Requirements:
Education: Bachelor's degree in social work, counseling, marriage and family or psychology from an accredited college or university
Previous Experience/Work History: 1 year experience in the CASSP system and experience in provision of trauma focused interventions/training preferred

JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. To work as a team member with a therapist on a specialty trauma team, to provide intensive family based services, utilizing the Ecosystemic Structural Family Therapy (ESFT) model, delivered primarily in the family's home to include: assessments, service coordination and linkages, crisis intervention and prevention, educational and daily living skills, collaboration with all systems involved with the youth and family and assist the therapist in the delivery of individual, family and group therapy.
SPECIFIC DUTIES:
Provide initial and on-going comprehensive assessments of identified families and youth, with an emphasis on assessment of trauma and the impact on the family and individual.
Assist the therapist in the provision of individual, family, and group therapy with identified families
Work collaboratively with assigned therapist to provide comprehensive services and linkages
Provide 24/7 on-call coverage on a rotating basis
Deliver services in a manner convenient to families which includes evening and weekend service provision
Consult with psychiatrist on a regular basis and attend psychiatric appointments with the youth and family
Collaborate on a regular basis with all systems involved with the youth and family
Provide on-going role modeling to youth and families
Provide crisis assessment, intervention and prevention
Develop and implement socialization and daily living skills training and activities for the youth and family
Provide case management services and linkages to all needed services for the youth and family
Attend and participate in required ecosystemic family therapy trainings in addition to required state and local trainings, to include enhanced training in trauma focused therapy and interventions
Demonstrate commitment to learning the Ecosystemic Structural Family Therapy (ESFT) model, by bringing videotaped sessions to supervision, learning genograms, eco-maps, structural maps and assure that identified traumatic events are included on the family timeline.
Present cases both to peers at PATH, Inc, and at training program, as requested
Complete all documentation in a timely, comprehensive manner
Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned

In this position, you will deliver direct consumer care under the supervision of the Registered Nurse. You will also report changes in consumers' conditions to the Registered Nurse. As an aide, you will assist with mobility, transport consumers, monitor consumers during activities, and assist with consumer identification. You can expect to provide nursing care that may require special levels of observation and the therapeutic use of physical restraint. Your work will also involve responding to psychiatric and medical emergencies. Additionally, you will participate in ongoing training, maintain confidentiality of consumer information, and interact positively and constructively with consumers' families and fellow staff.

JOB SUMMARY:

JOB SUMMARY:

5.Provide crisis intervention and management as indicated

13.Ensure individuals gain access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual
14.Ensure individuals have access and availability to meaningful activities appropriate to their ability and interests and schedule opportunities for residents to engage in community integration activities
15.Facilitate recreational and/or psych rehab activities for residents who are not linked with outside treatment or activities
16.Ensure all documentation is completed in a timely, comprehensive and legible manner and is in accordance with PATH policies and procedures

26.Participate in the integration of physical and mental health; maintain knowledge of program residents' health care needs, providing advocacy and education as needed

With the Director, provide clinical guidance and leadership to the BHID specialized team and residential direct support staff. The BHID Specialized Team provides direct care including support, structure, assistance, skill training and therapeutic interventions, for service participants, in collaboration with BHID residential staff. Services are provided in the home and community.
QUALIFICATIONS:
Master's degree from an accredited college/university in the human services field required.
Experience working with individuals with psychiatric disorders and/or Intellectual disabilities.
PHYSICAL DEMANDS:
Must be able to perform crisis management interventions, CPR/First Aid, and assist individuals with mobility issues as needed.

JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by providing quality services and being an effective team member. This position has the responsibility to carry out treatment, rehabilitation and support functions, psychoeducation, and provide crisis intervention to residents, as well as, support and consultation to their families/supports. Recovery principles, evidence-based practices and trauma informed care is to be used in the provision of services. Provide service coordination for assigned residents including coordinating and monitoring the implementation of their treatment and rehabilitation plan by the residential team.
SPECIFIC DUTIES:
1.Provide individual, group and/or family therapy at least weekly for all residents.

9.Monitor residential site to ensure health and safety of residents
10.Train residential staff in providing appropriate therapeutic responses to residents that are consistent with their treatment and rehabilitation plans.
11.Assist in interviews and evaluation of individuals referred for admission into the program. Complete biopsychosocial evaluations for potential admissions as assigned.
12.Complete all clinical paperwork for assessment, treatment planning, and interventions.
13.Ensure individuals gain access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual
14.Ensure individuals have access and availability to meaningful activities appropriate to their ability and interests and schedule opportunities for residents to engage in community integration activities
15.Facilitate recreational and/or psych rehab activities for residents who are not linked with outside treatment or activities
16.Ensure all documentation is completed in a timely, comprehensive and legible manner and is in accordance with PATH policies and procedures
17.Complete all documentation required for continued stay and communicate authorizations to PATH's QI department in a timely manner.
18.Complete monthly individual service plans in a timely manner
19.Complete individual treatment plans every 90 days or sooner as clinical issues arise
20.Participate in weekly treatment team meetings, which may include the program psychiatrist
21.Consult and collaborate with all team members on a regular basis
22.Participate in and document summary of team meetings, case conferences, stakeholder meetings and other meetings as assigned
23.Contribute to quality improvement/outcome measurement tracking and data gathering as assigned
24.Provide, on a rotating basis, Residential on-call support, 24 hours/7 days a week to provide advice to residential staff which includes responding to phone calls within 10 minutes and providing face to face intervention as indicated to resolve a crisis
25.Assist in training program staff in implementing, and consistently using, evidenced-based practices
26.Participate in the integration of physical and mental health; maintain knowledge of program residents' health care needs, providing advocacy and education as needed

RESPONSIBILITIES:
DRUG AND ALCOHOL (D&A) PROGRAM
Involves providing individual counseling, group counseling, and care management for people of all ages with substance use and other mental disorders.
Provides psychosocial assessment; ongoing individual, family, and group counseling; client/family education; and care management for cases assigned.
Participates in regularly scheduled clinical supervision with assigned supervisor.
Maintains timely and adequate documentation (including reports, summaries, and letters) on each client assigned.
Works in close cooperation with other social service agencies, hospitals, and community resources toward planning and initializing services with clients.
