Job Description
Accept and manage patients within your designated geographical area of coverage
Plan, prepare, and implement customized therapy programs designed to restore overall function and promote independence in the home setting
Develop and maintain individualized care plans/goals in collaboration with physicians, patients, and their families
Ensure timely and accurate documentation of all patient visits and therapy services
Effectively communicate discharge planning with patients, families, and interdisciplinary team members
Maintain compliance with all clinical and documentation standards
Qualifications
Physical Therapist Program Graduate.
TN Physical Therapist License or eligible.
Liability insurance
Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

As a Home Health COTA, You will:
Assist the Occupational Therapist in the implementation of vocational and educational activities designed to restore and reinforce individuals' task performance and promote their overall health and self-sufficiency
Conduct community reintegration activities to assist individuals with their occupational therapy needs during physical activity

Communicates and interacts effectively and tactfully with the residents, visitors, families, peers, and supervisors promoting a homelike environment.
Fully understands all aspects of residents' rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials. Also providing care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas.
