Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC)or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire (For those hired with wound certification, completion of a fast-track hybrid program for regulatory wound care requirements will be provided)
Knowledge and experience with clinical charting, incident reporting and investigation response is required.
Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
Experience in team building and demonstrating respect for the interprofessional team in wound management is required.
Must have knowledge of national wound guidelines and wound products.
Knowledge of nursing home regulations and survey process is required.

PROBLEM-SOLVING:
The PCA uses judgement to assess resident related situations and assists residents or consults an LPN or Director as required. The variety of tasks to be performed requires the PCA to prioritize duties to ensure the most important duties are accomplished in a timely manner. The PCA reports all hazardous situations, broken equipment, or unusual resident observations to an LPN or Director of The Ridgemont. Failure to follow prescribed procedures, may affect resident safety and satisfaction.
DECISION-MAKING:
The PCA works under general supervision to complete varied assignments using standard procedures. The PCA follows all community policies and procedures. Supervision is consulted if questions arise regarding appropriate techniques or unusual resident conditions.
WORKING ENVIRONMENT:
PHYSICAL - Mobility is necessary in order to monitor and administer resident care. Manual dexterity is used to assess resident condition, retrieve supplies, or perform prescribed treatments. Physical effort is required, often on a daily basis, to lift, push, and pull residents while assisting with the normal activities of daily living. Hearing within normal range is needed to determine blood pressure.
MENTAL/VISUAL - Concentration, attention to detail, and corrected vision are needed to prepare charts, assess resident's condition, and administer treatment to resident.
ENVIRONMENTAL - The PCA is exposed to chemicals and fumes associated with disinfectants used to clean residents' rooms. Exposure to body fluids and odors relating to secretions or wastes. Possible exposure to infectious diseases.
EQUIPMENT - Use of a stethoscope and blood pressure cuff to monitor vital signs, standard office equipment, including phone, fax, and copy machine, physician scales, washer, dryer, emergency call system (computer), whirlpool tub, and etc.
WORKING RELATIONSHIPS:
The PCA has daily contact with residents, family members, and visitors. The PCA also has daily contact with coworkers in order to provide optimal resident care.
MINIMUM QUALIFICATIONS / REQUIREMENTS:
- Reading, writing, and basic mathematical skills and the ability to receive and transmit basic information acquired through a high school level education, or equivalent.
- Must possess and retain current CPR certification.
- Effective interpersonal communication skills in order to cooperatively interact with residents, family members, visitors, and staff.
- Must be able to work a flexible schedule including weekends.
- Must pass criminal background and abuse registry checks as required by state law.

Essential Duties and Responsibilities:
Teamwork with the following and all other duties and responsibilities assigned.
Provides primary care including all ADL's, including but not limited to bathing, dressing, toileting, restorative nursing, and vital signs.
Monitors the safety of the client.
Provides restorative care for clients in their residence as per individual needs, physician order and support plan.
Attends meetings with other staff to discuss client needs and to participate in utilization review, case management, and quality assessment activities and analysis. Attends other meetings as required or appropriate for position
Maintains on going communication with the Care Coordinator regarding client needs.
Other Responsibilities:
Demonstrates respect and regard for the dignity of all clients, families, and fellow employees to ensure a professional, responsible, and courteous environment.
Initiates immediate intervention in abusive situations to protect the client and report allegations according to facility policy.
Notifies Care Coordinator and families when there are changes in condition and emergencies.

The employee must occasionally lift or move up to 100 pounds when moving clients by wheelchair. The employee must occasionally lift up to 10 pounds, which is generally supplies such as reams of paper, files, and forms. Specific vision abilities required by this job include: close vision and peripheral vision in order to monitor client behavior at close range; close vision for computer and paper detail work; color vision to assess changes in coloring of clients' skin or wounds, and to review color-coded spreadsheets or documents; and depth perception and ability to adjust focus from far clients to near clients and from computer to desk or unit hallways.
