JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Work primarily with adults with serious mental illness or emotional disorders, assisting individuals to make full use of Behavioral Health Services and natural community supports to achieve a more independent and fulfilling life. Services provided will include developing a partnership with the individual to assess needs, obtain services, and resolve crises.
QUALIFICATIONS:
Bachelor's Degree in Human Services field from an accredited college/university
One to two years experience in the behavioral health field
Valid driver's license required
Experience working with adults
Good interpersonal and organizational skills
Ability to use electronic health record required
SPECIFIC DUTIES:
Develop partnerships with assigned individuals to assess needs, obtain services and resolve crises
Assist individuals with identifying goals and defining the steps needed to achieve their goals
Complete all assessment and planning documentation including Adult Psychosocial Assessment; Strengths Assessment; Treatment Plans and other required documentation
Ensure all required documentation is completed in a timely, comprehensive, and legible manner and is in compliance with PATH policies and procedures
Assist in linking individuals with their chosen services and resources in the community such as needed psychiatric and medical treatment, residential services, financial entitlements, social/recreational activities, and general social services

JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. The Family Based Mental Health Worker will work as a team member with a therapist to provide intensive family based services, delivered primarily in the family's home to include: assessments, service coordination and linkages, crisis intervention and prevention, educational and daily living skills, collaboration with all systems involved with the youth and family and assist the therapist in the delivery of individual, family and group therapy.
QUALIFICATIONS:
Bachelor's degree in social work, counseling, marriage and family or psychology from an accredited college or university.
1 year experience working with children & families.
Must have valid driver's license.
Good interpersonal skills.
SPECIFIC DUTIES:
Provide initial and on-going comprehensive assessments of identified families and youth
Assist the therapist in the provision of individual, family, and group therapy with identified families
Collaborate on a regular basis with all systems involved with the youth and family
Develop and implement socialization and daily living skills training and activities for the youth and family
Provide case management services and linkages to all needed services for the youth and family

PURPOSE:
Provide direct assistance to individuals with a behavioral health disorder, in gaining access to critical mental health, medical, social, educational, and other agencies or resources whose functions are to provide the support, training and assistance required for a stable, safe, and healthy life within their community.
FUNCTIONS:
Provide BCM services to a caseload that will not exceed 30 individuals, meeting minimum performance and productivity standards as identified by BCM Supervisor.
Provide proactive outreach to the individual in their home, school, and community and engage with, empower, and advocate for needs.
Conduct ongoing assessment and assist individual with identifying strengths and needs.
Develop a personal goal plan in collaboration with the individual and any identified supports and routinely monitor progress.
Provide psychoeducation on behavioral health diagnosis and symptoms; respond to questions or concerns about their course of treatment including potential side effects from prescribed psychotropic medication.
Provide linkage to identified resources with individual's consent. This may include facilitating referrals for expanded mental health programming.
Assist in developing Activities of Daily Living (ADL) skills (e.g., developing a system for remembering to take medication as prescribed, navigating public transportation, budgeting, housekeeping, etc.)
Serve as a clinical liaison and coordinate care between all service providers and systems serving individual which may include but is not limited to: behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments.
As appropriate, assist clients with applying for MA and Social Security applications or troubleshooting other insurance barriers that impact access to care. This may involve accompanying clients to appointments at Public Assistance and Social Security offices.
Provide 24-hour crisis intervention on a rotating basis. Coordinate care with CRC or psychiatric hospital. This may involve accompanying the individual to the CRC or hospital.
Ensure continuity of care when individual is moving from one level of care to the next to prevent gaps in service coverage. This is especially critical for an adult being discharged from an acute setting.
Actively participate in all staff meetings and provide consultation and support to team members as needed.
Coordinate and lead interagency meetings for assigned cases for the purpose of service coordination and treatment and aftercare planning.
Maintain required records and documentation in compliance with Agency, county, state, and Managed Health Care regulations.
Maintains collaborative working relationships with service providers.
Participate in professional development as assigned, including mandatory annual trainings and 10 hours of elective trainings per year as recommended by BCM supervisor.
Function in compliance with agency policies/procedures and federal, state and local laws.
Participate in Continuous Quality Improvement (CQI) activities as requested by BCM Supervisor or Compliance Officer.
Other duties as assigned.
QUALIFICATIONS:
BCMs must meet one of the following criteria:
A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
Be a registered nurse.
A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in human services with 1 year in direct client contact, preferably with adults.
A high school diploma and 5 years of mental health direct care experience in human services with employment as an ICM staff person prior to 4/1/89.
Must have a valid Childline, PA Criminal History, FBI Clearances prior to seeing clients.
Requires reliable means of transportation and valid driver's license from state of residence with a minimum of one (1) year driving experience and a driving record free of at-fault accidents or moving violations for the past year.
Must be available to work some evening hours.

PURPOSE:
Provide direct assistance to children and adolescents with a behavioral health disorder, in gaining access to critical mental health, medical, social, educational, and other supportive resources within their community.
FUNCTIONS
Provide Blended Case Management (BCM) services to a caseload that will not exceed 25 youth and their families, meeting minimum performance and productivity standards as identified by BCM Supervisor.
Provide proactive outreach to youth and family in their home, school, and community and engage with, empower, and advocate for assigned youth and family.
Conduct ongoing assessment of the youth and family's strengths and needs.
Develop a comprehensive service coordination plan in collaboration with youth, family, and treatment team and routinely monitor progress.
Provide linkage to identified resources with youth and family consent. This may include facilitating referrals for expanded mental health programming including EPSDT, IBHS, Psychiatry, FBS, etc.
Serve as a clinical liaison and coordinate care between all service providers and systems serving youth and family. This may include but is not limited to: behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments.
As appropriate, assist clients with applying for MA and Social Security applications or troubleshooting other insurance barriers that impact access to care. This may involve accompanying clients to appointments at Public Assistance and Social Security offices.
Provide 24-hour crisis intervention on a rotating basis. This involves coordinating care with mobile crisis, the Philadelphia Children's Crisis Response Center (PCCRC), or psychiatric hospital. This may involve accompanying the youth and caregiver to the PCCRC or hospital.
Ensure continuity of care when youth are moving from one level of care to the next to prevent gaps in service coverage. This is especially critical for youth moving from out of home placement (e.g., foster care, group home, RTF, AIP) back home.
Actively participate in all staff meetings and provide consultation and support to team members as needed.
Coordinate and lead interagency meetings for assigned cases for the purpose of service coordination and treatment and aftercare planning.
Maintain required records and documentation in compliance with Agency, county, state, and Managed Health Care regulations.
Maintains collaborative working relationships with service providers.
Participate in professional development as assigned, including mandatory annual trainings and 10 hours of elective trainings per year as recommended by BCM supervisor.
Function in compliance with agency policies/procedures and federal, state and local law.
Participate in Continuous Quality Improvement (CQI) activities as requested by BCM Supervisor or Compliance Officer.
Other duties as assigned.
QUALIFICATIONS:
BCMs must meet one of the following criteria:
A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
Be a registered nurse.
A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in human services with 1 year in direct client contact, preferably with children.
A high school diploma and 5 years of mental health direct care experience in human services with employment as an ICM staff person prior to 4/1/89.
2. Must have a valid Childline, PA Criminal History and FBI Clearances prior to commencing work.
3. Requires reliable means of transportation and valid driver's license from state of residence with a minimum of one (1) year driving experience.
4. Must be available to work some evening hours.

JOB SUMMARY:

1.Provide supervision of residents at all times, unless the resident is using independence time.

QUALIFICATIONS:

Personal experience as a caregiver to a child involved with behavioral health services, juvenile justice, child welfare or special education.
Active Driver's license and personal car required.

Accompany crisis worker when dispatched and assist in engaging the family and youth; provide crisis interventions and provide added support to the family/caregiver.

To work as a team member to provide crisis intervention, support, safety plans and linkages to youth and families in Northeast Philadelphia, Region 1. The program operates 365 days/year from 8:00am - 10:00pm with rotating on-call coverage from 10:00pm - 8:00am.
QUALIFICATIONS:
Bachelor's degree in social work, counseling, marriage and family or psychology from an accredited college or university.
2 years experience in the CASSP system.
Good interpersonal and communication skills.
Active Driver's License and vehicle required.
SPECIFIC DUTIES:
Provide crisis assessment, intervention , education and support in person, with assigned team member, within a designated time frame to referrals received by DBH/CBH.
Utilize approved evidence based practices and tools when providing crisis assessment and intervention, including SAMHSA crisis intervention guidelines
Assure all interventions are family focused and family driven.
Assist the family in the understanding of the events that led to the crisis from everyone's perspective.
Assist the child/youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation and education.
To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed and the plan has been developed.
Assure appointments are scheduled at a time and place convenient for the family.

JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member and by providing individual, couples, or group therapy utilizing Cognitive Behavior Therapy, motivational interviewing, and other evidence-based practices to provide trauma-informed and culturally sensitive treatment.

Master's Degree in Human Services
Experience in working with individuals who have behavioral health issues
Eligible to be licensed
SPECIFIC DUTIES:
Provide individual, group and/or couples therapy for individuals
Evaluate and assess the needs of assigned individuals
Collect all pertinent information including demographic history, history of presenting problem, family background, testing evaluations, medical evaluation, etc., as indicated for ongoing treatment
Ensure that delivery of clinical treatment is congruent with models and practices listed in job summary
Work 1 - 2 evenings per week
