Spring Creek Rehab and Nursing is actively seeking a Director of Social Services for our Skilled Nursing Facility in Harrisburg, PA. The ideal candidate will have prior Long Term Care and managerial experience.

DUTIES:
Directs, establishes & plans the overall policies/goals for the Social Services Department
Responsible for assessments, care planning & helping residents adjust
Familiar with a variety of the field's concepts, practices & procedures
Relies on extensive experience & judgment to plan and accomplish goals
A wide degree of creativity and latitude is expected
Occasional Community Outreach
Meet with residents to discuss the financial plan
Gather documentation for Medicaid applications
Collect Private funds due to the nursing home
Assist with insurance authorizations

Job Description
The Cook prepares meals for patients, family members and employees. They also prepare meals in quantities according to menu and number of persons to be served. Is responsible for preparation of meal, cleaning up work area and serving area and for helping the residents determine what is to be served along with the main course for lunch and supper.
General Responsibilities:
Prepare various foods depending on menu, recipe & diet breakdowns for each meal in a WORLD CLASS way and on budget. Works with residents in the house assigned to provide for their food choices for the day.
Appropriately operates kitchen equipment in preparation of the meal.
Cleans work surfaces during shift (clean as you go): cleans floors, wipes down counter space (as spills occur), & completes the daily cleaning schedule prior to shift end.

Spring Creek Rehab and Nursing is seeking motivated and compassionate Licensed Practical Nurses to join our team!
We have Full time and Part-Time shifts available.
Day shift 7am-3pm -part time
Evening Shift 3pm - 11pm - full time and part time
Night Shift 11pm-7am -full time and part time
LPN with trach and vent exp - 12 hour day shift and 12 hour night shift
The LPN is responsible for the following:
Monitoring residents, their responses to treatment care plans.
Supervising and directing.
Document necessary information regarding nursing care, care plans and observation of residents overall conditions.
Perform various duties to provide quality nursing care to residents.
Analyzes information for appropriate interventions to prevent decline.
Record signs of change in condition.
Notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, etc.
Administers medications and treatments to residents; monitor administration of intravenous fluids; places orders for medication.
Maintain resident clinical files; keep charts updated; document appropriately.
Make rounds to see each resident daily.
Job Requirements
At least one year of experience in a Long Term Care environment is highly preferred.
Must be a graduate of an approved practical nursing program.
Must possess and maintain a current state PA license as a Licensed Practical Nurse (LPN)
Must be able to move, lift and position patients.

The Cook prepares meals for patients, family members and employees. They also prepare meals in quantities according to menu and number of persons to be served. Is responsible for preparation of meal, cleaning up work area and serving area and for helping the residents determine what is to be served along with the main course for lunch and supper.

Prepare various foods depending on menu, recipe & diet breakdowns for each meal in a WORLD CLASS way and on budget. Works with residents in the house assigned to provide for their food choices for the day.
Appropriately operates kitchen equipment in preparation of the meal.
Cleans work surfaces during shift (clean as you go): cleans floors, wipes down counter space (as spills occur), & completes the daily cleaning schedule prior to shift end.
