facility. · Thorough documentation of the nature of illness and treatment provided while enroute to the facility.
