The Role: The Vermont Support Line Operator provides phone support that is free, confidential, nonjudgmental and available to all Vermonters 24/7. The Vermont Support Line is staffed by individuals who acknowledge that they have experienced a broad range of struggles of their own and are willing to be open about them utilizing the principles of Intentional Peer Support.
Serves: All Vermonters over the age of 18
Supervised by: Shift Supervisor
Hours Available: Per Diem - Substitute
Responsibilities:
Provide safe, confidential, non-judgmental support to Vermonters who call and/or text the Vermont Support Line
Interact with callers using a “peer” approach as taught in the Intentional Peer Support (IPS) model
Respond to callers and texters via phone or text within a reasonable timeframe
Communicate via email, chat, and text with co-workers and supervisors
Participate in ongoing training (as advised by a supervisor), skills building and team building including team meetings, core competencies, co-reflections, etc. Some of these trainings are offered remotely and are recorded for viewing at a later date
Required to complete data collection after each call accurately and completely
Negotiate adversity and challenging conversations with callers - including, but not limited to: conversations about death or wanting to die, substance use, altered states, extreme states, and oppressive systems
Assist a caller in crisis and refer call accordingly to appropriate resources
Awareness and adherence to policies and procedures for Vermont Support Line and Pathways Vermont
Qualifications:
● Must live and work in Vermont
● Must have the ability to occasionally work third shift - overnight hours and/or weekend evenings
● Experience with mental health, substance use and/or other life challenges preferred, and a willingness to support others utilizing a “peer” model of service
● Ability to share from one's own personal transformative experiences

The Role: The Program Operations Specialist serves as a key point of coordination between service teams, housing staff, administrative staff, and the Department of Corrections. This position supports consistent and equitable services across our statewide service sites, the seamless integration of new residents into housing, supports move-ins and move-outs, track and monitor data, outcomes, and trends, and works closely with the department of corrections to solve problems and create innovative solutions. The Specialist plays a vital role in supporting resident transitions, promoting effective inter and intra-agency communication, and managing multiple perspectives, schedules, and needs.
Location: Statewide service range, mostly remote with in person reporting required occasionally
Supervised by: Director of Program Administration
Available position: Full Time, 35-40 hours per week, Standard Work Week is Monday - Friday 9am-4pm
Compensation: $23-$25/hour
Primary Responsibilities:
Primary administrative liaison for the Department of Corrections reporting and communication, including
Data entry in OMS: completion of quarterly and annual reports, ensuring referrals are responded to in a timely manner
Obtaining records: ensuring all necessary client data is amassed and shared with the service and housing teams prior to client admission
Ensuring service plans and other required documents are uploaded to OMS, including lease violations, client notices, etc
Coordinating move-ins for DOC participants
Securing and sharing relevant information with Medical and Service teams to support with community integration
Attend monthly P&P meetings and other program meetings
Completing quality data checks in foothold, ensuring internal reports completed and reviewed
Discharge support
Move-in kit ordering and support with oversight of DOC spending
Ensuring PREA and program specific trainings are up to date
Respond to and prepare agency records requests in a timely manner
Coverage of the Pathways Vermont On Call service between 9am-4pm Monday-Friday
Conduct program and agency mailings and annual surveys as needed

Review of client files upon discharge, ensuring organization and accuracy

Forensic Assertive Community Treatment (FACT) Service Coordinator

The Role: The Service Coordinator is a member of the Forensic Assertive Community Treatment (FACT) team. The service coordinator provides community-based support and coordination with other services to individuals enrolled in the program and acts as a member of an interdisciplinary team with a combined caseload. This position focuses on supporting participants with finding employment, securing benefits, and pursuing community resources and prosocial connection, but all positions on the team support participants on an as-needed basis.

Engage in relationship building/rapport building with program participants and provide service from a strengths-based, participant-centered perspective, informed by self-determination and harm reduction principles.

Provide one to one support and counseling on life skills, housing concerns, and health maintenance, including such tasks as medication support, housekeeping, and assisting participants to manage their monies, including preparing budgets with participants and computing expenses.
Coordinate and monitor referrals to community services, and advocate for participant participation in them; manage contacts with outside agencies and act as participant liaison where indicated.
Educate, inform, and advocate for participants regarding benefits and entitlements (Social Security, Public Assistance, food stamps, etc.). Assist with applications and appeals as needed.
Regular attendance is a requirement of the position.

Qualifications:
Bachelor's or equivalent work experience preferred
Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.

Lived experience of mental health challenges, homelessness, substance use, and/or the criminal justice system will be considered an asset

The Training and Implementation Coordinator is part of a small training team who will be supporting the implementation and training of Vermont's brand new Peer Support Certification program. The Coordinator will work in collaboration with the Director of Training and Advocacy and the training team to refine curriculum materials, promote training, evaluate eligibility of applicants, and to facilitate training content to cohorts of learners.
What is peer support certification? Vermont is one of the last states in the nation to develop and implement a formal training and certification for peer support providers.
Peer support is a mutual relationship between individuals with lived experience of trauma, mental health or substance use challenges that emphasizes a non-judgmental, values-driven
approach that promotes multiple perspectives, advocates for human rights and dignity, and focuses on genuine, mutual relationships that enrich the lives of those involved.
Certified peer support providers use their lived experience of trauma, mental health or substance use challenges in combination with specialized training to build genuine, mutual relationships that support individuals to live self-determined lives of their own choosing.
Certified Peer Support specialists will complete specialized training and examination prior to receiving their certification through the Office of Professional Regulation.

The Role: The Property Manager is a member of the Housing team and works in partnership with the Housing First services teams. Oversees lease ups, move-in, move-outs and regular inspections pursuant to Housing quality standards between tenant, landlord and housing authority inspector if applicable
Responsible for property management for Pathways units including but not limited to repairs and maintenance, managing tenant lease and rent responsibilities, unit turnovers and regular landlord inspections.
Be well versed in the obligation of tenants, landlord and housing authority as outlined in tenants HUD subsidy contract and/or DOC program agreement.
Engage with the Department of Corrections to efficiently move clients from incarceration to housing in the community in a timely manner.
Ensure Pathways remain in compliance with all housing contracts, including, but not limited to: HUD, Shelter Plus Care, Section 8, DOC and DMH. Remain current with HUD rules and regulations as it pertains to HUD subsidies and DOC Grant funds.
Accurate and timely maintenance of electronic database to track leasing, maintenance and payment information for all units occupied by Pathways tenants
Recruit landlords and units to the program as needed, track all outreach contacts on behalf of Pathways
Submit rent requests to finance department to ensure timely and accurate payment of rent to property owners
Ensure collection of tenant rent
Create regular invoices documenting client rent payments and outstanding balances
Organize and submit vendor invoices for payment
Organize services to promote retention as needed (including cleaning, maintenance)
Address tenant and/or property owner/neighbor complaints
Secure and inspect vacant units on a regular basis
Recruit, onboard and direct local contractors and vendors as needed
Oversee contracted work and ensure timely and satisfactory completion
Work with property owners to maintain and improve properties utilized by Pathways
Participate in lease negotiations and renewals
Participate in landlord/tenant mediation as needed to resolve tenancy issues
Draft and issue warning, termination and other notices as necessary

Qualifications:
Bachelor's or equivalent work experience preferred

Soteria House Services Team Lead supports a hospitalization prevention residence for individuals experiencing an early episode of psychosis/extreme state/non-consensus reality who wish to avoid or minimize their exposure to medications. Soteria House operates as a Therapeutic Community Residence (TCR). Services are funded by the Vermont Department of Mental Health (DMH) and TCR compliance is overseen by the Department of Aging and Independent Living (DAIL).
The Role: The Service Coordinator is a member of the Soteria community which includes residents, Resident Support Staff, Shift Supervisors, and two Co-Directors. The position is focused on working with Soteria residents to develop and implement a personalized service plan. Service Coordinator also assists residents in their transition from the residence to the larger community and supports other leadership tasks at Soteria. The Service Coordinator also helps develop relationships with other community providers and resources. Time will be spent at Soteria with the residents as well as in the larger community after a resident transitions from the home. Transition goals will be driven by the resident and may include supported employment, housing support, social support and more.

Essential Coverage Tuesday 8am-4pm, Friday 8am-3pm, Saturday 8am-3pm
Remaining 13 hours are flexible on wednesday and thursday and should include leadership and staff meetings, as well as flexibility to be on site at different times of day to accommodate resident service planning needs
Soteria Service Coordinator Responsibilities:
Engage in relationship building with residents and provide service from a “choice” perspective as well as participant-centered, harm-reduction, and strengths-based approaches
Collaborate with residents in the formulation and maintenance of a dynamic service plan, centering on how to best support the resident in achieving their self-expressed (linear and non-linear) goals while in the program. Plan is reviewed and modified at least monthly
Facilitate weekly network/service meetings with residents and their community support networks
Maintain a system by which to delegate appointments/transportation needs to other members of the team (i.e. a resident calendar, emailing, etc.)
Assist residents in establishing needed community-based medical and mental health services
Support all residents in accessing health insurance and affordable healthcare
Support residents in identifying sustainable permanent housing
Accompany residents to apartment showings and assist in answering landlord queries and/or completing housing applications
Complete applications with residents for appropriate financial assistance
Coordinate closely with Soteria Co-Directors, Supervisors and Resident Support Staff
Transport residents to appointments and meetings as necessary and/or assist in coordinating with Resident Support Staff. Assist in finding long-term transportation options and resources
Record significant interactions with program residents and service plan changes in Foothold AWARDS database
Read notes daily and ensure there are consistent notes in the resident record
Mediate family conversations and serve as main point of contact with residents' families and/or guardians, as well as service providers in the community
Assist residents in managing their monies, including assistance in computing expenses and preparing budgets
Intake Team Member Responsibilities
Meet with applicants in the community for intake interviews/assess for intake criteria
Complete intake check-list, which includes
Coordinating with medical providers and community partners to obtain required medication orders, self administration checklists and hospital discharge summaries/notes from other community providers
Reviewing admission packet with residents
Ensuring that resident and/or providers sign required auxiliary documents
Conduct intake at point of entry.
Exit process responsibilities
Oversee completion of exit check list, which includes:
Developing exit plans with residents
Communicating with families and community partners to support exits and coordination of care
Coordinate conversations with staff and leadership to have effective exit conversations with residents and implement timelines that meet resident and programmatic needs
Provide oversight for the Transitional Resident Program: facilitate weekly network/service meetings, update service plans a minimum of monthly, and ensure weekly contact is made with transitional residents
Soteria TCR Services Responsibilities:
Provide back-up coverage for Resident Support Staff
Support with quality oversight/data collection for services outcomes
Maintain and update the Disaster Relief Kit's Foothold Facesheets monthly
Ensure all necessary documents are in a resident and transitional residents' Google drive file and maintain their Facesheet (with the exception of medications, which is the role of the Soteria nurse)
Collect, maintain, store, and dispose of all participant information according to applicable requirements for confidentiality, security, and privacy
Leadership Responsibilities:
Supervise the Peer Resident Support Staff and interns as needed
Attend and co-facilitate weekly team meetings and leadership meetings
Participate in co-reflection, and team and agency meetings as requested
Participate in interviews with other leadership staff as requested
Participate in rotating on-call schedules with the leadership team as requested for coverage needs.
Participate in agency and program specific trainings
Other duties as assigned including, but not limited to: support leadership during staffing transitions (hiring, scheduling staff, etc)

Working Conditions/Physical Demands:
