Essential Duties:

Summary of Program and Position:
Voluntary Case Management (VCM) program serves families on Oahu who have been referred by Child Welfare Services (CWS) after a report to the Child Abuse and Neglect Hotline has been made and the family has been identified as moderate to moderate high risk. Each family will be assigned a case manager who will address the range of family system issues that place children at risk of child abuse and neglect. They may also be assigned a skill builder who will work with the family to increase skills such as, but not limited to, parenting skills, co-parenting skills, problem solving skills, and stress management skills.
The Case Manager provides voluntary, culturally appropriate, and strengths-based services related to child safety concerns to families referred by the Department of Human Services who are moderate to high risk for child abuse and neglect. These services are focused on addressing all reported concerns, increasing parents' ability to establish and maintain safety for their children, meet their children's needs, problem solve, and access community resources. Services are provided at the home or other “natural” environments preferred by the families. The Case Manager works with referred clients to engage families, refer, and coordinate services, plan, ensure children's safety, provide parenting and family strengthening activities including persistent and creative outreach, monitor progress and document family status. This position is under the direct supervision of Program Supervisor.
Essential Duties:
Ensures that the delivery of service is done in a manner consistent with the standards and guidelines established by the funding agencies, state law, and the Parents And Children Together policies and procedures.
Provides assessment and lead case management services that ensure that families gain access to appropriate resources in an efficient and timely manner.
Assesses each family's strengths and weaknesses, identifies their needs, and develops goals and objectives as part of the Family Partnership Plan and Family Partnership Plan Activities developed with the family. Completes required assessments (Child Safety Assessment and Comprehensive Strength and Risk Assessment Tool), including the Adult-Adolescent Parenting Index according to required timelines.
Provides social services consisting of parenting skill building, child development education, alternatives to corporal punishment, increasing knowledge and skills in meeting child and family needs, assistance in developing a support network, preparation for longer-term services, and other areas as needed to reduce risk factors for child abuse and neglect and increase protective factors.
Links families to needed community resources and conducts follow up activities as needed, teaching the family to identify and access resources throughout the process. Ensures all services on the Family Partnership Plan and the Family Partnership Planning activities are in progress and/or completed.
Meets at least once a month with the Program Supervisor to discuss progress on all families, and notifies their supervisor of progress, challenges, and any significant and critical issues, including incident reports. Consults weekly with Program Supervisor on top tier families and as needed for a family in crises. Case consultation with supervisor includes following recommended actions and the review and approval of all reports by supervisor.
Completes clear, concise, and non-judgmental written and electronic documentation, such as initial assessments, monthly updates, and closing reports, and ensuring that all paperwork is completed and submitted in a timely manner for review and approval by the Program Supervisor.
Minimum Qualifications:

Family Visitation Centers provide a safe, neutral setting for children to visit with their non-custodial parents or transition from one parent to the other. After a divorce or separation, family visitation services may be arranged by agreement of both parents and by court order.
The Visitation Monitor I will provide and monitor visitations to families identified and/or referred by the Judiciary, attorneys, mediation programs or themselves as needing such services related to family violence and child development. This position communicates with family members, other staff members, Program Management, and service providers. In addition, the position performs a wide variety of safety measures, office and clerical duties
Essential Duties:
Provides family visitation services to all families referred to the program, observes family dynamics and behaviors, provides interventions and recommendations as needed. Responsible for the supervision and monitoring of children and parents during visitation.
Ensures a positive environment for family members and the FVC staff, including welcoming all children and parents and checking in participant clients and items to be used during visitation to ensure only allowed items are permitted in the visitation, and ensuring clients are in appropriate spaces within the Center.
Provides safety for family members and the Family Visitation Center staff as scheduled, including monitoring of the parking lot, monitoring the location of family members in the center, and intervening in potential safety threats as required. Minimizes potential for unsafe visitation environment through ushering of clients to other locations or other de-escalation means as needed.
Responsible for the supervision and monitoring of children and parents before, during, and after visitation.
Accurately completes all necessary forms, reports and correspondence in a timely fashion. Maintains appropriate documentation including records of families coming to and from the Center, and assessment and observation forms. Attends court hearings as required by subpoena.
Answers the telephone and performs general office and clerical duties as assigned.
Participates in all team and staff meetings, conferences, trainings, and workshops as arranged by the Program Supervisor.
Minimum Qualifications:
Minimum of High School Diploma or equivalent, with (1) one-year experience in direct services to children and high-risk families, required.

The Hana Like Home Visitor Program works with families to provide supportive coaching to teach strategies that enhance the parent or caregiver's skills to provide a safe, healthy, nurturing environment for children ages birth to five years. Home visitors also work with prenatal mothers to support a wide range maternal-child health.
The Parent Educator provides evidenced-based visiting support services to prenatal mothers with young children to improve maternal and child health, child health and safety and child development knowledge. This position communicates with family members, internal team members, service providers and community partners and provides team leadership for service provision. This position is under the direct supervision of the Program Supervisor.
Essential Duties:
Conduct evidenced-based home visiting services to families with young children, ages 0-5 in a manner consistent with standards and guidelines established by funders, state law and policies and procedures of the agency and in a manner that will improve the self-confidence of parents. In partnership with families, curriculum materials are used to promote positive parent and child interaction and parenting, increase child development knowledge, promote family's interaction with the community and improve health-seeking behaviors. Monitors the quality of services to all families, including the implementation of the team approach.
In partnership with the family and the team, develops a meaningful Family Goal Plan (FGP) that addresses risk factors and maternal child health and safety.
Administer developmental screens such as the ASQ and ASQ-SE, hearing and vision screen, parent assessments, such as AAPI/HOME, as well as those screens required by the MIECHV project such as screening for prenatal care, tobacco use, postpartum care, and breastfeeding.
Enter and maintain all contact documentation on families by completing required forms, screens, and assessments, and maintaining and inputting documentation in program databases and funder database systems in an accurate and timely manner.
Provide linkage to community resources and follow-up with support services as needed, teaching families to identify and access resources throughout the process.
Conduct creative outreach activities designed to engage hard-to-reach families, in a consistent, yet respectful manner.
Maintain effective working relationships with referral source, EID (Early Identification) community partners, state agencies, funders, and other appropriate providers of home visitation services.

These programs promote the development of healthy youth, families and community with an abundance of positive experiences for youth and family members including educational, recreational, community building and support services. The Youth Development Specialist performs a variety of duties related to the development and implementation of positive youth development and parent involvement projects or services in a program for at-risk youth. Ulupono Family Strengthening Program works to increase the internal resources of families to handle challenges they face with their children. 3. Manages assigned caseloads in addition to the completion of required client intake and discharge processes, including comprehensive assessments, determination of safety risks of the client, and provides each client with thorough walk-throughs and orientation into the shelter.
4. Provides case management, individual advocacy, crisis intervention, and individual counseling for clients admitted to the shelter and notifies the Shelter Manager in the event of crisis situations.

Works directly with community resources such as landlords, housing authorities and others to promote and coordinate victim transition services. Coordinates rent collection when needed and works with agency management when rent payment is delinquent.
6. Provides crisis intervention and crisis counseling for clients experiencing episodes of trauma response, mental health crisis, or substance use crisis.
7. Provides clear, concise, and non-judgmental written documentation such as assessments, service planning, progress notes, opening and closing summaries, DHS reporting, follow-up calls, monitoring, collateral contacts, crisis calls and other pertinent information in client individual files in a timely manner. Implements and supports special education services, for children with special needs and their families, through the Department of Education (Part B provider) referral process.
