The Program Administrator is responsible to ensure that overall, the following is completed: assessments, development, implementation, management, and delivery of Program Services. DUTIES AND RESPONSIBILITIES OF THE PROGRAM ADMINISTRATOR:
Assure all programs are compliant and accountable with all regulatory requirements.
Supervise assigned Director-level employees with program compliance and accountability.
Coordinate and train Director-level employees in applicable licensing regulations and PMH policy and procedures.
Provide on-going reviews of individual charts for completeness and compliance with regulatory requirements.
Supervise, monitor and evaluate services.
On-call for program emergencies and support.
Participate in the IP process, development, team reviews, and implementation in accordance with the 6400 regulations.
Oversees development and implementation of the Individual Program Plans.
Coordination of Licensing Reviews for ID and Behavioral Health Programs.
Perform other duties and responsibilities by the Assistant Executive Director.
REQUIRED SKILLS AND KNOWLEDGE OF THE PROGRAM ADMINISTRATOR:
Minimum of five years supervisory experience.
Valid driver's license.
Master's degree or higher from an accredited college or university and five (5) years' experience working directly with individuals with developmental disabilities.
OR
Bachelor's degree from an accredited college or university and ten (10) years' experience working directly with individuals with developmental disabilities.

DUTIES AND RESPONSIBILITIES OF THE PROGRAM SPECIALIST:
Provide and supervise activities for the individuals in accordance with their specific IP, including opportunities for community integration.
Coordinate and provide oversight for the responsibilities of Direct Support Professionals.
Complete attendance and payroll records for Direct Support Professionals.
Hold regular meetings with Direct Support Professionals to review program needs/progress and provide necessary training.
Assure budgetary compliance and fiscal accountability for all residential programs.
Ensure program adherence to established policies and procedures.
Provide the assessment for the development of the IP, the IP Annual update, and the IP revisions as required under CH. 6400.181(a).
Participate in the IP process, development, team reviews, and implementation in accordance with the 6400 regulations.
Perform other duties and responsibilities by the Program Operations Director.
REQUIRED SKILLS AND KNOWLEDGE OF THE PROGRAM SPECIALIST:

The Triage Nurse will perform selected nursing acts in the care of the individuals Passavant Memorial Homes serves in a safe, competent, and humane manner, and will report to work in-office at the Harmar location.

DUTIES & RESPONSIBILITIES OF THE TRIAGE NURSE:
Perform clinical nursing skills, with knowledge and understanding of fundamentals.
On-site monitoring of the community residential settings as required.
Monitor staff completion of new medication orders and documentation.
Provide on-going assessment and planning to assure health needs are met.
Administer injections as needed (i.e. Depo-Provera shots, Hepatitis B vaccines, TDap Boosters).
Complete dietary in-service for specialized diets and send to community homes.
Filing of labs, physicians orders, consults and other paperwork relative to consumers.
Communicate daily with hospital staff and provide updates to the Clinical Services Director and Regional Program Directors.
Complete other duties as assigned by the Clinical Services Director.
REQUIRED SKILLS AND KNOWLEDGE OF THE TRIAGE NURSE:
Graduate of an accredited school of professional and/or practical nursing, with a current license by examination from the State of Pennsylvania, required.
Valid PA Driver's License, required.
An understanding of the Intellectual Disabilities field, preferred.
One (1) to two (2) years' experience in medical/surgical or geriatric nursing, preferred.

Perform the tasks outlined in the individual's Individual Plan (IP).
Complete the assigned tasks in the manner in which they are requested.
Perform the requested services for the individual within the units authorized within the IP.
Deliver services in a manner consistent with the policies and procedures developed and provided by Passavant Memorial Homes.
Provide supervision and positive role modeling at home and in the community.
Complete the necessary Pre-Service and Annual In-Service training planned by the individual/representative and facilitated by HCS.
Observe the individuals' behaviors and changes in physical condition, and report accurately.
REQUIRED SKILLS AND KNOWLEDGE OF THE ISO SUPPORT SERVICE WORKER/CAREGIVER:
Minimum of 18 years of age, required.
Valid driver's license, required.
Current car insurance and registration, required (position requires use of personal vehicle).
Successfully pass background check, required.
Experience working directly with individuals with developmental disabilities, preferred.
