The Forensic Assertive Community Team (FACT) Psychiatric Nurse is a clinical staff member of the FACT
Community based multidisciplinary treatment team. The Nurse has the lead responsibility for assessing physical health needs; making appropriate referrals to community physicians; collaborating with medical providers and pharmacies; teaching wellness skills; and providing management and administration of medication under the direction of the team Nurse Practitioner. Additionally, the Nurse is responsible for providing a full range of services that are culturally relevant and recovery based. These services may occur in the home, community, or office. Service provision is designed to promote recovery by improving psychiatric symptoms, preventing relapses, teaching skills, and providing direct assistance and securing necessary community resources.
1. Develop a working knowledge of FACT as an evidenced based model and practice according to
the guidelines set forth by OMH. Participate in daily morning meetings with the purpose of reviewing client progress/needs, creating daily visit schedule and developing team relationships.
2. Conduct person-centered comprehensive health assessments, including personal and family medical and psychiatric histories, current medical needs, substance use history, and daily living skills with the purpose of linkages to and collaboration with providers in order to improve health outcomes.
3. Develop and implement strength-based treatment plans that are responsive to client needs and facilitate achievement of goals on an as needed basis.
4. Oversee all aspects of medication management in compliance with agency procedures and
protocols. This includes administration of injectable medications, managing Clozaril protocols, ordering of medication, overseeing processes for medication distribution, utilizing a medication step system towards client self-management, and managing all aspects of the medication room.
5. Provide support, treatment, case management and community advocacy and linkage services, per
the treatment plan. Complete required documentation of interventions as well as client's responses and outcomes.
6. Readily respond to all crisis situations and urgent needs using sound clinical judgement in order to ensure the safety and well-being of the individual. Provide on-call crisis intervention services on a rotating schedule amongst team members. Always act to ensure client's safety, readily assess needs, provide support, and intervene immediately as needed.
7. Mohawk's programs and offices are multi-level with stairs only. Individual services in the community include visits to individuals' apartments which can be on the second and third floors. Employees must be able to travel to all levels of the facility in a safe and reasonable time frame.
8. Provide transportation to individuals when necessary. In addition, an employee must be approved to drive agency and personal vehicles for business purposes by maintaining a valid NYS driver's license and must be approved by the agency's vehicle insurance carrier. An employee must always drive in a safe manner and use all safety devices.
Requirements
Skills/Qualifications for the Forensic ACT Team Psychiatric Nurse include:
1. Licensed Registered Professional Nurse by New York State.
2. Bachelor's degree in nursing preferred.
3. Three years' experience in a psychiatric setting, with at least one year's experience in an outpatient setting is preferred.
4. Must have a strong commitment to the rights and ability of each person with a mental illness to make own decisions and choice, live in the community, work, and have access to continuous support and services to enhance wellness.

You have published at least one paper at NeurIPS, ICLR, or ICML - where you were the lead author or made significant conceptual & code contributions.
You have significantly contributed to an LLM training effort which became newsworthy (topped a Huggingface benchmark, best in class model, etc.), preferably using multiple GPU's.
