JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Work primarily with adults with serious mental illness or emotional disorders, assisting individuals to make full use of Behavioral Health Services and natural community supports to achieve a more independent and fulfilling life. QUALIFICATIONS:

SPECIFIC DUTIES:
Develop partnerships with assigned individuals to assess needs, obtain services and resolve crises
Assist individuals with identifying goals and defining the steps needed to achieve their goals
Complete all assessment and planning documentation including Adult Psychosocial Assessment; Strengths Assessment; Treatment Plans and other required documentation
Ensure all required documentation is completed in a timely, comprehensive, and legible manner and is in compliance with PATH policies and procedures
Assist in linking individuals with their chosen services and resources in the community such as needed psychiatric and medical treatment, residential services, financial entitlements, social/recreational activities, and general social services

JOB SUMMARY:

Complete initial contact records for individuals seeking services at the agency. Provide crisis management as needed. Ensure all clinically necessary information is recorded within the intake assessment and that all individuals are referred to the most appropriate program to meet their needs. Coordinate linkages to other agencies and programs as needed. Provide administrative and D&A case management as needed. Assist in the development and implementation of Quality Improvement (QI) activities including reviewing daily case conference records for all admissions into the outpatient programs at PATH. Work cooperatively with other departments to ensure successful client engagement, monitoring and billing.
QUALIFICATIONS:
Bachelor's degree in Human Services or related field required;
Minimum of two years experience working in human services with some quality improvement preferred;
Ability to use Electronic health Record required.Basic Word, Excel and Power Point Skills desired; Knowledge of program evaluation, continuous quality improvement, and managed care procedures related to the behavioral health field preferred. Schedule intake assessments by screening individuals over the phone or through the walk-in clinic
Collaborate with financial liability department regarding insurance and billing issues
Provide linkages to other agencies and services as needed
Provide coverage of intake/crisis phones
Identify accessibility needs for individuals at the time of initial telephone contact & coordinate with treatment team in advance to ensure the needs are met.
Utilize the ASAM criteria assessment tool with individuals who are in need of substance abuse services
Coordinate crisis and case management services with other staff in COC/Outpatient Department
Function as a member of the agency's internal crisis team and after successful completion of mandatory training restrain, or place in restraints, those individuals who are unable to control themselves and present danger to themselves or others
Participate in crisis assessment to ensure individual receives necessary supportive intervention and assistance in managing and resolving the crisis and develop a crisis plan to assure coordination of recommended disposition
Notify supervisory staff of emergencies and urgent issues in a timely manner
Complete or assist others in completing an involuntary commitment petition
Make approved home visits
Complete Daily Case Conference and ensure timely follow up
Ensure all individuals seen through intake department are referred to appropriate services through regular review of the waitlist for each department
Communicate with your supervisor about individuals needing higher level review
Assist individuals with gaining access to needed psychiatric, medical, social, residential financial and other services as appropriate for the individual
Ensure collaboration with individuals' treatment teams, including the psychiatrist when coordinating care
Maintain and update resource file
Provide support to the SUOP department in completion of CM activities
Identify individuals for BHSI and ensure timely completion and submission of all required documentation
Monitoring of all Service Provision reports to track & coordinate follow up

Specific Duties:
Respond to/Process HR requests via electronic ticketing system on daily basis, including processing of employee actions, such as new hires, transfers, performance awards, promotions, demotions, resignations, terminations, leaves of absence, suspensions and/or 48-hour letters, etc.

To assist, perform or arrange the completion of maintenance functions in all agency facilities. This includes making repairs, requesting support from outside contractors and other team members.
When scheduled, will open and/or close locations according to procedures and in keeping with the Security System.
Perform site checklists to assure safe operation of all mechanical systems and the environment in general.
Monitor and ensure preventative maintenance schedule for all PATH facilities (including interior and exterior). Complete checklists. This may include landscaping, cleaning, and other facility maintenance work orders.
Assist supervisor in training the staff in facilities maintenance issues.
Arrange to complete necessary maintenance as issues arise.
Work with supervisor to prioritize necessary work schedule.
Painting as supervisor assigns.
Supervise outside contractors at PATH sites when needed.
Oversee the organization of the long-term storage facility.
Assure adequate positioning of copy paper supplies at various copiers at company sites.
Drive agency vehicles when necessary for site relocations, residential client moves or material pickup.

Be on call during work hours to respond to emergency requests regarding facility support functions.
Assist in monitoring evacuation during fire drills conditioning daily.
Assure snow removal from sidewalks, walkways, parking lots and driveways of all PATH properties.
Assure coordinated effort to facilitate refuse removal conditioning including at administrative sites and residential sites.
Order and pickup hardware materials necessary for ongoing maintenance projects.
Responsible for being on call for a week at a time, under a rotating schedule within the department which may include evening, morning and weekend hours.
Performing routine maintenance around the building such as fixing nonstructural damage, for example, window, door or wall repair.
Fixing potential safety hazards to avoid injuries.
Repairing broken or leaking plumbing to avoid water damage and restore full use of water fixtures
Applying preventative measures to the building to reduce the risk of future problems
Support and maintain facilities inventory system

JOB SUMMARY:

Prepare and submit 837P electronic billing files and CMS1500 paper claim forms. Download and review response files to the 837 billing files. Post electronic and manual payment files as received. Perform all billing and cash posting tasks in a timely manner in order to ensure maximum revenue and facilitate cash flow. Provide the Accounts Receivable Manager and Chief Revenue Officer with the necessary information to resolve all billing and funding discrepancies.

Download and post CBH 835 RA files including review, analysis, and correction of any incorrectly or unposted services.
Review, correct, and resubmit services rejected on the CBH 835 RA, or refer to A/R Manager for adjustments when indicated.
Review, correct, and resubmit services on the monthly CBH Open Services Reports, or refer to A/R Manager for adjustments when indicated.
Submit TPL Discrepancies to CBH for resolution.
Submit appeals for CBH services over 90 days as needed, and upload billing file with services within the required time frame when/if approved.
Review the CBH Unbilled Claims report on a monthly basis.

JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. This member of the ACT multidisciplinary team is responsible for overseeing a consumer caseload of individuals and provide rehabilitation services, including symptoms management, life skills development, vocational support, educational support, socialization support and budgeting/money management skills.

SPECIFIC DUTIES:
Provide service coordination (case management for an assigned group of consumers) including coordinating and monitoring the activities of the individual treatment team.
Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the consumer and the ITT.
Educate and support consumers' families, and advocate for consumers' rights and preferences.

Independent Contractor
Skilled, Independent Contractor will provide trauma-informed therapy to underserved populations on-site at community-based mental health outpatient clinic. This includes diagnostic assessments and individual, group, and family therapy for a diverse caseload of individuals (4 years of age and older) experiencing a variety of behavioral health issues.
FUNCTIONS:
Provide weekly client-centered, trauma-informed, individual, group, and family therapy as per the specifications outlined in the contract.
With the individual's consent, make concerted effort to engage identified support system in treatment.
Provide initial and ongoing assessment of presenting issues, diagnostic impressions, and recommended course of treatment.
Create treatment plan and routinely monitor progress in collaboration with client.
Coordinate care with other involved professionals (including psychiatry, case managers, primary care physicians, etc) to ensure continuity of care.
Enter intake and therapy appointments into electronic medical records at least 48 hours in advance.
Maintain accurate records of performance metrics and time spent including minimum billing expectations commensurate with scheduled hours. This includes entering progress notes within 3 days of service, including supportive documentation (e.g., treatment plans, safety plans, case management notes, briefs, triage notes, discharge summaries, no show or cancellation notes).
Encourage client's consistent attendance by routinely reviewing attendance policy and providing weekly reminder calls. Discharge inactive cases in a timely manner to promote access.
Provide linkage to identified resources as needed. This may include referrals for expanded mental health programming including Intensive Outpatient Program (IOP), Drug & Alcohol (D&A), Blended Case Management (BCM), Psychiatry or community services.
Provide crisis intervention including safety planning as needed. This includes follow up and coordination of care when individuals go to the CRC or acute level of care.
Attend all staff group supervisions and individual supervisions as required.
Meet weekly billable requirements as agreed upon.
Complete all mandatory trainings to maintain state, CBH, and NIAC credentialing; maintains all Continuing Education for licenses or certifications.
Complete all mandatory training to maintain state, CBH, and NIAC credentialing. This includes obtaining Evidence-Based Practice (EBP) training or other training as requested and actively participating in appropriate EBP consultation group(s), that are held for one hour/month (if applicable).

Function in full compliance with all agency policies/procedures and federal, state, and local laws.
Participate in Interact's Compliance Program, including reporting suspected violations of the compliance program or applicable laws or regulations to immediate supervisor or Deputy Director of Continuous Quality Improvement.
Maintain necessary clearances, continuing education, licenses, and certifications to practice in the Commonwealth of Pennsylvania.

PAYMENT:
Agency will provide office space and support services including clinical supervision, administrative assistance, phone and computer, IT support, office supplies, and access to RELIAS for training and professional development.
Contractor will be paid an agreed upon rate per hour for billable services provided.
Contractor will submit invoices for billable services by the end of business day on Mondays. All required paperwork (including progress notes and up-to-date treatment plans must be completed) before the program supervisor will sign-off on invoices. Services delivered without a corresponding progress note or current treatment plan may not be paid. Late invoices (30+ days after service delivery) must be approved by the program administrator.
QUALIFICATIONS:
Contractors must meet one of the following criteria:
Master's or Doctoral degree in Human Services field.
(i) Has a graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes a clinical practicum.
