ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
1. Prepare and develop fiscal operating and capital budgets for programs and departments of Northern Rivers and Member Agencies.
2. Maintain the PowerPlan Budgeting and Financial Projection System on a continual basis, specifically the processing of employee change forms, new hire paperwork, and terminations to keep the system updated in real time.
3. Process position requisitions in the payroll system to ensure proper coding and allocations based on the budget.
4. Prepare revenue and cost analysis on a regular basis as required by management, regulators, and funding sources.
5. Develop budgets and cost analysis for new programs and changes in funding and payment mechanisms.

Position Overview
The Client Outreach and Engagement staff members will act as the triage point for Law Enforcement Assisted Diversion (“LEAD”) in Albany, Cohoes, and Watervliet. As such, this position must ensure effective and efficient communication and collaboration between all partners involved in referring and receiving referrals and diversions for LEAD.
This position will provide person-centered case management in the community to individuals referred to LEAD. Case management services may include accompanying clients to necessary services such as homeless shelters, the Department of Social Services, court appointments, probation appointments, medical services, and meeting at a client's home or in places in the community that have been mutually agreed upon.
This position may also provide screening, outreach and engagement services on an as-needed basis. The Case Management staff members will conduct street outreach as needed to engage referred and diverted individuals who have yet to engage with their assigned case manager.
The Outreach and Engagement staff must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers in order to best serve LEAD participants.
Position Requirements:
High School Diploma or equivalent (GED/ TASC) required; College highly preferred.
One year of experience in homeless services.
Flexibility to work hours outside of regularly scheduled work hours, within reason.
Demonstrated ability to work well with clients and cohesively as a team member.
Previous experience working in a human services agency is a plus!
Clean and Valid New York State Driver's License; actively licensed for at least one (1) year with reliable transportation*.

Provide services as identified on the youth's Treatment/Service Plan following the specific frequency, scope, and duration.
Assist families to better understand and parent their child with mental health and developmental needs.
Attend all mandated training as assigned by their supervisor.
Regular and ongoing communication with other providers and family to advocate for a youth's needs, mental health wellbeing and any non-healthcare needs (ex. housing, employment, social support).
Timely and accurately document the delivery of services in an electronic health record.
Local travel required to provide the services in the youth home, school, or other community settings.
Maintain a positive, strength-based and person-centered approach at all times.
Provide other support and rehabilitative services as the agency determines, based upon new contracts or changing regulations.

Position Overview:
The Restorative Practice Coordinator (RPC) will coordinate and facilitate the establishment and implementation of Restorative Practices, to be utilized within the day treatment and residential programs at St. Catherine's Center for Children. The RPC will function as a leader as well as trainer, educating and consulting with staff surrounding trauma informed practices, and coaching staff during and post crisis in a manner that aligns with Restorative Justice.
Position Requirements:
A Bachelor's degree in Social Work from an accredited university or a Bachelor's degree in a related field is required.
Master's degree in Social Work from an accredited university social work program (M.S.W.) or a Master's degree in a related field is preferred.
Ability to relate to a wide range of people. This includes but not limited to a broad range of people who are culturally and socio-economically diverse, as well as people with various developmental and functional levels.
Demonstrated effective leadership qualities and confident supervisory skills.
Must be able to respond effectively and therapeutically in crisis situations.
Must possess excellent communication skills and solid writing aptitude.

The RPC has an interdependent relationship with the School Administration team, as well as the Residential Management Team to coordinate and implement the Restorative Practices framework across programs with consistency and fidelity.
Meets with supervisor to review progress toward professional goals, as well as the design and implementation of trauma informed systems and processes across programs.

Maintain consistent communication with all teachers to ensure the use of universal Tier 1 supports and that the appropriate steps are being taken to track behavioral data.
Be responsible for the integration of Restorative Justice concepts, language and practices into day to day functioning and situations.
Model for direct care staff how to appropriately respond to students, colleagues, parents, and other members of the community during states of emotional intensity.
Will attend meetings to provide a restorative lens and make behavioral strategy and intervention suggestions (i.e. Treatment Plan Meetings, Incident Deconstructions, etc.)
Will work collaboratively to ensure that Restorative Practices Model is being implemented with fidelity and consistency across programs for continuity of care.
In conjunction with the Residential Management Team, as well as the School Administration Team, will consult regarding the development and maintenance of therapeutic living and learning environments.
Will attend and participate in Administrative / Management meetings as requested, or on an as needed basis.
Partner with the School Culture and Environmental Skill Building Coordinator to integrate Restorative Practice language and concepts into the Social Emotional Learning curriculum to maximize the quality of the content emphasizing a trauma informed and community centered program.
As a member of the treatment team, provides clinical support to the team by being available for real time coaching and support of restorative practice philosophy in an effort to move students towards treatment goals and ultimately permanency.
Be responsible for periodic review of all Individual Crisis Support Plans to ensure Restorative Justice (RJ) and Trauma Informed language is utilized.
Will complete and submit necessary documentation to support the implementation of Restorative Practice, as well as record any incident, progress, regression, etc.
Participate in all agency required training, as well as maintain certification of training if applicable.
Attend and complete the Therapeutic Crisis Intervention Train the Trainer program and work collaboratively with the agency's training department to educate incoming and retained employees in agency approved crisis intervention techniques.

Along with child and family input, develops a Safety and Crisis Plan.

Maintains the Care Plan and monitors the completion of Care Plan goals.

Position Overview:

We are seeking to hire a Director of Day Treatment Clinical Services for six months to cover a leave of absence.

Collaborates with the Coordinator of Centralized Intake to arrange timely and comprehensive admissions process that ensures residential and day treatment staff are informed of information necessary for initial assessment and treatment planning.
Arranges for assigned social worker to meet with child, family and referring agency on admission day and facilitate admission procedure.
Participates in weekly meetings of Day Treatment Placement Team, considering new admissions, the children's transitions from one program to another, and continuum of care for least restrictive environment and/or return home to permanent family.
Ensures that assigned social work staff facilitate interdisciplinary treatment team meetings at initial (30 Day), quarterly, and other as needed intervals, for the children and families in the Day Treatment Department, according to the requirements of regulatory bodies and the policies and philosophy of St. Catherine's Center for Children.
Ensures that assigned social work staff complete and document psycho-social assessments, and treatment plans developed by treatment teams at Treatment Planning Meetings, at initial (30 Day), quarterly, and other as needed intervals, for the children and families in the Day Treatment Department, according to the requirements of regulatory bodies and the policies and philosophy of St. Catherine's Center for Children.
Ensures that treatment planning is focused on return to permanent home or to least restrictive setting as soon as possible, with provision of aftercare services when needed.
Actively engages parents in treatment planning and fosters partnership with parents in all Day Treatment operations.
Ensures that child, family members, referring agency, and other involved community members participate in the assessment and treatment planning process for each child/family served, collaborate in the development and implementation of the service plan, and that the needs and goals of all involved parties are addressed.
Collaborates with the R&E May School CSE Chair in the development and implementation of social work services as mandated in the IEP.
Ensures that social workers participate in the development and implementation of the FBA, BIP, ICSP, and Safety Plan, according to the requirements of regulatory bodies and the policies and philosophy of St. Catherine's Center for Children.
Ensures that social work staff provide individual, group, family, and case management services for assigned children and families as required by the treatment plan, including mandated IEP services.

Oversees implementation of Restorative Practice Model, Multi-Tier Systems of Support, and Therapeutic Crisis Intervention strategies.
Actively supports the goals of the agency and Day Treatment programs in preventing discipline problems and in teaching social skills.
Provides timely review and response for all incident reports involving social work staff, according to required agency and regulatory protocols/procedures.
Completes and submits NIMRS reports, as required by NYS OMH.
Ensures that required Post Crisis Response steps are conducted for youth, parents, and staff following critical incidents that involve social work staff.
Ensures active participation by social work staff in weekly meetings of the Classroom Team meetings.
Participates in monthly Quality Assurance and Critical Incident Review meetings with the Principal for Continuous Quality Improvement.

Position Overview
The Residential Case Manager (RCM) acts as the primary case manager and care coordinator for children placed in residential care. This case manager coordinates the facets of services for the child while in care -- medical services with the agency nursing department, mental health care with a therapist and psychiatrist, and educational matters with the children's school. Additionally, the RCM monitors and supports the milieu treatment and actively engages with and communicates with the house staff linked to the child's care, treatment, and safety.
Position Requirements
Ability to work: Monday - Friday, 8am-4pm OR 9am - 5pm w/ flexibility as needed.
A Bachelor's Degree in a human services, with a demonstrated capacity to work with people from diverse backgrounds, required.
Experience working with clients who exhibit significant emotional, behavioral, and interpersonal challenges; mental health diagnoses and trauma history, preferred.
Ability to provide safe transportation to a client in the worker's personal vehicle, in the event that an Agency vehicle is not available for needed transportation.

Essential Duties & Responsibilities include
Act as liaison between child, family, school, the treatment team, and the referring agency; and the primary point of contact for the local department of social services caseworker and the child's family.
Arrange and supervise all child/family visitations; secure signed releases and consents from the family or agency of legal custody, including permission for the special trips and events which are beyond routine residence/school activities.
Transport the child to visitation, court hearings, and other required meetings not occurring at the St. Catherine's Center for Children program site, when necessary.
Maintain regular contact and coordinating treatment with the Day Treatment School social worker when a child is enrolled in the SCCC day treatment school. When a client is enrolled in a school other than SCCC Day treatment, the RCM will maintain sufficient contact with that school to ensure the child is optimally benefiting from the educational placement.
Participate in Committee on Special Education (CSE) meetings and maintain contact with the SCCC and CSE chairperson.
Liaise with child care staff and residence clinical staff through participation on the treatment team and during treatment reviews, service plan reviews, medication reviews, and staff meetings.
Document treatment and casework as required by SCCC and regulating agencies including:
Completion of thorough and up-to-date progress notes in the electronic record, New York State (NYS) Connections.
Development of the service plan in Connections, given the assigned role and duties.
Court reports, correspondence with other agencies, social summaries.
Maintenance of accurate, up-to-date data on the child and family.
Completion of quarterly treatment reviews.
Updating of permission release forms requiring parent signatures.
Collection and/or preparation of all materials necessary to bring records into compliance with internal agency and external regulations.
In conjunction with the House Management Team, develop and maintain a therapeutic environment at the site.
Implement the principles of motivational interviewing as an evidence-based practice as part of the therapeutic milieu at the program site and supporting activities for the children and families.
Attend all court hearings pertaining to the placement, permanency, and treatment of the child and family.
Directly observe all visits that require supervision as deemed by the county or court.
As the primary liaison for the child/family, maintain flexible work hours, at the supervisor's direction, to include evening hours outside of the traditional work day to accommodate family schedules if necessary. This may also include weekend hours on occasion.
Assist, as needed, in the agency's incorporation of elements of Family First Prevention Services legislation, including discharge planning, aftercare services, and ongoing changes requested by the NYS Office of Children and Family Services linked to the legislation; train in the Family Finding model.
Develop the discharge plan for each assigned child, identifying the resources to promote a smooth transition out of care; participate in discharge activities to ensure solid case closure.
Assume the role of Primary Contact Person, as assigned, to coordinate the discharge and aftercare activities that are part of agency protocols, completing assigned aftercare tasks.
Function as a member of the direct care planning team, meeting with RMT members, unit child care supervisors, consultants, nurses, and social workers to evaluate and plan for the therapeutic milieu.
Assume additional position-related duties and responsibilities as assigned by the Supervisor of Residential Case Management.

Position Overview:
The Restorative Practice Coordinator (RPC) will coordinate and facilitate the establishment and implementation of Restorative Practices, to be utilized within the day treatment and residential programs at St. Catherine's Center for Children. The RPC will function as a leader as well as trainer, educating and consulting with staff surrounding trauma informed practices, and coaching staff during and post crisis in a manner that aligns with Restorative Justice.
Position Requirements:
A Bachelor's degree in Social Work from an accredited university or a Bachelor's degree in a related field is required.
Master's degree in Social Work from an accredited university social work program (M.S.W.) or a Master's degree in a related field is preferred.
Ability to relate to a wide range of people. This includes but not limited to a broad range of people who are culturally and socio-economically diverse, as well as people with various developmental and functional levels.

Will attend meetings to provide a restorative lens and make behavioral strategy and intervention suggestions (i.e. Treatment Plan Meetings, Incident Deconstructions, etc.)
Will work collaboratively to ensure that Restorative Practices Model is being implemented with fidelity and consistency across programs for continuity of care.
In conjunction with the Residential Management Team, as well as the School Administration Team, will consult regarding the development and maintenance of therapeutic living and learning environments.

Partner with the School Culture and Environmental Skill Building Coordinator to integrate Restorative Practice language and concepts into the Social Emotional Learning curriculum to maximize the quality of the content emphasizing a trauma informed and community centered program.
As a member of the treatment team, provides clinical support to the team by being available for real time coaching and support of restorative practice philosophy in an effort to move students towards treatment goals and ultimately permanency.
Be responsible for periodic review of all Individual Crisis Support Plans to ensure Restorative Justice (RJ) and Trauma Informed language is utilized.
Will complete and submit necessary documentation to support the implementation of Restorative Practice, as well as record any incident, progress, regression, etc.
Participate in all agency required training, as well as maintain certification of training if applicable.
Attend and complete the Therapeutic Crisis Intervention Train the Trainer program and work collaboratively with the agency's training department to educate incoming and retained employees in agency approved crisis intervention techniques.

Serve as a bridge between families, service providers and the CR staff, supporting a productive and respectful partnership by assisting the families to express their strengths, needs and goals.
Connect families with appropriate services and supports, including community exposure and family engagement with entire CR team.
Supporting and assisting families during ALL stages of transition.
Promote continuity of engagement and supports as families' needs and service change.
Promote self-advocacy, self-efficacy, and empowerment in families.

II. QUALIFICATIONS
· Family Peer Support will be delivered by a New York State Credentialed Family Peer Advocate (FPA).
§ STEPS TO OBTAIN A FAMILY PEER CREDENTIAL
· Demonstrated “lived experience” as a parent or primary caregiver who has navigated multiple child serving systems on behalf of their child(ren) with social, emotional, developmental, health and/or behavioral healthcare needs. Preferred, lived experience with out-of-home placement.
1. A high school diploma, high school equivalency preferred or a State Education Commencement Credential (e.g. SACC or CDOS). This educational requirement can be waived by the State if the person has demonstrated competencies and has relevant life experience sufficient for the peer credential.
2. Complete Level One of the Parent Empowerment Program Training for Family Peer Advocates or approved comparable training.
3. Submit two letters of reference attesting to proficiency in and suitability for the role of a Family Peer Advocate (FPA).
4. Agree to practice according to the Family Peer Advocate Code of Ethics. An FPA with a Provisional Family Peer Advocate Credential must complete all other requirements of the Professional Family Peer Advocate credential within 18 months of commencing employment as a FPA.
· Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
· Must have a valid Driver's License and a satisfactory driving record if driving is a requirement for the position.
· Must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search.
· Must be cleared (not have any active or pending cases) by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.

Position Overview
The Community Based Services Provider (“CBSP”) provides a range of behavioral health support services to youth within our Home and Community Based Services (“HCBS”) program. Youth in this program have identified, or are at risk of, mental health, substance abuse, or developmental challenges. Essential Duties & Responsibilities include:
Meets regularly with families to assist with service plan goals, offering education and instruction on daily living skills; Empowering residents to identify better choices in their daily lives.
Ensures a safe, secure and healthy environment for all residents.
Performs routine and ad hoc documentation, using critical incident reports, intake information forms, progress notes in case files, logs, etc.
Assists with orderly transition of families, from intake through discharge.
Intervenes in crisis situations and assists with conflict resolution, as needed, by providing mediation and facilitating communication.
Participates in professional development programming.
Transport clients in agency vehicles to various destinations throughout the Capital Region, based on families' needs.
Maintain CPR/First Aid Certification.

Caseworker with Child (30%)
Maintains on-going qualitative social work/child, and social/childcare contacts in accordance with regulatory mandates. Lobbies and advocates energetically for the appropriate education/vocational placement for each resident. Prepares cases for presentation in six-month interdisciplinary case reviews and participates actively in the development of comprehensive service plan. Serves as youngster's advocate and represents in collaborative service planning with Department of Mental Health and Youth Development Services and the program's Nursing and Program Management Units. Facilitates case movement, which seeks the placement of each youngster in the most appropriate and least restrictive setting for required services.
Caseworker with Family (30%)
Maintains and facilitates on-going qualitative social work/family, family/child, and siblings contacts in accordance with regulatory mandates. Executes diligent efforts to locate family members of each youngster in the program. Implements creative strategies for involving family members in the service planning and delivery process. Lobbies energetically and facilitate the provision of family therapy and other supportive collateral services when indicated. Facilitates the development of realistic appropriate and timely discharge plans. Promotes the principles of the family-to-family initiative through partnership with direct care staff, birth parents and youth and neighborhood residents. Helps to establish communities of care for youth and families by building effective relationships with significant individuals and service providers in the location where the family resides.
Casework with Collateral Persons (15%)
Maintains regular contact and efficacious working relationships with significant others involved in the service planning and provision process, i.e. child care staff, educational personnel, court and legal representatives, foster parents, medical personnel and other caseworkers involved. Lobbies energetically and advocates for services which will empower and enhance client functioning. Assures a holistic, culturally sensitive, family centered and child-focused presentation of case particulars. Represents the agency in neighborhood service network meetings and other forums and participates in other community educational activities.
Record keeping (25%)
Maintains accurate and up-to-date electronic and paper case records. Complete, accurate and timely documentation of casework activities is a critical element of this position. Facilitates regulatory compliance with City and State standards of practices. Completes and submits all necessary reports (e.g., service plans, permanency hearing reports, progress notes), as required, in a timely fashion.

QUALIFICATIONS:
Experience required:
A record of success and achievement in foster boarding home or residential childcare work is helpful but not required. Demonstrated competency in the knowledge of the Family-to Family service model is desirable.
Education:
A bachelor's degree in social work or related field from an accredited college or university is preferred. Degrees in other disciplines are given full consideration.
Special Skills:
Familiarity with the community to which assigned and knowledgeable about the major culture groups within the neighborhoods served. The following qualifications are desirable: residence in the community in which the position is located; from the culture and ability to speak the language of the major groups served in the community. Job Overview:
The Foster Care Case Aide provides administrative and operational support for the agency's Foster Care Program. The Case Aide assists with the logistics of departmental operations, including family visits and service plan review meetings, supporting the transportation, care, and supervision of children during these events. This vital position assists the Family Specialists in ensuring that successful case activities occur for our foster care families and foster parents.
Transportation may also be needed for a child to access medical appointments, educational events, mental health counseling appointments, and visits with a variety of family members. During these activities, the Case Aide works closely with the foster care family specialists and receives their support and guidance. Additionally, this key individual provides administrative support to maximize the department's efficiency in overall operations and administrative functioning.
