Attends interdisciplinary meetings as needed to provide input into selected resident's plans of care.
Documents restorative services provided following facility policies and procedures.
Assists facility staff to provide person-centered cares based on indicated preferences.
Ensures punctuality and regular attendance for assigned shifts.
Other Functions
Performs other tasks as assigned.
Conducts job responsibilities in accordance with the standards set out in the Company's policies and procedures, applicable federal and state laws, and applicable professional standards.
Knowledge/Skills/Abilities
Ability to react decisively and quickly in emergency situations.

Prescriptive authority in Idaho or eligible to obtain licensure.
Licensed or eligible for prescriptive licensure in Idaho.
Knowledge

Ability to work effectively within a multidisciplinary team, including caregivers, students, and patients.
Preferred
Experience applying Functional Medicine within practice.
Certified through the Institute for Functional Medicine (IFM) is preferred.

• Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services, including the documentation requirements.
• Follow the labeling policies and practices of the facility.

• Follow facility procedures in regard to charting medications.

Essential Responsibility
Provide direct resident care services including medicine distribution to residents. Follow an individualized plan of care for all residents.
Perform resident assessments as required by the state standards/regulations and facility policy, under the direction of the Manager of Assisted Living and Director of Health Services.
Observe residents for change in condition and report any changes immediately to the supervisor and/or the Director of Health Services. Provide timely interventions to change of conditions and share this with care staff and family as instructed.
Document all pertinent/relevant observations, red flags, incidents, interventions, communication with the resident's responsible party, physician (as necessary), clinical procedures and daily medication assistance/care throughout the shift as indicated and no later than by the end of each shift.
