Address patient questions and requests via phone, in person, and through the portal, including but not limited to record requests, referrals, prescription refills, etc.

Summary

Summary
The Patient Care Coordinator is the point of contact to ensure patients' needs are met. The PCC coordinates patients' treatment plans and follow up, order changes, and renewals.
Essential Functions
Determines insurance eligibility and coverage for a variety of plans and patient-specific situations
