JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. This member of the ACT multidisciplinary team has the responsibility to carry out treatment, rehabilitation and support functions, education, and provide crisis intervention to participants and support and consultation to families. Provide service coordination (case management) for an assigned group of participants including coordinating and monitoring the activities of the individual treatment team.
QUALIFICATIONS:
M.A. degree in Social Work, Psychology, Human Services/Social Sciences, Healthcare or related field.
Internships and/or other supervised practical experience in a clinical or rehabilitation setting with persons with mental illness and/or co-occurring disorders.
Valid Driver's License required.
Must be available or scheduled emergency on-call service and will be required to carry a mobile phone during regularly scheduled working hours and scheduled on-call service. As essential personnel may be required to report to work during emergencies including inclement weather.
SPECIFIC DUTIES:
Provide service coordination (case management) for assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITT; provide individual supportive therapy and symptom management; ensure immediate changes; are made in treatment plans as clients' need change, educate and support clients' families; and advocate for clients' rights and preferences.
Conduct comprehensive assessment of psychiatric history (e.g. onset, course an effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment, social development and functioning; activities of daily living (e.g., care-care, living situation, nutrition, money management); and family structure and relationships.
Provide ongoing assessment of client's mental illness symptoms and clients' responses to treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior that put clients at risk (e.g., suicidality). Provide symptom education to enable clients to identify their mental illness symptoms.
Provide direct clinical services including individual supportive therapy and psychotherapy to clients on an individual, group and family basis in the office and in community settings to teach behavioral symptom- management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt to and cope with internal and external stresses.
Provide treatment, rehabilitation, and support services, with some interventions directed or performed by staff with specialty training and skills (e.g., vocational specialists).
Perform shift management in coordination with other shift managers according to established policies and procedures.
Provide on-call crisis intervention covering nighttime hours and serve as a back-up to evening and weekend staff.
Document client progress to maintain a permanent record of client activity according to established methods and procedures.
Provide individual and group treatment in the office and in community settings to a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has client-determined goals.

DESCRIPTION OF WORK

Essential Duties and Responsibilities
Manage an LSRP practice area including staff, clients, and revenue.
Support new and ongoing remediation projects as a LSRP.
Maintain professional contacts with other LSRPs, the NJDEP, and the regulated community through participation in professional societies or meetings.

Job Description
Independent Contractor
Skilled, Independent Contractor will provide trauma-informed therapy to underserved populations on-site at community-based mentalhealth outpatient clinic. This includes diagnostic assessments and individual, group, and family therapy for a diverse caseload of individuals (4 years of age and older) experiencing a variety of behavioral health issues.
FUNCTIONS:
Provide weekly client-centered, trauma-informed, individual, group, and family therapy as per the specifications outlined in the contract.
With the individual's consent, make concerted effort to engage identified support system in treatment.
Provide initial and ongoing assessment of presenting issues, diagnostic impressions, and recommended course of treatment.
Create treatment plan and routinely monitor progress in collaboration with client.
Coordinate care with other involved professionals (including psychiatry, case managers, primary care physicians, etc) to ensure continuity of care.
Enter intake and therapy appointments into electronic medical records at least 48 hours in advance.
Maintain accurate records of performance metrics and time spent including minimum billing expectations commensurate with scheduled hours. This includes entering progress notes within 3 days of service, including supportive documentation (e.g., treatment plans, safety plans, case management notes, briefs, triage notes, discharge summaries, no show or cancellation notes).
Encourage client's consistent attendance by routinely reviewing attendance policy and providing weekly reminder calls. Discharge inactive cases in a timely manner to promote access.
Provide linkage to identified resources as needed. This may include referrals for expanded mentalhealth programming including Intensive Outpatient Program (IOP), Drug & Alcohol (D&A), Blended Case Management (BCM), Psychiatry or community services.
Provide crisis intervention including safety planning as needed. This includes follow up and coordination of care when individuals go to the CRC or acute level of care.
Attend all staff group supervisions and individual supervisions as required.
Meet weekly billable requirements as agreed upon.
Complete all mandatory trainings to maintain state, CBH, and NIAC credentialing; maintains all Continuing Education for licenses or certifications.

Maintain necessary clearances, continuing education, licenses, and certifications to practice in the Commonwealth of Pennsylvania.

Manage own tax and financial affairs.

PAYMENT:
Agency will provide office space and support services including clinical supervision, administrative assistance, phone and computer, IT support, office supplies, and access to RELIAS for training and professional development.
Contractor will be paid an agreed upon rate per hour for billable services provided.
Contractor will submit invoices for billable services by the end of business day on Mondays. All required paperwork (including progress notes and up-to-date treatment plans must be completed) before the program supervisor will sign-off on invoices. Services delivered without a corresponding progress note or current treatment plan may not be paid. Late invoices (30+ days after service delivery) must be approved by the program administrator.
QUALIFICATIONS:
Contractors must meet one of the following criteria:
Master's or Doctoral degree in Human Services field.
(i) Has a graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes a clinical practicum.

To complete or assist others in completing an involuntary commitment petition, as indicated. Follow procedures regarding the initiation of a commitment as needed.
Utilize approved interventions to safely de-escalate a crisis and/or behaviors that present a danger to self or others.
Collaborate with the crisis case manager on resources and linkages needed and assure the linkages are in place.
Complete initial intakes as part of PATH's open access intake system
Determine level of care recommendations & work with COC to ensure resources and referrals are provided
Provide individual, family, group therapy as needed and assigned using approved treatment modalities and interventions
Be aware of youth financial liability information to ensure appropriate for PATH services and/or provide accurate referrals
Participate in training and on-going discussion regarding the norms and values of the variety of ethnic and cultural groups Philadelphia.
Obtain language/ translation services as needed to work effectively with youth and families.
Assure any follow up appointments are scheduled at a time and place convenient for families.
Complete all required documentation in a timely and legible manner and in accordance with PATH policies and procedures.
Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.

Possess excellent communication and interpersonal skills. Evidence and demonstration of cultural humility required. Act 33, Act 34 and FBI Clearances required. Child Protective Service Law Training (CPSL) completion required. Current driver's license and current vehicle insurance.
RESPONSIBILITIES:
DRUG AND ALCOHOL (D&A) PROGRAM
Involves providing individual counseling, group counseling, and care management for people of all ages with substance use and other mental disorders.
Provides psychosocial assessment; ongoing individual, family, and group counseling; client/family education; and care management for cases assigned.
Participates in regularly scheduled clinical supervision with assigned supervisor.
Maintains timely and adequate documentation (including reports, summaries, and letters) on each client assigned.
Works in close cooperation with other social service agencies, hospitals, and community resources toward planning and initializing services with clients.
Participates in formal and informal treatment team meetings to facilitate collaborative care for clients.
Provides community education as time permits. This involves public speaking, and planning and participating in educational workshops, seminars, etc.
This position may require school-based intervention and/or work in satellite office (Grove City and Greenville).
Attends all mandated Community Counseling Center in-service meetings and trainings, as well as continuing education classes to enhance job skills and maintain/purse licensure.
OUTPATIENT MENTALHEALTH PROGRAM
One (1) through nine (9) as above in D&A Program
Assesses strengths and therapeutic needs of clients.
Provides strength-based services that are client-centered and recovery-focused.
Recommends and refers for special evaluations and services, such as psychiatric assessment, or psychological testing.
Involves and encourages participation of collateral supports (i.e., family, friends, other providers) in treatment and treatment planning.
Maintains and upholds Community Counseling Center's policies, respects confidentiality, and exhibits a professional demeanor at all times while at work.
Violation of policies/procedures/job duties will be subject to disciplinary action.

Participates in CCC CQI (compliance quality improvement) plan to ensure quality of service to CCC consumers.
