For our HHAs / PCAs every day is different. As an HHA / PCA with NurseCore you will provide basic patient care in a medical facility or the patient's home. Carry a caseload of assigned clients

Collaborate with a variety of community providers and resources to obtain needed services and supports, utilizing community and family resources to create a sustainable support system

Escalate care management to practice-based resource when medical assessment is needed

Participate in and/or conduct frequent non-medical management coaching, education, follow-up visits and phone calls to patients to monitor progress and identify new barriers or concerns
Assist with financial or other social issues that may provide barriers to patient compliance
Provide education/guidance to patient and family on tools to manage chronic illnesses, develop individual and web-based tools and resources to improve compliance and meet goals
Identify and connect patients with community resources to assist with improving compliance with treatment protocols and social issues (e.g. legal aid)

Ensure diagnostic, post-hospitalization and specialty referrals have been executed and results received and acted upon as needed
Document plan of care, patient utilization, care management activities and other required information in state and practice databases

Tasks include collecting and recording of specimens and body fluids.2.Under the direct supervision of a nurse, performs direct patient care affecting the well being, comfort and safety of patients.3.Performs technical procedures and reports findings to the nursing staff. Tasks include: performing venipuncture for blood samples, performing EKG's, placing electrodes and obtaining telemetry readings.4.Collects patient data as defined by the Skill Inventory List and documents these data on the appropriate flow sheets.5.Collects specimens, performs basic tests or sends specimens to appropriate laboratory for analysis per Hospital policies.6.Attends to patients' environmental needs by straightening belongings, offering comfort measures at the direction of the nurse. 7.Participates in Quality Assurance activities as directed by nurse.8.Orients patient/family to unit and patient room, and provides copies of Patients' Bill of Rights, as directed by nurse.9.Performs other duties as assigned by nurse to support unit functioning and patient care delivery.REQUIREMENTS:High School Diploma and two years of experience in health care delivery in positions such as: Nursing Assistant; Home Health Aide; EMT; Medic/Corps Person; or two clinical semesters completed as a Nursing Student; or equivalent combination of education and experience.

Completion of a state-approved certified nursing program and/or successful passing of a competency evaluation program which meets state regulations.
Meets state specific certification requirements and possesses unrestricted certification as a PCT.
At least One - two years of work experience as a PCT in a healthcare setting.
Current ACLS, BLS, NIHSS, PALS*, TNCC* as required by client facility; other health and screening tests as required by specific facilities and/or regulatory agencies.
