This is a part-time position to be scheduled a minimum of 2-3 days/week. A dental hygienist is an oral health professional licensed to provide educational, clinical and therapeutic services to the public. A dental hygienist's primary duties are to care for patients using an individual oral health regiment. ESSENTIAL FUNCTIONS: Listed below are some services that are in the scope of care that a dental hygienist may be able to perform. The Medical Case Manager provides ongoing medical case management for HIV positive patients, ensures access to appropriate services and coordinate service delivery. Emphasis is on treatment adherence, retention in care, and viral load suppression. Essential Functions Complete in-depth psychosocial assessment. Develop individualized treatment plan. General Summary of Responsibilities: The Substance Use Counselor will provide clinical services, including but not limited to conducting substance use assessments and screenings, providing treatment recommendations for people with substance use disorder, as well as those with co-occurring diagnoses (substance use and mental health). Additionally, the substance use counselor will initiate treatment planning, conduct individual therapy, group therapy, process referrals, provide case management and coordinate discharges. This position functions as a fully integrated member of the multidisciplinary team to provide therapeutic services to assist individuals with substance use diagnoses to obtain and maintain abstinence. The Substance Use Counselor will apply an integrated approach to address each individual client based on their medical needs, mental health needs, and relapse prevention needs. Essential Functions: Complete screening and assessments to gather detailed information about client history in order to best diagnosis, determine treatment needs, and treatment recommendations. Responsible for providing 1:1 case management to individuals with substance use disorders Develops client-centered treatment plans and goals to address needs that are identified during assessment and interactions with client, family, and referral source. The clinical duties of the Medical Assistant have a multi-purpose function. The Medical Assistant will be cross trained to perform activities from registration and appointments to administering and documenting immunization shots. The Medical Assistant will cross-train in all departments, namely Internal Medicine, OB/GYN, and Pediatrics. These responsibilities include, but are not limited to: Participate in the Pre-visit Planning/Huddle process for PCMH compliance. Follows up with lab reports, imagining reports, and patient referrals during pre-visit planning. Administers medications, vaccines, urine samples, pregnancy tests, strep tests, etc., as needed. The Deputy Director of Ryan White Programs is responsible for developing and utilizing methods, processes and techniques to achieve clinical quality management outcomes. The Deputy Director of Ryan White Programs will revise policies and procedures and clinical work flows for treatment and care services. Essential Functions Supervise licensed social workers and other program support staff. Master CAREWare operations and serve as backup for imports from EMR and uploads to state CAREWare system. Review client level data reports and progress in achieving Ryan White work plan/scope goals during staff supervisions. Essential Functions Oversee the day-to-day operations of the substance use unit. Provide all services associated with substance use treatment and counseling. The Pediatric Nurse Practitioner is responsible for delivering comprehensive, compassionate, and developmentally appropriate healthcare to infants, children, adolescents, and young adults. This includes performing thorough health assessments, diagnosing and managing acute and chronic conditions, prescribing treatments in accordance with current clinical guidelines, and developing culturally-sensitive, family-centered care plans. The provider educates and counsels patients and families on topics related to health promotion, disease prevention, growth and development, and illness management. Responsibilities also include ordering and interpreting diagnostic tests, prescribing medications in compliance with state and federal regulations, and monitoring treatment outcomes. This role requires timely delivery of clinical services, participation in the on-call rotation, accurate and detailed documentation, and adherence to Park West Health System policies and procedures. The Pediatric Provider actively engages in continuing education, provider meetings, and quality improvement initiatives to support clinical excellence and patient satisfaction. Duties and Responsibilities: Asses the health status of infants, children, adolescents, and young adults and develop appropriate plans for health supervision and illness management. This includes: obtaining a comprehensive developmental, health, and medical history; performing physical examinations based on age and history; performing age-appropriate and condition-specific screening, laboratory tests and diagnostic procedures; assessing and diagnosing childhood illness; formulating a culturally-sensitive, family-centered plan of care recognizing the child and family are active participants; and consulting with other health care providers as necessary. Provide comprehensive health assessments by teaching, counseling, and advising children and their families about growth and development, health status, illnesses, management plans, and health promotion, disease and injury preventing activities, and provide anticipatory guidance. Promote growth and development of infants, children, adolescents, and young adults. Utilize prescriptive authority, in accordance with state and federal laws and regulations to treat illness, improve health status, and provide preventive care. This includes: prescribing medication based on a child's clinical history, current assessment, and health care needs; prescribing medication based on current research and a thorough knowledge base in pharmacotherapeutics; providing patient education regarding intended effects, protentional side effects, and specific instructions for medication administration; and monitoring therapeutic and potential adverse effects on medications. Provides clinical services to Park West patients in a timely manner during times allocated for this purpose. Take call on weekdays, weekends, and holidays in rotation with the other providers in the practice. Maintain accurate and detailed patient documentation (i.e. medical records). Read and follow the policies and procedures implemented by Park West Health System. Utilize current medical guidelines for management of medical problems. Participate in/attend all provider and pediatric operations meetings. Promote medical advancement and improved patient satisfaction through quality improvement project involvement when possible/appropriate. Maintain competency by participating in relevant continuing education programs and certification maintenance programs. Performs all other duties as required. Qualifications Education: Bachelor of Science in Nursing, as well as a Master of Science in Nursing from an accredited institution.
