Provides clinical leadership to promote increased compliance with a range of quality and cost measures and standards of care. Manages service delivery of inter-professional and para-professional team members working on an individual case or population of cases. What You Will Do
Manages and provides full scope of advanced nursing practice for targeted patient populations. Evaluates patient responses to therapy / interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes.
Identifies need for new/revised clinical protocols. Collaborates with physicians and others within the practice to develop protocols and provides training as appropriate.
Manages and provides comprehensive advanced nursing care including physical examination, comprehensive history, screening for physical and/or psychological conditions, emergent interventions, pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling and patient education. Establishes a treatment plan based on clinical findings and. Determines when further evaluation by collaborating physician, specialist or emergency care is warranted.
Collaborates with patients, families, primary care physicians and other team members to provide assessment and care planning. Assesses, plans, and provides intensive and continuous care management across client settings.
Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements.
Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required
Valid driver's license, as determined by operational/regional needs may be required
Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans required
Maintains NPI, Medicaid and Medicare provider numbers preferred
Maintains a collaborative practice agreement with a physician in compliance with New York State regulations preferred
Must be certified by ANCC or another accrediting Nurse Practitioner body - in order to bill Medicare and meet credentialing requirements required
For Psychiatric Nurse Practitioners only:
Current PMHNP-BC certification required
Education:
Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) required

JOB SUMMARY:
The MRSS Qualified Mental Health Specialist (QMHS) will provide Mobile Response and Stabilization Services (MRSS) to youth (age 2-20) and families who have called the crisis hotline with a family identified crisis. The MRSS Qualified Mental Health Specialist (QMHS) will respond face-to-face (in person or via video conferencing) with another MRSS Team member to high risk crisis situations within an hour to assess the youth and stabilize the family. After the initial 72 hours, the family will receive on-going stabilization and care coordination services from the MRSS QMHS and other members of the MRSS team in various community settings. The team works to provide support - including education and consultation for parents and/or caregivers, case coordination, and symptom management and monitoring for up to 42 days.
This is a Part-Time Position with flexible work hours. 2nd shift availability - (12pm - 8:30pm) is required for this position
ESSENTIAL DUTIES:
Provide mobile response to a family identified crisis within one hour.
Engage youth and families in de-escalating the crisis and assess for risk.

Job Summary
Home Base, a Red Sox Foundation and Massachusetts General Hospital program, is dedicated to healing the invisible wounds - including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, family relationship challenges and other issues associated with Military service - for Veterans of all eras, Service Members, Military Families and Families of the Fallen through world-class clinical care, wellness, education, and research.
The Massachusetts General Hospital seeks a dynamic Licensed Clinical Social Worker (LCSW)/Licensed Independent Clinical Social Worker (LICSW) to serve on the Special Operations Team. In this role, the LCSW/LICSW will provide and oversee the provision of psychiatric, psychosocial, and overall mental health services and referrals for military Special Operators into the Home Base program. They will also be dedicated to supporting the Special Operations Forces (SOF) program and will provide coverage to other clinical operations as needed. The LCSW/LICSW may help cover approximately 2-3 weekend shifts per year.

This position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

-Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.

PRN Social Worker - As needed/PRN - Monday through Saturday
MAIN FUNCTION:
Provides direct social work services to patients and families identified as having complex needs. Works with the RN Case Manager to identify inpatients whose social needs necessitate comprehensive psychosocial interventions. Intervenes as necessary with patients seeking emergency care or other ambulatory care services. Must be able to communicate effectively with physicians, staff, patient/family and outside agencies. This position must be able to handle multiple priorities and be self-directed. It requires good organizational skills and analytical thinking.
RESPONSIBLE TO: Manager, Care Management
MUST HAVE REQUIREMENTS:
Bachelor's degree or Master's degree (preferred) in Social Work
Graduate of a school of social work accredited by the Council on Social Work Education.
Current LSW or LISW license in Ohio.
2 years recent experience as a social worker in a medical setting.
No written disciplinary action in the last 12 months.
OTHER REQUIREMENTS:
Employee in this position may be exposed to blood, body fluids and/or airborne pathogens and is required to wear barriers.

POSITION EXPECTATIONS:
* Provides direct social work services to patients and families for social, environmental and emotional problems and makes referrals to appropriate community agencies.
* Documents the initiation of the discharge planning assessment tool within 24 hours of referral.
* Maintains and develops community resource information pertinent to patient care. Maintains professional relationship with community agencies.
* Interviews patients and / or family members and significant others; meets their psychosocial needs within the scope of social work license and established procedures and protocols.
* Formulates and completes discharge plans for patients of all ages in collaboration with physicians and other disciplines participating in the care of the patient.
* Provides private setting for interviews with patients and families.
* Assists in arranging transportation to nursing care facilities, medical appointments and home if needed.
* Provides information to patients/families regarding Medicare, Medicaid and other financial assistance programs.
* Provides charting duties and record keeping as required by department and hospital policy.
* Strives to meet patient/family needs on a timely basis to provide for optimum post hospital care, within the restrictions of insurance guidelines and limitations.
* Assists patients with Advance Care Planning and in obtaining accurate information on Advance Directives and Patient Rights.
* Maintains knowledge base of community resources available in Wayne County and surrounding area.
* Keeps abreast of current federal and state regulations, as well as professional standards.
* Attends educational programs to meet professional competencies and maintains a valid license to practice in Ohio.
* Assists with complex discharges issues related to psychosocial needs of the patient and/or family.

* Assesses for depression and anxiety using the PHQ-9 screening tool and the GAD screening tool as
indicated and refers to Master's level social worker when appropriate.

* Assesses for behavioral health needs including but not limited to suicide risk and substance abuse disorders and follows up with appropriate interventions and refers to Master's level when appropriate.
