Mobile Health Specialist (Community Paramedic): Job Responsibilities
Home Visits: Conduct in -person home visits to manage acute illnesses and injuries.
Treatment & Collaboration: Provide treatment in line with evidence -based guidelines and collaborate with a multi -disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases.
Risk Assessment: Identify risk factors, such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
Documentation & Compliance: Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
Vehicle Requirements: Respond on an on -call basis using a personal vehicle, with the option to use a Care2U vehicle when available and pick up necessary equipment from designated locations.
Compensation and Job Type:
On -Demand: $40 per hour, with three hours of pay for the first patient and two hours per each additional patient (most responses average 75 minutes). Responses are as needed 365 days a year from 8 am to 10 pm.

Position Summary:
We have an exciting opportunity to join our team as a Per Diem Paramedic - NYU Langone Long Island.
In this role, the successful candidate is responsible for performing high quality pre-hospital emergency medical care at the advanced life support level. Transports the sick and injured safely, through the Nassau County System, as per regional and state protocols. Demonstrate the ability to perform clinically appropriate patient assessments, document patient medical history accurately, and appropriately document on e-PCR.
Job Responsibilities:
Perform other duties as assigned.
Preforms clinically appropriate patient assessments and appropriately treats the patient in accordance with NYS DOH BEMS and Nassau County Guidelines. Safely transports patient to the most appropriate hospital and properly transfer care to the hospital staff (i.e. communicating patient's condition and any treatment rendered).
Accurately completes Electronic Pre-hospital Call Report (e-PCR) including but not limited to, insurance information and all appropriate signatures in accordance with NYU Langone Hospitals EMS Policies for every patient contact. Accurately completes e-PCR for every call dispatched to whether patient contact was made or not.
Accurately completes NYS DOH BEMS Check Out Sheet and documents any deficiencies with corrective actions if any taken, in accordance with NYS DOH BEMS Part 800. Accurately completes the Unit Activity Log in accordance to NYU Langone Hospitals EMS Policies and Procedures and submits all forms at the end of tour.
Ensure Out of service time is minimized and documented on Unit Activity Log with appropriate notifications made. Complies with uniform policy, maintains a neat, clean, professional appearance and wears appropriate identification, visible at all times, wears uniform with all appropriate personal equipment and spare uniform in accordance with NYU Langone Hospitals EMS and NYU Winthrop Hospital Policy and Procedures. Reports to work on time, clocks in and out in accordance with NYU Langone Hospital Policy and Procedures. Complies with Controlled Substance Policies and Procedures in accordance with NYS Department of Health Part 80 Guidelines.
Is familiar with contents, and complies with policies of Nassau County REMSCO, which was supplied to employee and complies with NYS DOH BEMS Regulations. Notify Nassau County Medcom at the beginning of the shift and logs on in a timely fashion. Constantly monitors the radio for assignments. Answers the radio promptly when assignment is dispatched and promptly communicates changes in signal status to Medcom. Maintains professional radio decorum and complies with local guidelines for radio protocol.
Adhere to Regional, NYS DOH BEMS and NYU Langone Hospitals EMS Policies and Procedures when operating emergency vehicles. Thoroughly checks vehicle and equipment at the beginning of each shift, replenishing supplies where needed and ensures the vehicle is clean and sanitary. Thoroughly cleans and maintains the vehicle and all equipment dispatched to the unit and on the vehicle. Ensures the vehicle is restocked and fueled at all times in accordance to NYS DOH BEMS Part 800 Compliance.
Ensures high-quality patient care is conducted at all times, on-scene and en-route to the hospital. Works well with partners, other providers at the scene and all employees of the hospitals to ensure the highest quality of care is provided. Cooperates and maintains professional relationship with others in facilitating the delivery of care to patients. Cooperates and works well with supervision, and others to achieve the goals of the department. Demonstrates awareness of and remains compassionate, respectful and professional towards co-workers, patients, families, cultural background and differences. Assess the patients ability to communicate with the crew and assists in the provision of an interpreter if needed.
Participates in Department, Hospital and Regional Quality Improvement Programs. Attends Department Meetings and Training Sessions.
Minimum Qualifications:
To qualify you must have a High School or GED. Completion of NYS DOH BEMS Approved Paramedic Program. Completion of CEVO III or CEVO IV or EVOC Course. Current and Valid NYS Driver License. NYS DOH BEMS Paramedic Certification. Nassau County REMAC Certification. Weapons of Mass Destruction Certificate. HAZMAT Awareness Certificate. Required to obtain Nassau Regional EMS MAC Certification within 10 days of hire. 6 Months Ambulance experience as a Paramedic.

Job Summary: Providing both emergency and non-emergency transport of patients. Empress EMT'S & Paramedics will collaborate with their direct supervisors, Municipal & County Fire, Police departments as well as other medical staff, patients and family members. To provide the highest level of patient care and customer service. At the direction of the Operations Department, the EMT will perform or assist with a wide variety of patient care activities in both the pre-hospital and hospital setting. The EMT is expected to provide supportive and definitive care to the patient of groups or group of patients. The EMT will provide care in the emergency setting as well as care to the patients during inter-facility transportation.
Requirements for Employment:
Current New York State Certification (EMT-B) and current CPR Certification. The EMT is required to maintain valid certification at this level at all times. And any other local region requirements.
A valid Driver's License and a clean driving record, which meets the underwriting guidelines of our insurance carrier. The EMT is required to maintain his/her driving status. The EMT is required to maintain a valid driver's license at all times.
Negative Drug Tests. Empress Ambulance Service will arrange testing day and time
Satisfactorily completes all aspects of the pre-employment process administered by/ for Empress Ambulance Service, LLC including drug screening/background check and CPAT
Employees are hired as and are considered probationary for a period of 180 days.
Work Environment: Works outdoors and indoors with occasional exposure to hazardous conditions and to blood/body fluids, requires respirator use, fumes/odors, extended day, and temperature changes. Frequently works with others, face-to-face contact with others with exposure to noise, mechanical equipment, and electrical equipment. Performs shift work and performs duties frequently in a mobile environment and in confined areas.
: Main Duties
Assesses, plans, implements and evaluates patient care within the scope of established treatment protocols and the current standard of care.•Provides accurate and pertinent written documentation of all patient care and EMS responses.
Completes ALL required Empress and NYS paperwork.
Maintains all patient care equipment, vehicles, supplies, daily logs and patient care record as directed by the company.
Operates Empress Vehicles and equipment as required, according to company policy. Maintains a safe and orderly work environment and promotes safe work practices for all employees.
Reports to work on time and understands that due to the nature of EMS, the EMT's assigned shift may exceed the assigned time for the end of the shift. Mandatory overtime may be assigned.
Work schedules encompass a variety of shifts. The EMT may be required, as part of his/her normal duties, to work ANY of the shifts available.•Follows all Empress Policies and Procedures.
Attends mandatory continuing medical education, updates and staff meetings as required by Empress.
Complies with the established guidelines for uniform dress and standard equipment regulations.
Assist in the provision of public information and educational programs designed to promote EMTS and the services offered by Empress, to the community and allied EMS agencies.
The employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to stand and walk. The employee is required to sit; climb or balance; step, stoop, kneel, or crouch.The employee must frequently lift and/or carry a maximum of 150 pounds unassisted and up to 300 pounds as part of a two-person crew. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Will be required to render appropriate medical care to patients suffering from various communicable diseases. The EMT will follow the recommendations for personal protection as currently issued by New York State Department of Health EMS Bureau.
Will be prepared to encounter violent or abusive patients. The EMT will take necessary steps to insure his/her own safety at all times.
