Assisting non-working clients in accessing Public Assistance. Completing psycho-social assessments within the first 30 days of intake. Monitoring and tracking client progress. Empowering clients to actively participate in their own planning and goal setting. Conducting regular meetings with clients (weekly/bi-weekly or as needed).Referring clients for job development opportunities if appropriate. Connecting clients with suitable resources to help them achieve their goals. Meeting with clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency. Reporting critical incidents immediately to the Site Director/Director of Social Services. Collaborating closely with other Housing Specialists and Employment staff to identify client readiness for employment and housing. Treating all clients, visitors, and employees with kindness, respect, and dignity. Serving as an advocate for clients when necessary. Providing periodic case management and counseling, extending one year beyond the client's departure. Establishing and nurturing connections with other programs and units within the organization to facilitate inter-agency referrals. To learn more, please visit ************************* ABOUT THE POSITION The Director of Intake and Case Evaluation is responsible for ensuring the smooth operation of the Intake Department's complicated operation, revising and updating the process to improve efficiencies while maintaining quality, addressing the accumulation of cases that have built up in response to an overwhelming demand for representations, and expanding the process to include the evaluation of a broader range of claims of innocence. The Director of Intake and Case Evaluation provides overall direction and management of the Intake department and supervises a cross-functional team of Intake professionals, including analytical, legal, administrative, and strategic staff. In this capacity, the Intake and Case Evaluation Director is responsible for: a) developing department strategy and annual goals, b) effectively managing an experienced team to keep them focused, aligned with each other, motivated, and high-performing, c) producing reports and analyses of progress toward goals, d) general oversight of pro bono partner law firms in collaboration with the Intake Supervisor of Law Firm Partnerships, and e) devising new strategies to improve the efficiency of the evaluation. The Director of Intake and Case Evaluation reports to the Chief Program Officer and works closely with the Director of Post-Conviction Litigation. We are interested in every qualified candidate who is eligible to work in the United States. However, we are not able to sponsor visas and we are not an e-verify employer. The duties of this position require approximately 40 hours of work per week to be performed virtually/in-office. We currently have a hybrid in-office model, with all employees in the office three days per week and the remainder of the week remotely. Understand any medications that they are prescribed, helping them learn how to refill prescriptions as needed Connect youth with community resources Ensure ongoing contact with outside persons and entities such as: natural supports, family, referring agency/case carrying agency, ILP, other and community resources Support youth: With the development of independent living skills Support the goals outlined in the Transition Independent Living Plan (TILP) and/or the Appraisal Needs and Service Plan Work with youth to identify additional goals and tasks related to employment, education, and housing Establish and monitor client budgets and submit purchase requests With transportation as needed when it fits within the resident's goals All social work activities with clients is supervised by the social worker supervisor, generally the Program Manager or Program Director. Lighthouse TLP is a housing first low-barrier, high tolerance program providing housing and comprehensive services to youth ages 18-24 who are experiencing housing instability. ABOUT THE ROLE: The Case Manager works out of the Lighthouse Transitional Living Program in West Los Angeles. Lighthouse TLP is a housing first low-barrier, high tolerance program providing housing and comprehensive services to youth ages 18-24 who are experiencing housing instability.
