About the Position
We are looking for a Residential Care Associate II to join our team at our Madrone Ridge SRTF program in Junction City, Oregon! The Residential Care Associate II will provide support to the residents according to their individualized treatment plan with a focus on personal care services, environmental intervention, and limited group social and recreational treatment activities as assigned by utilizing an approved evidenced-based group curriculum. They will provide limited treatment services and personal care supports for clients that help them develop appropriate skills to increase or maintain their level of functioning. The Residential Care Associate II will administer prescribed medications, follow physician's orders for all medications and/or procedures, document on medical charts and count sheets according to established procedures, and responsibly assist clients with self-administration of medications within agency policies and procedures. They will transport clients to and from the program as necessary to engage and provide treatment services. They will maintain compliance with established productivity standards for documentation of services.
This position falls under the AFSCME bargaining unit. Wages, benefits, and working conditions are set in accordance with the collective bargaining agreement.
Training Program
With our MHACBO-accredited training program, you will earn CEUs towards applicable mental health certifications, while attending your required, paid training, and receiving qualified clinical supervision gaining on-the-job experience.
Work Schedule: Tuesday through Saturday, 2:00pm - 10:30pm (Full Time, Swing)
What You'll Make
$21.63 - $22.96 per hour DOE/Credentials. An additional $3.00 per hour night shift differential will apply during the work hours of 10:00pm - 6:00am.
Additional 5% Language Differential offered for Bilingual or Multilingual candidates.
About the Program
Madrone Ridge is a secure, co-ed, 8-bed program serving adults living with a mental illness. Madrone Ridge is a beautiful newer building located on the grounds of the Junction City Hospital.

What You'll Need
Must be eligible for Qualified Mental Health Associate registration through the Mental Health & Addiction Certification Board of Oregon (MHACBO). Eligibility requires 1 out of 3 Criteria below:
Bachelor's degree in Behavioral Science: Psychology, Counseling, Social Work, Human Development, or related field; OR minimum three (3) years of verifiable behavioral health experience working with individuals with behavioral health needs in a supervised setting; OR equivalent combination of behavioral science education in addition to satisfactory supplemental behavioral health experience and training
Will be required to register as a QMHA - R through MHACBO during the first week of employment.
Driving is an essential function of this position. Successful applicants must provide a valid driver's license from their state of residence or obtain and submit one within 30 days of hire, maintain a safe driving record, and have the ability to pass a DHS criminal background check.
Physical Demands: This position requires the ability to frequently talk, listen, use hands and fingers, reach, stand, walk, sit, bend, stoop, climb stairs, as well as lift, carry, push, and pull up to 40 pounds. It may require the ability to occasionally use ladders, squat/kneel, and perform other physical tasks as applicable. In addition, you must successfully complete crisis de-escalation and CPR trainings, which require performing various physical, hands-on maneuvers and techniques and may require an individual to lift and transfer clients.

What You'll Do:
Maintain and update patient medical records, including vital signs, treatments, and medications, and review immunization records.

Provides assistance to the surgical team during the intra-operative phase of surgical care for the Northwest Surgical Services region. All RNFAs will participate in a regional on-call rotation to provide surgical assistance for elective, emergent and urgent surgical cases.
Essential Responsibilities:
Representative Functions or Duties: Practice standards of performance: Pre-operative. Performs pre-operative assessment and evaluation. Collaborates with the surgical team on plan of care. Intra-operative: Provides knowledge based technical assistance to the surgeon during the operative procedure according to AORN standards, licensure, state regulation and facility/practice protocols, including: Using instruments and medical devices. Assist with or provide activities of daily living (ADLs) including baths, linen change of occupied & unoccupied bed, dressing, feeding, providing water, grooming & home hygiene, AM & HS care, mouth/foley/peri-care. Perform bladder scanning, straight cath (mini-cath) in and out of adults= 18 years. Collect specimens & record specimen collection. Reinforce pulmonary hygiene including use of incentive spirometer, maintain oxygen, & connects patient to pulse oximeter. Provide nourishments; monitor& set up NG feedings, G-tube feeding or J-tube feeding for the nurse to administer. Administers minor treatments & procedures such as, but not limited to, EKG test administration, non-medicated enemas (Harris flush, soap suds), non-sterile dressing changes; sitz baths, apply ostomy appliances to old, healed sites; perform oral suctioning; safely apply heat/cooling measures; perform surgical preps; complete pre-op checklist. Perform wound cleaning and irrigation, and apply simple dressings. Apply CPM machine, sequential stocking. Perform POC testing and quality control checks. Provide bed exercises, including range of motion & positioning, & assistpatient in ambulating when necessary. For each patient, documents care provided, including but not limited to: oral intake, all output, vital signs, weights, and measurements in a concise, accurate, legible & timely manner in the medical record. Transports patients for transfer, discharge, or other needs as directed. Provide post-mortem care. Maintains prescribed standards of cleanliness & organization for assigned work areas replenishing stock supplies as necessary. Remove faulty, non-functioning equipment from service, label, & report to the appropriate personnel. Order supplies & equipment as needed. Enters equipment repair requests in EMSR. Comply with patient safety practices/policies. Comply with nursing department policies regarding patient rights and restraints. Participates in providing appropriate emergency procedures as indicated. Respond to codes & other life-threatening situations as appropriate or directed. Demonstrate understanding of assigned role in external/internal disasters. Participate in emergency drills & mock codes. Utilizes house-wide computer to input pending & actual discharge (within 10 minutes of patient departure). Skills assessments & validations are utilized to ensure that employees providing patientcare to specific age groups (i.e. infants, adolescents, elderly, etc.) are proficient identifying the special needs & behaviors associated with a particular patient population. Responds to patient alarms, i.e. bed alarms, telemetry alarms, etc. Maintains effective communication and collaboration with physician, staff, and responsible nurse. Maintains current knowledge and competency in rhythm and dysrhythmia identification. Compiles and records data for patient record and unit purposes. Notify nurse of any change regarding arrhythmia changes and pertinent cardiac events. Completes other tasks as assigned by the RN or supervisor.
Qualifications Basic Qualifications: Experience
Six (6) months of work experience in a healthcare setting.
Successful completion of a basic arrhythmia recognition competency course no more than two (2) years prior to hiring date, and/or successful completion of an equivalent EKG arrhythmia test upon hire, and annually thereafter.

The Lift Team assists direct patientcare providers using safe patient mobilization techniques such as performing equipment assisted lifts, transfers, turns and repositioning of patients in clinical environment. The Lift Team strives to promote a safe patient handling environment, for both patients and care providers. Lift Team member's coach healthcare clinical staff in use of lift techniques and transfer equipment as needed. This team also participate in Massive Transfusion Protocols (MTP), transporting vital blood products and labs in an expedient and dependable manner.
Function/Duties of Position
* Rounds regularly, 3 times per 8 hour shift, to inpatient ICU floors. Collaborates with clinical staff to assess patient needs before each task (lift or reposition), including type of lift to be performed, condition of patient, and state of patient's physical disability. Performs patient transfers or repositioning using the appropriate lifting and transfer equipment. Identifies availability of necessary equipment and supplies for use in transfer or reposition.
* Responds to patient falls and urgent situations in an expedited manner (in less than 10 minutes). Collaborates with clinical staff to assess patient needs before lift or transfer. Identifies availability of necessary equipment and supplies for use in lift or transfer, using patient retrieval systems, such as HoverMatt or HoverJack, when necessary.
* Responds to requests for patient lifts or transfer from on-site and off-site (CHH and Dental School only) clinics (in less than 20 minutes). Collaborates, if possible, with clinical staff on scheduling prior to patient appointment. Responds to requests via Vocera, pagers or through the Transportation Dispatch.
* Participates in MTP's on the highest priority basis. Responds promptly to the Transportation Dispatchers directives. Maintains continual communication with dispatcher using Vocera and face-to-face communication with MTP clinical personnel. To ensure timeliness of this service, the following must be observed: use of elevators is not permissible and the first response must occur within 30 seconds of pager notification.
