Provides medical and nursing expertise to patient care staff on issues and challenges involving evaluation of terminal decline, pain and symptom management, patient/caregiver education and proactive care planning. • Participates in the on-call NP coverage rotation after hours and weekends for in-person pre-admission F2F visits with completion of admission and completes admissions without required pre-admission F2F visits.
