About the Role
As a Belmont Village Caregiver, you will provide hands-on personal assistance to a group of seniors living within our large Assisted Living and Memory Care community. The Caregiver role is ideal for someone who is dependable, detail oriented, patient, caring and enjoys a high level of personal engagement with older adults.
Your Typical Daily Responsibilities
Provide personal assistance with Activities of Daily Living to an assigned group of senior residents living in our Assisted Living and Memory Care community
Encourage socialization and inspire participation in community activities while ensuring resident safety and well-being
Document care and services provided daily by following customized service plans and communicating to supervisor any observed changes in care needs
Ensure care, safety and cleanliness standards are met according to regulatory compliance
Provide hands-on assistance with toileting, hygiene and incontinence care as needed
Requirements
Must be at least 18 years of age
High School diploma or equivalent
Must be able to communicate clearly in verbal and written English
Experience as a primary caregiver or relevant training as a care provider
Ability to work the defined schedule for this position which may include weekends and holidays

Responsibilities
Hospital/Surgical Center Responsibilities
May need to round in the hospital on rare occasions with inpatients
May need to assist in hospital consults on rare occasions.
Assists the surgeon with major and minor surgical procedures in the operating room.
Positions, preps and drapes the patient, as needed.
Closes surgical wounds and performs post-operative wound care
Applies dressings, casts, splints, braces, etc.
Performs minor surgical procedures within scope of licensure.
Responsible for coordinating patientcare between the office and the hospital, writing orders for surgical patients both pre and post operatively, and reporting the condition of care of hospitalized patients to the physician
Sees patients in the clinic both independently and with supervision. Dictates clinic progress notes. Orders appropriate tests and develops patient treatment plans with physician approval.
Performs joint aspirations/injections.
Interprets x-rays.
Applies casts, splints, braces, etc. under supervision of the physician.
Removes staples, sutures as needed
Assists in fielding/triaging physician calls.
Reviews lab results, x-rays, and MRI/CT results prior to entering patient exam room
Confers with physician regarding new patient findings and potential diagnosis and treatment.
Administers therapeutic injections and performs minor procedures as needed
Reviews pre-operative instructions and explains the surgical process to patients who are about to have surgery
Provides diagnosis and treatment services to established patients and postoperative patients
Prescribes medications, treatments, in accordance with physician protocols and ensures that all prescriptions and required notes are in patient?s medical record
Formulates and updates patientcare plans; consults with physician as needed
Orders pre-operative tests, CT scans, and MRIs, and documents, utilizing the EHR software
Follows applicable ICD 10 and billing guidelines.
Returns patient phone messages at designated times throughout the day; follow through with appropriate action; documents action taken in patient chart.
Physical requirements
Position requires full range of body motion including handling and lifting patients, as well as manual and finger dexterity and eye-hand coordination.
Employees will occasionally be asked to lift and carry items weighing up to 30 pounds. Position requires lifting associated with patient transfers (up to 50 lbs.)
Normal visual acuity and hearing are required.
The employee will work under stressful conditions and work irregular hours.
The employee will be exposed to bodily fluids on a regular basis.
The employee will have frequent exposure to communicable diseases, blood-borne pathogens, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinical environment.

The PatientCare Tech provides basic patientcare and function as a CNA. Job Responsibilities
Provides basic care to patients under the guidelines set forth according to Hospital policies and procedures.
Answers call lights, telephones, and/or respond to patient requests; acts on routine requests and/or relays information to proper personnel.
Serves patient food trays, snacks, and/or nourishment
Assists with patient mobility, (i.e., lifts, positions patient in bed, transfer between wheelchair and bed, and between cart and bed)

Collects and labels specimens to be sent to the Laboratory.

Maintains a safe, clean, and aseptic environment in assigned work areas.
Education/Experience
Excellent verbal and written skills.
Education equivalent to high school diploma or GED.
Completion of a certified nurse assistant training course or one year satisfactory work performance as a nurse assistant.
Current valid State certification is strongly recommended.
Maintains current CPR Certification.
Must be able to demonstrate the ability to perform required competencies within allotted time periods.

I. JOB SUMMARY:
Provides residents with daily nursing care in accordance with current federal, state and local standards, guidelines and regulations, facility policies and as may be directed by the Charge Nurse, Supervisor, Assistant Director of Nursing, Director of Nursing or Administrator to ensure that the highest degree of quality care is maintained at all times.
II. QUALIFICATIONS
A. Successful completion of a nursing arts course in an accredited nurse training program as evidenced by a diploma, certificate or other written verification from the school and successful completion of the Department approved nursing assistant competency evaluation or successful completion of a United States military training program that includes the content of the Basic Nursing Assistant Training Program
B. Be at least 16 years of age, of temperate habits and good moral character, honest, reliable, and trustworthy.
C. Provide evidence of employment or occupation, if any, and residence for two years prior to initial employment as a nursing assistant.
D. Must possess a high school diploma or G.E.D.
E. Must possess current CPR certificate or become certified within two weeks of employment.
F. Demonstrates interest and/or knowledge in caring for the ill, disabled, and elderly. Must be able to deal tactfully with customers, family members, visitors, personnel etc.
G. Must be able to read, write, and speak the English language in an understandable manner.
H. Must possess the ability to make independent decisions when circumstances warrant such action.
I. Must possess the ability and willingness to work harmoniously with other personnel.
J. Must be personable, enthusiastic, and personable.
III. PHYSICAL REQUIREMENTS
A. Must be able to move intermittently throughout the work day.
B. Must be able to cope with the mental and emotional stress of the position.
C. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be
fully met.
D. Must function independently, have flexibility, personal integrity and the ability to work effectively with residents, visitors, personnel, and support agencies.
E. Must be in good general health and demonstrate emotional stability.
F. Must be able to relate to and work with ill, disabled, young, emotionally upset, and at times hostile people.
G. May be required to lift, push, pull, and move equipment, supplies, etc., in excess of fifty (50) pounds throughout the day.
H. Is subject to exposure to infectious waste, diseases, conditions, etc. including exposure to the (HIV) Aids and Hepatitis B viruses.
IV. ESSENTIAL FUNCTIONS
A. Ensure that all nursing procedures and protocols are followed in accordance with established policies, including dress code.
B. Provides assistance with activities of daily living to a specific number of residents and/or as directed by the staff nurse.
C. Assists in developing a written plan of care for each customer that identifies the problems/needs of the customer.
D. Attends and actively participates in resident care plan conference, and Resident Council and/or Family Council when invited.
E. Reports all concerns, complaints made by the customer or their family to the Administrator.
F. Makes rounds to assure customers are safe and comfortable.
G. Maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.
H. Participates in In-service Education Programs.
I. Assists in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department, and its policies and procedures.
J. Recommends to the Director of Nursing the equipment and supply needs of the department.
K. Inventories and stores the new customer's personal possessions.
L. Assists customer's with packing personal possessions when they are transferred to a new room or when being discharged.
M. Participates in and receives the nursing report upon reporting for duty.
N. Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools.
O. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement.
P. Answers call lights promptly, answer telephone calls as needed and run errands as requested.
Q. Obtains blood pressure, temperature, pulse, respiration, heights and weights as needed.
R. Does complete check of skin condition when dressing, undressing,
turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse.
S. Observes customer's physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated.
T. Performs personal services for customer's (i.e., reading, writing letters, etc.) as time allows.
U. Encourages customers to help themselves and promotes self-care in aiding rehabilitation.
V. Assists with care of dying and expired customers.
W. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident's eating habits to the Staff Nurse.
X. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated.
Y. Serves between meals and bedtime snacks.
Z. Wears transfer belt at all times and utilizes as needed.
AA. Keeps the nurses' call system within easy reach of the resident.
BB. Reports all hazardous equipment and conditions to the Staff Nurse immediately.
CC. Participates in the development, maintenance and implementation of the facility's Quality Assurance program for the Nursing Department.
DD. Performs other related duties as assigned.
