Part Time Physician Assistant/Nurse Practitioner Independent Contractor position rounding at Skilled Nursing Facilities (SNF) on patients undergoing PT, OT, ST. Will need to monitor patients? progress towards their rehab goals and assist facility with certain metrics involving their skilled therapy patients.
Looking for providers in both Sacramento/Stockton Area.
Each patient will need 3 visits a week per facility. 1 of those visits will need to be during business hours on a weekday with close collaboration with the certain staff at each SNF. The other 1-2 visits are typically follow up visits with a very flexible schedule (can be evenings, weekends etc)
Experience in inpatient rehab, pain management, orthopedics, and/or neurology preferred.
Position is very flexible; outside of the first visit, provider can round just about any time including evenings and weekends. There is a scribe service that will perform all documentation as well.
However the position also requires flexibility as well. Weekend availability is required and it will be necessary to drive to all facilities, some of which are closer than others. It will also be necessary to cover other PA?s/NP?s when they are periodically away. During this time volume may be higher.
Job requires:
-Excellent communication
-Willingness to learn rehab as well as how to assist the snf with metrics. There will be a learning curve at first. Training is all done remotely.
-High level of organization
-MS office/Google Drive proficiency
-Physician Assistant or Nurse Practitioner License/Certification
-Car and willingness to drive to all locations

-Previous experience in inpatient rehabilitation or ortho

Self Development: Accountable for self-directed learning and participation in continuing education to meet own professional development. Completes mandatory education timely.
Job Requirements
Required:
* 6 months experience, within the last 3 years, in an acute care hospital setting, healthcare clinic, skilled nursing facility, or home health setting as a CareAssistant or Nurse Assistant, medical assistant or nursing student with one semester in clinical setting, or complete hospital patientcare training.
* AHA BLS
* Basic computer skills/keyboarding/ Medical terminology course or equivalent knowledge

7. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood

8. Providing proper care and observation of patient's skin to prevent breakdown of tissue over
bony prominence.
9. Reporting on patient's condition and significant changes to the Case Manager. Also aware
of the caregiver or other individuals living with the patient and interpersonal issues.
10. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist
11. Offering and assisting with bedpans and urinals. Providing assistance as assigned with
light laundry needs.
12. Performing range of motion and other simple procedures as an extensional therapy service
as ordered with appropriate completed/demonstrated skills competency.
13. Providing respite for patient's/family/caregiver when on-site, as appropriate.
14. Keeping patient's living area clean and orderly, as assigned.
15. Adhering to the organization's documentation and care procedures and standards of
personal and professional conduct.

Position Summary:
The Wine Shop Attendant plays a vital role in creating a welcoming, memorable experience for every guest who visits our Home Ranch hospitality facility. This position is on the front line of our hospitality, combining warm guest service, product knowledge, and operational support in a fast-paced and physically demanding environment.

Wine Shop Attendants are expected to engage guests with enthusiasm, provide knowledgeable and friendly service, and represent the winery's brands with professionalism and care. A strong work ethic is essential, as the role requires long periods of standing, lifting and moving cases of wine, restocking shelves, maintaining a clean and organized retail space, and supporting daily operations in a fast-paced environment.
Outstanding teamwork is critical to success in this role. Wine Shop Attendants work closely with fellow team members to ensure smooth service during busy periods, support events and tastings, and assist wherever needed to deliver a beyond-expectations guest experience. This position is ideal for individuals who thrive in a collaborative setting, take pride in hard work, and enjoy balancing hospitality with hands-on, physical responsibilities.
KEY RESPONSIBILITIES:
Develop in-depth knowledge of Bogle Vineyards family history, wines, winemaking techniques, sustainability commitment as well as a general knowledge of the Delta region
Create a memorable shopping experience for guests
Greet all customers promptly and warmly, providing excellent customer service
Promote the sale of wine and merchandise
Introduce and promote membership in our wine clubs
Ensure that wine shop and all merchandise displays are kept in immaculate condition
Handle sales transactions, including operating cash registers/iPads, processing returns, discounts and other troubleshooting
Acquire, input and maintain up to date and accurate customer information
Fulfill and stage all orders for guests, including table purchases and online orders
Monitor and stock inventory of wine and merchandise continuously throughout shift
Perform opening and closing shift tasks
Regularly assist with a wide variety of hospitality special events
QUALIFICATIONS:
Must be at least 21 years old
Ability to follow written and verbal instructions
Must be able to walk, lift, bend, reach, twist and stoop for up to 8 hours/day
Must be able to lift and carry up to 55 pounds
Confident with organization and multitasking skills
Excellent verbal and written communication skills
Must be a team player and willing to assist others when needed
Ability to work flexible shifts and schedules, including frequent weekends and holidays
Must follow all safety measures in place
Two (2) years of retail experience is preferred
Previous POS/software experience preferred
PHYSICAL REQUIREMENTS:
Must be able to lift and carry at least 55 pounds frequently. Must be able to walk, sit, bend, twist, and/or stand for long periods of time.
COMPENSATION:
$17.40 /hr, plus tips
Individual offers are made contingent upon a candidate's prior experience and education level as they relate to the role. Job offers are contingent upon successfully passing pre-employment screenings, which include a standard 8-panel drug test and a comprehensive background check.
