As part of the Critical Time Transition Program (CTTP), the Case Manager provides intensive Critical Time Intervention (CTI) services for youth ages 11-17 with complex mental health needs and their families/caregivers, supporting transitions from higher levels of care to community settings. CTI is a phased, time-limited care management model emphasizing early and assertive engagement, trauma-informed practice, strengths-based approaches, resilience, psychiatric rehabilitation, positive behavioral strategies, and family/caregiver skill-building. Case Managers work across the community and a Transitional Residential Setting (homes, schools, hospitals, outpatient clinics), maintaining frequent contact with youth, families/caregivers, and involved agencies. Services prioritize in-person engagement. Case Managers actively coordinate care in the community, support skill-building, and strengthen family and youth self-advocacy while facilitating safe and effective transitions to the lowest appropriate level of care.
Responsibilities:
Carry a caseload of approximately 8 youth and families
Provide weekly, phased contacts with youth and families/caregivers, including assertive engagement, linkage to services, monitoring, and skill-building

RESPONSIBILITIES:
· Build relationships and support parents in coping with the trauma of foster care involvement, making decisions for their families, communicating effectively with those involved in their case, and maintaining relationships with their children during foster care placement.
· Prepare parents and participate in Family Team Conferences including but not limited to Transitional, 20 Day, Six Month Service Plan Review, Trial Discharge and Final Discharge conferences.
· Report to Case Planning Supervisor and work alongside case planners.
· Collaborate with Co-Parenting Facilitator and all service providers directly responsible for care outcomes.
· Be trained by Rise as part of a group of parent advocates newly hired to work in foster care agencies, with a focus on healing and self-care; building knowledge of child welfare law, policy and practice; building relationships for support and mentorship with other advocates; and creating an ongoing support and advocacy network.
· Learn to use Rise's TIPS handouts and resources to inform and support parents.
· Contribute to defining and achieving success for this effort to build effective and meaningful roles for parent advocates in foster care agencies.
· Recognize and celebrate parents' progress toward reunification.
· Encourage parents to engage in supportive interventions that will strengthen their families.
· Help families navigate resources.
· Coach parents to strengthen problem-solving skills while discovering their strengths, hopes, dreams and priorities.
· Support and supervise family time between parents and children, as well as siblings and other significant resources as needed. Provide transportation as needed.
· Complete ongoing documentation in Connections with 7 business days of the event date.
· Work in partnership with the case planning team on completion of Permanency Hearing Reports and court documents as it pertains to the Case Aide's communication, contact, and collaboration with the family.
· Support families through the Public Assistance process, PATH process, and provide referrals, as needed.
· Attend all interdisciplinary team meetings and actively provide information regarding services being provided to the parent.
Fulfills duty of Mandated Reporter, which entails reporting any suspected instances of child abuse or maltreatment.
· Meet with Supervisor one-on-one on a consistent basis to discuss safety and permanency issues of caseload.
· Communicates and work effectively with SCO foster care support staff such as Case Planners, Socio-therapists, Fair Future Coaches and Education Specialists, and medical and mental health staff and with NYC Children's Services, outside medical and mental health centers, schools, consultants, other SCO programs, and all other relevant parties.
· Adheres to SCO's personnel policies.
· Performs other duties as assigned.
QUALIFICATIONS:
· Previous life experience being impacted by the child welfare system as a parent including having had a child placed in foster care or having had a court-ordered supervision case. Other prior child welfare involvement such as having personally experienced child welfare investigations may also qualify. Previous child welfare cases must be closed for at least one year.
· High School Diploma. Associates degree preferred.
· Approved background clearances and fingerprinting are required.
· Excellent oral and written communication.
· Basic computer literacy skills, including Microsoft Office.

Position Overview
The Resident Assistant serves to ensure a safe, secure, healthy, clean, and positive environment for families as they make their transition from homelessness through after care. This position assists families in service plan implementation by obtaining and providing feedback regarding their progress toward service plan goals. Position Summary: The Child Care Worker provides stability to the children and families of the Therapeutic After School Program (TASP) by providing a flexible response to the clinical and childcare needs of the program. The Child Care Worker will function as an integral part of the TASP team.
The role involves working with a group of up to 10 children. All of the children are at risk of placement outside the home, and our mission is to support them in a preventive and stabilizing capacity. Additionally, many of our kids have a traumatic background and may not respond to traditional methods of childcare work. Because of this, staff should expect to encounter challenging behaviors and be prepared to respond with patience and professionalism.
Responsibilities:
Provide safe and responsible transport for youth, families and other Children's Home affiliates as required by the program. Transportation responsibilities may be as much as three to four hours per day.
Develop and maintain therapeutic relationships with the children and their families that are responsive to their needs and are in accordance with the established treatment plan. Promote growth of the parent-child relationship as indicated by that plan, as well as the agency's philosophy of family centered practice.
Ensure the standards of a high performing team member are role modeled on a daily basis. Maintain good relationships through collaboration and advocacy on client's behalf with DSS, community agencies, schools, staff and other service providers.
Complete required reports and documentation in a timely manner, meeting all standards of content and professional presentation. Ensure timely, accurate recording of all components of clients' records, including: progress notes, treatment planning, case specific materials, Connections Case Management System, etc.
Use family-centered practice approaches in service delivery and case planning in treatment, permanency planning and discharge. Develop and implement meaningful and innovative best practices that are consistent with progressive trends, directions and research.
Continuously evaluate the strengths and needs of children and family to provide appropriate planning. Promote and guide the development of enhanced coping, socialization, parenting and independent living skills through the treatment planning process.
Provide for the safety and well-being of at-risk children, through the judicious use of discipline, therapeutic crisis intervention, CARE principles, and supervision. Effectively communicate all behavioral, emotional, and medical needs to the Director or the assigned Case Manager.
Provide for the overall growth and development of each child through instructing, counseling and direction in matters of personal conduct, hygiene, appearance, social skills, school, routines, and work habits. Foster the physical, spiritual and emotional needs of each child.
