Payroll-HR Support Associate is responsible for the overall operations of the payroll and Human Resources Administrative functions of the facility. Duties include Payroll, Personnel On-Boarding/Records, and Benefits Administration, and provide administrative support to the Executive Director and Diakonos Director of Human Resources.
DUTIES AND RESPONSIBILITIES
Payroll Functions
Correct missed punches daily in timekeeping system, with appropriate documentation and approvals.
Submit hours and payroll changes per current payroll processing schedule prior to pay day.
Ensure updates for newly hired and terminated staff are posted to payroll software promptly and per schedule.
Respond to employee inquiries regarding payroll in a timely manner.
Processes required enrollments and terminations documents through payroll and insurance providers to ensure accurate record keeping and proper deductions.
Act as liaison between employee and support staff.
Ensure monthly Quality of Care Report completed.
HR Administrative Support Functions
Performs recruitment activities including posting, tracking and updating open positions and candidate status using job boards and related software, as assigned. Maintains records related to same.
Responsible for completing and maintaining all functions of OKScreen such as background check, licenses, employee roster, provisional and permanent employees, re-checks, auto flag activities and ensuring compliance.
Conducts new employee orientations at facility to ensure employees gain an understanding of benefit plans and enrollment provisions.
Strives to ensure employee understanding of benefit programs by, regularly counseling employees/dependents as situations arise. Resolves employee issues related to health and welfare plans, refers difficult or very complex complaints to insurance agent and/or support team as needed.
Acts as a resource for employees to ensure their understanding and compliance with benefit and HR policies and regulations.
Assists with annual benefit open enrollment period each year, as needed. Processes benefit changes, including enrollment and termination forms within deadlines.
Ensures incident reports and related Workers Compensation claims are completed and submitted in a timely manner.
Assists in the completion of responses to unemployment claims and provides backup documents as required.
Maintains authorized personnel filing system that meets the needs of the facility and complies with current employment practices.
Assists with preparation of annual affirmative action plan, if applicable.
Completes personnel-related reports for management as requested.
Office Administration Functions
Answers phones, order office supplies, correspondence, maintains copy machines, and related office equipment, as required.
Files all documents as required.
Maintains a good working relationship with residents, patients and their families, and facility personnel and support staff.
Attends in-service training classes, daily stand-up meetings, and other meetings as required
Provide supporting documents for audits.
Personnel Functions
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility.
Assist in recordkeeping and documentation of attendance and tardiness for employee performance appraisals, as assigned.
Confirm license/certification and current standing with state approved agencies for all new hires prior to orientation date. Update Kronos and OKScreen.
Maintain confidentiality of all pertinent employee information.
Report known or suspected incidents of fraud to the Administrator.
Ensure that departmental computer workstations left unattended are properly logged off or the password protected automatic screen-saver activates within established facility policy guidelines.
Staff Development
Provide each newly hired personnel with orientation schedule.
Facilitate quality orientation program by communicating new hire orientation schedule to unit manager and unit staff who will participate in orientation.
Attend in-service training programs as scheduled.
Assist with recordkeeping for facility in-service education, and orientation classes for newly hired personnel.

Position Summary:

In charge of all facility maintenance tasks, both indoors and outdoors.
Responsible for keeping up with all maintenance related documentation and turn into Executive Director as required .
Supervise the day-to-day maintenance functions of the facility .
Follow required maintenance standards .
Follow procedures for performing daily maintenance tasks , help come up with ways to improve existing procedures .
Schedule and prioritize assignments with assistance from the Executive Director .
Be able to comfortably interact with residents and family members while performing duties .
Submit accident reports to the Executive Director on the shift in which they occurred.
Must be able to keep information confidential to protect the residents and comply with HIPPA .
Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Executive Director.
Report any known or suspected unauthorized attempt to access facility's information system.
Assist in the orientation and training of personnel as needed .
Teach personnel how to use required equipment and housekeeping chemicals as directed.
Be knowledgeable of basic plumbing, air-conditioning, appliances, vehicles, electricity, etc.
Be able to drive in inclement weather
Keep inventory of supplies and order as directed following your budget.
Monitor and refill toilet paper and paper towels.
Ensure that personnel are performing assigned tasks in accordance with established maintenance procedures.
F ollow the manufacturer's guidelines when servicing equipment.
Ensure that painting, rework or repairing of facility equipment is accomplished in accordance with established policies.
Counsel/discipline personnel as requested or as necessary in a professional manner. Report such actions to the Executive Director.
Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status.
Review complaints/grievances of department personnel and make oral/written recommendations/reports to the Executive Director. Follow facility's established procedures.
Conduct monthly Safety Meetings, Drills, and oversee OSHA Compliance.

Summary:

Attach pre-admission documentation to admission papers as appropriate
Assure that work/assignment areas are clean and records, files, etc., are properly stored before leaving such areas on breaks, end of work day, etc.
Assist in developing procedures to ensure records are properly completed before filing
Collect, assemble, and check admission papers and forward to the appropriate department
Obtain the resident's assigned room number. Place on all necessary admission papers
Maintain a record of authorized information taken from records i.e., type information, name of recipient, date, department, etc., in accordance with our facility's established privacy policies and procedures
Abstract information from records as authorized/required for insurance companies, Medicare, etc.
Notify nursing service when the resident has arrived at the facility to assure that a member of the nursing staff escorts the resident to his/her assigned room
Maintain various registries as directed including register for admission and discharge of residents
Review/explain the resident, and/or guardian, room rates, billing procedures, visitors/guest privileges, restrictions, resident care procedures, etc., as appropriate
Establish and maintain resident master card file
Notify nursing service and call for assistance, as necessary, if resident is disturbed/violent upon admission or if resident shows symptoms of infectious/communicable disease
Refer admission problems to proper authority, i.e., office supervisor, credit manager, nursing service, social services, etc.
Maintain a resident waiting list in accordance with established policies and procedures
Keep abreast of current Medicare/Medicaid regulations governing admission/discharge requirements of health care facilities
Maintain daily and monthly census report as directed
Assist the MDS Coordinator in scheduling resident assessments
Perform miscellaneous duties and assist business office personnel as required/directed

Develop, implement, and maintain a facility-wide infection prevention and control program.

As Infection Preventionist you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

DUTIES AND RESPONSIBILITIES
Major Duties and Responsibilities
Develop and implement an ongoing infection prevention and control program to prevent, recognize, and control the onset and spread of infections in order to provide a safe, sanitary, and comfortable environment.
Establish facility-wide systems for the prevention, identification, reporting, investigation, and control of infections and communicable diseases of residents, staff, and visitors.
Develop and implement written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control.
Oversee the facility's antibiotic stewardship program.
Oversee resident care activities that increase risk of infection (i.e., use and care of urinary catheters, wound care, incontinence care, skin care, point-of-care blood testing, and medication injections).
Lead the facility's Infection and Prevention Control Committee. Develop action plans to address opportunities for improvement.
Participate on the facility's QAA Committee. Perform duties as assigned.
Implement an annual infection control risk assessment process and participate in the facility's
annual facility assessment review process.
Review and/or revise the facility's infection prevention and control program, its standards, policies, and procedures annually and as needed for changes to the facility assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections.
Provide education related to infection prevention and control principles, policies, and procedures to staff, residents, and families (where appropriate).
Ensure public health is notified of reportable diseases.
Collaborate with other departments in fulfilling requirements related to occupational health and safety. (I.e. employee health).
Maintaining documentation of infection prevention and control program activities.
Additional Assigned Tasks
Treat all residents with dignity and respect. Promote and protect all residents' rights.
Establish a culture of compliance by adhering to all facility policies and procedures.
Comply with standards of business conduct, and state/federal regulations and guidelines
Remain current on new developments related to infection prevention and control by attending professional institutions, reading professional journals, attending professional seminars, or reading new laws, rules, and regulations.
Serve as resource for staff regarding infection prevention and control, including the identification of when a resident needs to be placed on isolation precautions.
Work with environmental services to prevent cross-contamination in the care environment.
Work with community partners and other healthcare facilities to help prevent transmission of infection during care transitions.
As a condition of employment, complete all assigned training and skills competency, as determined by the facility assessment and facility training plans.
Participate in all life safety and emergency drills and trainings. Fulfill responsibilities as assigned during implementation or activation of the facility's emergency plan.
Assist with admission, discharge, or care of residents as needed.
Use proper lifting and body mechanics while delivering care to residents.
Perform administrative duties as assigned.
Maintain confidentiality of protected health information, including verbal, written, and electronic communications.
Report noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Report any retaliation or discrimination to HR.
Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Fulfill individual responsibility to report any suspicion of a crime to law enforcement and the state agency. Protect residents from abuse, and cooperate with all investigations.
Report any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
Report work-related injuries and illnesses immediately to supervisor.
Education
Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
Experience
Must have, as a minimum, two (3) years of experience as an LPN or RN.
Specific Requirements
Must possess a current, unencumbered, active license to practice as an LPN/RN in this state.

Education
Must possess, as a minimum, a Nursing Degree from an accredited college or university.
Experience
Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
Must have, as a minimum six (6) months experience in rehabilitative and restorative nursing practices.

Develop, maintain, and periodically update written policies and procedures that govern the day-to-day functions of the nursing service department.
Maintain a reference library of written nursing material (i.e., PDR'S, Regulations, Standards of Practice, etc.) that will assist the nursing service department in meeting the day-to-day needs of the resident.
Develop, maintain, and periodically update the Nursing Service Procedures Manual and nursing service objectives and philosophies.
Develop and implement a nursing service organization structure.
Make written and oral reports/recommendations to the Administrator, as necessary/required, concerning the operation of the nursing service department.
Develop, maintain, and periodically update written job descriptions for each level of nursing personnel in accordance with pertinent laws and regulations.
Develop methods for coordination of nursing services with other resident services to ensure the continuity of the residents' total regimen of care.
Develop, implement, and maintain an ongoing quality assurance program for the nursing service department.
Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed.
Assist the Quality Assessment and Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
Assist the Discharge Planning Coordinator in developing, implementing and periodically updating the written procedures for the Discharge Planning Program.
Assist the resident and Discharge Planning Coordinator in planning the nursing services portion of the resident's discharge plan.
Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
Monitor the facility's QI, QM, and survey reports. Assist in developing plans of action to correct potential or identified problem areas.
Assist in calculating the number of direct nursing care personnel on duty each shift. Report such information to the Administrator or his/her designee to ensure that accurate staffing information is posted.
Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.
Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
Report any known or suspected unauthorized attempt to access facility's information system.
Determine the staffing needs of the nursing service department necessary to meet the total nursing needs of the residents.
Recommend the number and level of nursing personnel to be employed.
Assist the Administrator and/or the HR Director in the recruitment and selection of nursing service personnel.
Ensure that all nursing assistants are enrolled in or have graduated from an approved training program.
Assign a sufficient number of licensed practical and/or registered nurses for each tour of duty to ensure that quality care is maintained.
Assign a sufficient number of certified nursing assistants for each tour of duty to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.
Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks.

Participate in functions involving discharge plans as may be necessary.
Evaluate and implement recommendations from established committees as they may pertain to the assessment and/or care plan functions of the facility.
Nursing Care Functions
Conduct or coordinate the interviewing of each resident for the resident's assessment.
Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents.
Assist facility directors and supervisors in scheduling the resident assessment/care plan meetings.
Contact and assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family members.
Ensure that care provided is in accordance with the resident's wishes.
Staff Development
Develop and participate in the planning, conducting, and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility.
Assist the In-service Director/Educator in developing any training activities needed concerning resident assessment/care plan skills (including, but not limited to initial or refresher courses relative to techniques for interviewing residents, rehabilitation principles, commonly used psychotropic drugs, care plan functions, etc.).
Attend and participate in annual facility in-service training programs as scheduled (e.g., OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.)
Attend and participate in advance directive in-service training programs for the staff and community.
Attend and participate in continuing education programs designed to help you keep abreast of changes in your profession, as well as to maintain your license on a current status.
Conduct training programs for appropriate staff on the completion and use of the MDS.
Care Plan and Assessment Function
Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies.
Ensure that appropriate health professionals are involved in the assessment.
Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification.
Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.
Education
Must possess, as a minimum, a Nursing Degree from an accredited college or university.
Experience
Must have, as a minimum, two (2) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
Must have, as a minimum, six (6) months training experience in rehabilitative and restorative nursing practices.

Report to Director of Clinical Services (DCS) all employees with excess tardiness or absenteeism in accordance with facility policy.
Work with Human Resource Director and DCS when scheduling modified work duty employees in accordance with work restrictions and facility policy.
Complete in timely fashion necessary nursing department employee records upon hiring, job reclassification, and discharge.
Assist employees in accurate completion of forms (i.e., leave and day off requests, time cards, etc.) pertinent to scheduling and staffing.
Complete monthly nursing schedule coordinating requests to ensure appropriate coverage of units.
Complete and post work sheets/time schedules. Coordinate variances with the DCS.
Assist in completion and filing of designated reports in accordance with established policies and procedures.
Perform miscellaneous duties pertaining to staffing and assist nursing supervisory personnel as necessary.
Answer employee calls regarding staffing/scheduling functions. Prepare written correspondence as necessary.
Answer applicant calls regarding position availability and coordinate interviews as deemed appropriate.
Ensure newly hired nursing staff has time cards and schedules prior to orientation date.
Assist in obtaining nursing care staff information needed for daily posting requirements.

Review complaints and grievances pertinent to scheduling and staffing. Make necessary oral/written reports to the DCS.

Confirm license/certification and current standing with state approved agencies for all new hires prior to orientation date.
Maintain confidentiality of all pertinent employee information.
Assist with maintaining records of current certifications/licenses for nursing department employees.
Report known or suspected incidents of fraud to the Administrator.
Ensure that departmental computer workstations left unattended are properly logged off or the password protected automatic screen-saver activates within established facility policy guidelines.
Staff Development
Provide each newly hired nursing personnel with orientation schedule and assign staff member to provide orientation.
Facilitate quality orientation program by communicating new hire orientation schedule to unit manager and unit staff who will participate in orientation.
Attend in-service training programs as scheduled.
Assist with recordkeeping for facility in-service education, and orientation classes for newly hired personnel.
Safety and Sanitation
Report all unsafe/hazardous conditions, defective equipment, etc., to your supervisor immediately.
Follow all established safety procedures and precautions when operating office equipment.
Equipment and Supply Functions
Ensure nursing supervisory supplies have been replenished in work areas as necessary. Report equipment/supply needs to your supervisor.
Ensure that work/assignment areas are clean and equipment and supplies are properly stored before leaving such areas on breaks, end of day, etc.
Budget and Planning Functions
Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc.
Resident Rights
Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
Ensure that resident's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints are followed.
Knock before entering a resident's room.

General Purpose:
Provide direct nursing care to the residents and supervise the day-to-day nursing activities performed by nursing staff. Such supervision must be in accordance with the Nurse Practice Acts and current Federal, State, and Local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to maintain the highest degree of quality care at all times.
Administrative Functions
Direct the day-to-day functions of the nursing staff in accordance with rules, regulations, and guidelines that govern the long-term care industry.
Confirm that all nursing staff assigned to you comply with the written policies and procedures established by this facility.
Meet with your assigned nursing staff, as well as support personnel, in planning the shift's services, programs and activities.
Verify that all nursing staff comply with the procedures set forth the in the Nursing Policies and Procedures Manual.
Make written and oral reports/recommendations concerning the activities of your shift, as required.
Cooperate with other resident services when coordinating nursing services to ensure the resident's total regimen of care is maintained.
Verify that all nursing service staff are in compliance with the Nurse Practice Acts and their respective job descriptions.
Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing department.
Participate in surveys (inspections) made by authorized government agencies.
Periodically review the resident's written discharge plan. Participate in the updating of the resident's written discharge plan, as required.
Assist in planning the nursing portion of the resident's discharge plan, as necessary.
Admit, transfer, and discharge residents, as required.
Complete accident/incident reports, as necessary.
Charting and Documentation
Complete and file required forms/charts upon resident's admission, transfer, and/or discharge.
Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.
Receive telephone orders from physicians and record on the Physicians' Order Form.
Transcribe physician's orders to resident charts/EHR, medication cards, and treatment/care plans, as required.
Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to care.
Chart communication with physician, other professionals providing care to the resident and pertinent family communications.
Record new/changed diet orders. Forward information to the Dietary Department.
Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nurse Supervisor.
Fill out and complete transfer forms in accordance with established procedures.
Enter appropriate documentation concerning unauthorized discharges in the resident's medical record in accordance with established procedures.
Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
Sign, date and time all entries made in the resident's medical record.
Write nurses' notes to reflect that the care plan is being followed when administering nursing care or treatment.
Medication Administrative Functions
Prepare and administer medication as ordered by the physician.
Responsible for IV medication/fluid administration.
Verify the identity of the resident before administering the medication/treatment using the 5 Rights of medication administration.
Verify that prescribed medication for one resident is not administered to another.
Verify that adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor.
Order prescribed medications, supplies, and equipment as necessary, and in accordance with established procedures.
Verify that narcotic records are accurate for your shift.
Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift.
Review medication administration records for completeness of information, accuracy in the transcription of the physician's order and adherence to stop order policies.
Immediately notify the Nurse Supervisor and attending physician of any medication administration errors and/or adverse reactions to medications.
Nursing Care Functions
Perform physical assessment through observation, inspection, palpitation, percussion and auscultation.
Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
Promptly respond to call lights and other resident needs.
Make rounds with physicians, as necessary.
Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.
Review the resident's chart for specific treatments, medication orders, diets, etc., as necessary.
Make periodic checks to confirm that prescribed treatments are being properly administered by nursing staff and to evaluate the resident's physical and emotional status.
Notify the resident's attending physician and responsible party when the resident is involved in an accident or incident.
Notify the resident's attending physician and responsible party when there is a change in the resident's condition.
Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required.
Monitor seriously ill residents, as necessary.
Review care plans daily to confirm that appropriate care is being rendered.
Confirm that nursing staff are aware of the resident care plans. Verify that the nursing staff refer to the resident's care plan prior to administering daily care to the resident.
Other Duties
Attend in-service education programs in order to meet facility educational requirements.
Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information.
Maintain confidentiality of resident and facility records/information.
Protect residents from neglect, mistreatment, and abuse.
Protect the personal property of the residents of the facility.
Others as directed by the supervisor or administrator.
Minimum Qualifications:
Registered Nurse with current state license in good standing.
Excellent technical, assessment and documentation skills.
Good Leadership qualities.
Organized and detailed in work performance.
Ability to multitask and work under pressure.
Good communication skills with excellent self-discipline and patience.
Genuine caring for and interest in elderly and disabled people in a nursing facility.
Comply with the Residents Rights and Facility Policies and Procedures.
Perform work tasks within the physical demand requirements as outlined below.
