Trains and supports individual in independent living skills, home management skills, cooking, cleaning, socialization, including supported employment in the community, and other identified skills needed to foster independence. Follows procedures within 24 hours for incidents identified according to Department of Human Services policy. Completes activity logs that occur during the work time frame as identified within the person centered plan objectives. For any change affecting the individual, reports, as applicable, any change affecting the individual receiving services. If necessary, ensures the consumer is reminded when it is necessary to take and/or refill medication. Completes and provides documents as required. Meets initial and on-going annual training requirements. Maintains compliance with required personnel documents. Perform any other related duties as required or assigned. EDUCATION AND EXPERIENCE High School diploma or GED; One (1) Year of relevant, supervised work experience with public health, human services or other community service agency; OR Two (2) years' verifiable successful experience working with individuals with developmental disabilities. CNA Certification or Medical Assistant certification will qualify for a special entry rate. Additionally, all Supported Living Companions hired after 11/01/07 must have the following: The ability to understand written activity plans, execute instructions, and document services delivered. Have the ability to communicate effectively with consumers. Have the ability to access emergency service systems. Have the ability to access transportation services required as appropriate. Have satisfactorily passed a criminal background check, and adult and child maltreatment registry checks. Criminal background and adult maltreatment checks must be repeated every five (5) years, and child maltreatment checks must be repeated every two (2) years.     Job Description Manager of one residential facility. Works under the direct supervision of the Director of Residential Services/Administrative Coordinator. Responsible for serving as a role model for all residents and subordinate staff; is responsible for daily operation, security and safety of the residential services facility assigned. Is responsible for the general supervision and training for all subordinate staff and residents in the residential services facility. ESSENTIAL DUTIES AND RESPONSIBILITIES Supervise and assist subordinate staff in training residents in independent living skills, home management skills, cooking, cleaning and socialization skills, utilizing the work wheel (as necessary) and task analysis. Coordinates schedules for both Waiver and generic service categories. Ensure that all DDS regulations and guidelines are met with respect to the requirements of the residential services facility. Is responsible for providing for the safety and security of the residential facility. Services as liaison between the person, parents/legal representation, Case Manager, and Waiver/DDS officials. Completes in a timely manner monthly and quarterly reports and various other records (i.e., attendance, drills, maintenance, etc.). Coordinates mediation, investigations, and disciplinary actions with appropriate personnel. Will purchase and keep inventory of all supplies bought and received for staff/residents use. Will complete incident reports and behavior reports and relay them to the appropriate personnel. Reviews cases and focuses on recipient's progress in meeting goals and objectives established through the Plan of Care. Monitors all supportive living services as assigned. Responsible for maintaining accurate records for client money management, bills, and having a clear audit trail for assessment purposes as required. Responsible for processing paperwork for clients who move in or move out, all processing verification documents, HUD files, and other duties relating to housing, Section 8 or other as required. Will follow Pathfinder Policies and procedures, applicable state regulations, CARF regulations, DDS regulations, HUD regulations, Waiver regulations and other licensing requirements necessary for quality of care.
