QUALIFICATIONS:
High School Diploma or GED. Maintenance and Housekeeping Experience at a commercial and residential facilities.
SPECIFIC DUTIES:
Corporate Sites:
General Cleaning:
Clean and disinfect office spaces, conference rooms, and shared areas.
Dust and wipe down equipment, including screens, remotes, and projectors.
Reorganize furniture to the standard layout.
Dust and wipe down surfaces, including desks, counters, shelves and furniture.
Vacuum, sweep, and mop floors daily. Spot-clean entryways and lobbies throughout the day to address heavy foot traffic.
Organize and straighten up shared office items (i.e. chairs, desks).
Restroom Maintenance:
Sanitize restrooms, replenish supplies (soap, paper towels, and tissue).
Ensure restrooms are fully stocked and in clean, operable condition at all times.
Clean and disinfect toilets, urinals, sinks, and countertops.
Polish mirrors and chrome fixtures.
Mop restroom floors with disinfectant and ensure they are dry to prevent slipping.
Breakrooms and Kitchens
Clean countertops, sinks, and tabletops.
Wipe down appliances, including microwaves, refrigerators, and coffee machines.
On a rotational schedule clean inside appliances and dispose of expired or no longer working content
Empty trash cans and replace liners.
Sweep and mop floors to remove food or drink spills
Reception and Lobby Areas
Dust and clean furniture, decorative items, and reception desks.
Vacuum or mop floors to maintain a welcoming appearance.
Clean glass doors, partitions, and windows to remove fingerprints and smudges.
Waste Management:
Collect trash and recycling from individual offices, meeting rooms, and common areas.
Empty trash and recycling bins and transport them to designated disposal areas.
Replace liners and clean trash receptacles as needed.
Special Tasks:
Assist with light maintenance tasks such as changing light bulbs or restocking janitorial supplies.
Support event setups or breakdowns, including arranging tables and chairs.
Clean outdoor areas near entrances, including walkways or steps.
Perform a walk-through of the site to identify priority cleaning areas.
Maintain records of cleaning schedules and tasks completed.
From time to time support facilities within the FMX ticketing software to address custodial issues.
Health and Safety:
Follow cleaning protocols to maintain compliance with workplace hygiene standards and safety protocols.
Identify and report potential hazards, such as spills or equipment malfunctions.
Ensure familiarity with emergency exits and protocols for fire alarms, power outages, or flooding.
Ensure cleaning materials will be safely stored in a locked area with their original label on the product when not in use
Residential Sites:
1. Common Areas:
Provide deep cleaning and emergency support to Residential Sites, case by case.
2. Vacant Units:
Deep-clean vacant units to prepare for new tenants.

JOB SUMMARY:

QUALIFICATIONS:
High School graduate with 2 years of increasingly responsible administrative experience, Associates Degree or Bachelor's degree, and at least 6 months of experience.

Specific Duties:
Respond to/Process HR requests via electronic ticketing system on daily basis, including processing of employee actions, such as new hires, transfers, performance awards, promotions, demotions, resignations, terminations, leaves of absence, suspensions and/or 48-hour letters, etc.
Disseminate notices for employee child abuse clearances, employee health appraisals, Federal Bureau of Investigation finger printing, driver licenses, licenses and certificates monthly and distribute to appropriate supervisors.
Maintain electronic employee records in accordance with accurate naming convention and confidentiality.
Collaborate with recruitment staff to post job ads on careers pages and process incoming resumes.
Prepare and complete all written requests for verification of employment (mortgage, employment, welfare, childcare, garnishment, etc.)
Provide additional support for securing child abuse clearance, criminal history checks, transcripts, and any necessary credentialing documents for new hires and current staff.

QUALIFICATIONS:
Bachelor's degree in Human Services or related field required;
Minimum of two years experience working in human services with some quality improvement preferred;

Coordinate crisis and case management services with other staff in COC/Outpatient Department
Function as a member of the agency's internal crisis team and after successful completion of mandatory training restrain, or place in restraints, those individuals who are unable to control themselves and present danger to themselves or others
Participate in crisis assessment to ensure individual receives necessary supportive intervention and assistance in managing and resolving the crisis and develop a crisis plan to assure coordination of recommended disposition
Notify supervisory staff of emergencies and urgent issues in a timely manner
Complete or assist others in completing an involuntary commitment petition
Make approved home visits
Complete Daily Case Conference and ensure timely follow up
Ensure all individuals seen through intake department are referred to appropriate services through regular review of the waitlist for each department
Communicate with your supervisor about individuals needing higher level review
Assist individuals with gaining access to needed psychiatric, medical, social, residential financial and other services as appropriate for the individual
Ensure collaboration with individuals' treatment teams, including the psychiatrist when coordinating care

CMCRT provides crisis intervention, problem resolution, support, linkages and safety plans for adults in Northeast Philadelphia. The teams also provide assistance, when needed, to other areas of Philadelphia.
QUALIFICATIONS:
Master's Degree in Behavioral Health Discipline ; At least three years of experience in the provision of behavioral health support, treatment and/or rehabilitation services; Licensed Clinical Social Worker, Licensed Social Worker or Licensed Professional Counselor; Experience using an Electronic Health Record, skilled with Microsoft Office software including Excel; Good communication and writing skills
SPECIFIC DUTIES:
Provide oversight of the daily operation of the CMCRT program
Assure staff receive ongoing training to increase clinical skills to provide crisis intervention services and that evidence-based practices are utilized, including Resolution Focused Crisis Intervention
Provide weekly direct clinical supervision of Supervisors and assigned team members. Supervision to include joining staff when responding to crisis calls to observe skills in the field.

SPECIFIC DUTIES:
Provide crisis assessment, intervention, education, and support in person, with assigned team member, within a designated time frame to referrals received by DBH/CBH.
As a CPS, share lived experience to connect with and provide support to individuals in crisis and aid in developing a crisis support plan.
Utilize approved evidence-based practices and tools when providing crisis assessment and intervention, including SAMHSA crisis intervention guidelines and resolution models.
Assure all interventions are person-centered and driven by the individual and their family/supports.
Assist the family/supports, if release of information allows, in the understanding of the events that led to the crisis from everyone's perspective.
Assist the individual in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation, and education through linkage to resources and additional supports.
To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed, and the plan has been developed.
Assure appointments are scheduled at a time and place convenient for the individual.
To complete or assist others in completing involuntary commitment petitions.
To arrange and follow through with voluntary or involuntary hospitalization admissions.
To follow all procedures regarding the initiation of a commitment.
Link the individual to the CRC or other 24/7 crisis staff and remain with them until the connection and collaboration is completed.
Develop with the individual and their family/supports, if available, a safety/ crisis plan obtaining everyone's perspective, strengths and ideas.
Be knowledgeable of local community resources and services and provide care coordination.
Provide up to 72 hours of intervention by scheduling follow up appointments at a time convenient for the individual, which includes evening and weekend appointments in the home and community.

Schedule appointments in consultation with the nurse; Communicate scheduled appointments to all relevant parties. Assist in preparation of individuals for medical appointments. Accompany and transport individuals to and from medical appointments. In consultation with nurse and appropriate team members, develop appropriate questions to ask and identify relevant information to bring to the medical appointment.
After Appointments:
Share and document information with the nurse and other relevant team members following medical appointment. In consultation with nurse and other relevant team members, complete follow-up as recommended. JOB SUMMARY:

JOB SUMMARY:

JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. The RTA provides direct care including support, supervision, structure, assistance, skill training and therapeutic interventions, in groups and with individuals, in collaboration with RTF-A treatment team. 21.Participate in scheduled meetings, including stakeholder's meetings, case conferences and community meetings, as well as staff meetings and supervision.
22.As assigned, schedule necessary medical and dental appointments for residents.
23.Assist residents in maintaining overall health by: becoming knowledgeable about their health histories, accompanying them to appointments and emergency services as needed, and encouraging individuals to follow healthy living choices and wellness recommendations from their doctors.
24.Collaborate with jobs, schools or other programs residents attend.
25.Ensure all required documentation is completed in timely, comprehensive and legible manner and in accordance with PATH policies and procedures. This includes, prior to the end of shift, ensuring shift notes are completed and document residents' progress towards their goals, as well as completing incident reports for any non-routine incidents, including noting when supervisor was informed.
