Job Duties: Serve as a Paramedic on a Specialty Care Team (SCT), providing specialized care utilizing critical care skills and intensive observational monitoring techniques to assess patient condition and provide an advanced level of specialized care to all critically ill patients (surgical, medical and pediatrics). Conduct long distance inter-facility critical care patient transfers to include critical care transport to higher level of care hospitals and medical centers. Administer critical care drugs as necessary which require specialized knowledge of the monitoring and infusion equipment and the effects, side effects, doses, and dosages calculations due to the potency and potential to cause serious complications. Operate specialized medical equipment i.e.., resuscitators, defibrillators, electrocardiographs, oxygen apparatus, intravenous pumps, rapid intravenous infusers and ventilation devices. Administer oxygen, intravenous (IV) fluids, blood transfusions and all types of medications during emergent events by various routes to include but not limited to intravenous, intraosseous, subcutaneous, intramuscular, by mouth and rectal. Provide airway support including opening and maintaining the airway using appropriate methods, perform advance airway techniques, utilize Rapid Sequence Induction for intubation procedures, confirm intubation success through monitoring of pulse oximeter, capnography or other secondary confirmation methods to ensure correct placement of emergency advance airway devices. Provide cardiac care as required, to include, but not limited to, treating chest pain and cardiac emergencies by monitoring cardiac electrical activity and obtains and interprets 4,12, or 15 lead Electrocardiogram (EKG) tracing, monitor and treat cardiac patient pain, level of consciousness, and associated respiratory conditions by providing oxygen therapy. Initiate pharmacological interventions to treat dysrhythmias, treat for shock, perform manual defibrillation or transcutaneous external pacing of the heart and applies and uses manual defibrillator and external mechanical cardiopulmonary resuscitation devices, perform setup and use of various cardiac monitors for defibrillation, cardioversion or pacing of irregular and life threatening cardiac arrhythmias in accordance with the American Heart Association (AHA) guidelines varying the Advance Cardiac Life Support (ACLS) approach as necessary for small children/infants. Provide shock treatment and fluid therapy by closely monitoring patients exhibiting signs of shock for changes in level of consciousness and vital signs and initiate and monitor IV fluids and blood products and discontinue their usage following established procedures/protocols. Provide treatment of traumatic injuries and mobilize for transport. Provide pediatric and OB care by performing an advanced assessment and treatment of the pediatric patient, performing emergency childbirth and treatment of predelivery complications and provide newborn resuscitation. Other care as required may include but not limited to recognizing, stabilizing and treating a wide variety of medical emergencies, burns (heat, electrical and radiation), behavioral issues, employing communicable disease control and containment measures as required for highly contagious patients. Operate Emergency Vehicles (EV)/Ambulances. Vehicle may be first response type vehicles, ambulances or other emergency transport vehicles. Operate EV/Ambulances safely and adhere to traffic ordinances and regulations pertaining to the operation of the EV/Ambulance. Perform routine operator safety checks and thorough emergency vehicle maintenance inspection. Use telephone and radio communications equipment and systems. Inventory all supplies and equipment. After each patient call, replace linens, blankets and other medical supplies; checks, cleans and disinfects reusable equipment; sends supplies and equipment for sterilization as required and check ambulance supplies and equipment to ensure the vehicle is sanitary and ready for the next run. Prepare appropriate patient and vehicle documentation and reports to ensure medical requirements are met. When not performing other duties as needed and/or as required, assist in providing care in the Emergency Department as needed within their scope of practice. This includes, but is not limited to, patient assessment, medication administration by various routes, obtaining blood samples, obtain intravenous access and intubation. Assist with patient care for Trauma and Code Blue activations as well as assisting with transfusing and managing blood products. Assist the Transfer Center staff in coordinating provider consultations, as well as emergent, urgent and routine patient transfers inbound and outbound from NMCP. This includes but is not limited to obtaining the most optimal Military or civilian treatment facility and mode of transport based on the patient's healthcare needs and current conditions. Assist in the coordination of facility and provider acceptance, scheduling of transport assets, and all other logistical aspects of the transfer mission. Update informatics and documentation systems to record and track patient transfers. Assist the command during periods of crisis, natural disaster, patient surge and mass casualty incidents serving as a Paramedic where needed, and/or supporting the transfer center as a central communications hub for all incoming and outgoing patients. Participate in regular staff meetings to facilitate communication for departmental process improvement. Participate in Quality Assurance/Quality Improvement assessments of all patient care reports. When not performing other duties as needed, and/or required, the Paramedic will perform as a trainer/educator for various training and courses both through the MTF's Staff Education and Training (SEAT) department, and as needed, on-the-job training with staff and active duty personnel when the situation presents itself. These courses include, but are not limited to, BLS, ACLS, and PALS. Assist in the training of NMCP staff through participation and assistance during Code Blue, Trauma and other emergency drills. During the transport of patients, educate active duty personnel performing “ride along” training on transport environment and patient assessment and treatment. Provide patient care to patients being transported into or out of Military Treatment Facilities. Complete the state approved emergency vehicle operations course (EVOC) and function as vehicle operators for the ground ambulances. Complete team orientation training and participate in any additional training as required by the Transport Team Medical Director. All patient care will be reviewed by the Transport Team medical director through a standard quality assurance process. The Medical Director may direct contracted personnel to complete additional training based on quality assurance reviews. Contracted personnel will be expected to participate in all quality improvement programs directed by the Transport Team Medical Director. Paramedics unable to be credentialed or whose credentials are revoked will need to be replaced by a qualified paramedic by the contractor. Paramedics will be required to renew their national registry paramedic certification on a biannual basis. Although training provided by Naval Medical Center Portsmouth can be put towards their continuing education (CE) hours, they will not be specifically given time during their assigned work hours to complete their CE requirements. Utilize the Transport Team Medical Director approved electronic medical record and participate in the quality assurance process by conducting peer reviews. Provide patient care commensurate with the scope of practice in the Emergency Medicine Department (EMD) when on duty for the IFTT but not scheduled for a patient transport mission. Provide patient care on clinical units (e.g. Operating Room) as assigned by the Medical Director or Program Manager as part of ongoing clinical skills sustainment Qualifications Experience : At least twelve (12) months of experience as an NR-P after graduation. Licensure : As required to operate any Emergency Vehicle (EV)/Ambulance as applicable by the duty location assigned.
