This is a role that is key for direct service delivery within the Community Supports Team Service area and designed to truly make a difference in the lives of the families that are served, improving the quality of life and connection to community.
What you will do
The Community Supports Team Qualified/Associate Professional under the supervision of the Team Lead; is a crucial team member providing therapeutic services to members and their families to aide in improving their overall quality of life and decrease symptoms that would require a higher level of care. You will be working with the members within a clinical capacity by providing therapeutic interventions and activities that will make meaningful impact on the lives of those members. You will be maintaining all clinical records, by documenting all engagements within given regulation. By collaboration in the Person Centered Plan, you will be assisting members along the continuum of care while ensuring that all needs of the member, and their role within the family, are met. This will include minimizing the negative effects of psychiatric symptoms or substance dependence that have interfered with the members daily living and development within the community.
Additionally you will have the opportunity to work autonomously with your existing team and team leader. This will occur through your connections to community engagement while working in collaboration with various behavioral health resources. You will also assist the members and their families during times of crisis in a clinically appropriate manner, while designing specific interventions that will allow for growth of independence and success. This could include developing socialization skills, adaption skills, behavior and anger management all developed within a holistic manner to help treat the member as a whole person. You will also participate in case management linking the member to meaningful services and referrals within their community.

What you will do

Who we are looking for
We are seeking a talented individual who shares our focus and dedication for those we serve and support. This is a role that is key for direct service delivery within the Community Supports Team Service area and designed to truly make a difference in the lives of the families that are served, improving the quality of life and connection to community.
What you will do
The Community Supports Team Qualified/Associate Professional under the supervision of the Team Lead; is a crucial team member providing therapeutic services to members and their families to aide in improving their overall quality of life and decrease symptoms that would require a higher level of care. You will be working with the members within a clinical capacity by providing therapeutic interventions and activities that will make a meaningful impact on the lives of those members. You will be maintaining all clinical records, by documenting all engagements within given regulation. By collaboration in the Person Centered Plan, you will be assisting members along the continuum of care while ensuring that all needs of the member, and their role within the family, are met. This will include minimizing the negative effects of psychiatric symptoms or substance dependence that have interfered with the members daily living and development within the community.
Additionally you will have the opportunity to work autonomously with your existing team and team leader. This will occur through your connections to community engagement while working in collaboration with various behavioral health resources. You will also assist the members and their families during times of crisis in a clinically appropriate manner, while designing specific interventions that will allow for growth of independence and success. This could include developing socialization skills, adaptation skills, behavior and anger management all developed within a holistic manner to help treat the member as a whole person. You will also participate in case management linking the member to meaningful services and referrals within their community.

What you will do
As a CST Qualified Professional, you will be responsible for providing direct care and coordinating services for individuals with mental health and substance use challenges. Working under the supervision of the CST Team Lead, you will play a key role in assessing client needs, developing treatment plans, and ensuring that all services are delivered effectively and in accordance with best practices.
Your responsibilities will include:
Conducting comprehensive assessments of clients to identify strengths, needs, and goals.
Developing, implementing, and monitoring individualized treatment plans in collaboration with the client, family, and CST team members.
Providing direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention when necessary.
Coordinating and linking clients to community resources, including housing, employment, education, and healthcare services.
Advocating for clients within various systems, such as healthcare, judicial, and social services, to ensure they receive the support they need.
Monitoring client progress and adjusting treatment plans as needed to ensure positive outcomes.
Maintaining accurate and timely documentation of all services provided in accordance with agency policies and regulatory requirements.
Collaborating with other CST team members, including the Team Lead, Peer Support Specialists, and other professionals, to ensure a holistic and integrated approach to care.
Participating in regular team meetings and case conferences to discuss client progress and coordinate care.
Promoting the values of recovery, person-centered care, and cultural competence in all interactions with clients and team members.

A Bachelor's degree
A minimum of 2 years of experience working with individuals with mental health or substance use challenges, preferably in a community-based setting.

A commitment to person-centered care and cultural competence.
A valid driver's license and reliable transportation.

Additionally you will have the opportunity to work autonomously with your existing team and team leader. This will occur through your connections to community engagement while working in collaboration with various behavioral health resources. You will also assist the members and their families during times of crisis in a clinically appropriate manner, while designing specific interventions that will allow for growth of independence and success. This could include developing socialization skills, adaptation skills, behavior and anger management all developed within a holistic manner to help treat the member as a whole person. You will also participate in case management linking the member to meaningful services and referrals within their community.
