The Care Coordinator is a highly visible customer service and patient-focused role. The incumbent in this role works directly with our patient population and their families, insurance representatives and outside vendors, physicians, clinicians and other medical personnel to ensure the referral process runs smoothly. He/She operates in a dynamic and professional environment to ensure the highest level of quality healthcare is delivered to our members.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Coordinates and processes patient referrals to completion with precision, detail and accuracy.
Definition of completion:
Prioritizes HPP patients in Primary Care Physicians panel, stats, expedites and orders over 5 days.
Orders have been approved (when needed).
Schedules patient (Preferred Providers List of Specialist) and notifies them of appointment information, including, date, time, location, etc.
Uses Web IVR to generate authorizations (Availity, Careplus, Healthhelp NIA and any other approved web IVR for authorization processing).
Completes orders with proper documentation on where patient is scheduled and how patient was notified.
Referrals have been sent to specialist office & confirmed receipt.
Prepares and actively participates during physician/clinician daily huddles utilizing RITS Huddle Portal and huddle guide. Effectively communicates the physicians/clinicians needs or outstanding items regarding to patients.
Enters all Inpatient and Outpatient elective procedures in HITS tool.
Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.
Participates in Super Huddle and provides updates on high priority patients referrals.
Addresses referral based phone calls for Primary Care Physicians panel.
Completes and addresses phone messages within 24 hours of call.
Checks out patients based on their assigned physician/clinician. (Note: If assigned Care Coordinator is unavailable at the time of check out, a colleague shall assist patient. This process does not apply to Care Specialist)
Retrieves consultation notes from the consult tracking tool.
Follows up on all Home Health and DME orders to ensure patient receives services ordered.
Provide extraordinary customer service to all internal and external customers (including patients and other
ChenMed Medical team members) at all times. Utilization of patient messaging tools.
Performs other related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of medical terminology, CPT, HCPCS and ICD coding desired
Detail-oriented with the ability to multi-task. Must be open to cross-functionally training in referrals and back office duties
Able to exercise proper phone etiquette with the ability to navigate proficiently through computer software systems
Team-oriented with the ability to work extremely well with patients, colleagues, physicians and other personnel in a professional and courteous manner
Exceptional organizational skills with the ability to effectively prioritize and timely complete tasks
Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database, and presentation software
Ability and willingness to travel locally within the market up to 10% of the time
Spoken and written fluency in English; Bilingual a plus

Experience with Web IVRs and obtaining referrals/authorizations for multiple payers strongly preferred
Healthcare experience within the Medicare Advantage population preferred.
Medical Assistant certification preferred
CPR for Healthcare Providers is preferred

What You Will Do: The role of the Patient Care Coordinator is to serve as a liaison between the Clinical staff and the patient to coordinate expenses, clinical care, and administrative needs. Track patient workup to ensure current preconceptual labs, communicable testing, pap smear, annual exam, and mammogram (if applicable) are complete.

Maintain patient charts during ART meeting review.
Educate new patients after initial consultation with physician, prior to discharge, and refer to the primary nurse if immediate cycling is anticipated.
Review and verify all IVF/FET Care plans are completed.
Coordinate treatment plans with RN and physician to monitor testing and treatment cycle and schedule appointments according to treatment protocol.
Release/obtain medical records to/from patients, authorized providers, and insurance carriers; maintain compliance with HIPAA laws and regulations.
Maintain physician correspondence for new patient referrals, graduating patients, clinical notes, embryo transfer follow-ups, pregnancy outcomes, and birth congratulations.
Monitor out-of-town patient lab work and results.
Coordinate with nursing team to ensure lab work/results are recorded in chart.
Monitor IVF treatment process and ensure timely patient flow.
Assist Nurse Manager with situations and patient issues as needed.

Job Description
Our friendly and skilled Patient Care Coordinators are key members of our care team. They welcome our patients, coordinate check in/out, and ensure all follow-up care is scheduled before patients exit our office. They also assist with administrative tasks including routing correspondence, coordinating patient documentation, and eligibility/schedule review to ensure a smooth flow for our providers' schedules. Travel is required to our other Chicago ENT practices. This position is a Monday-Friday role that requires a 20-hour week minimum.
This role will report to the Scheduling Lead.
PRIMARY RESPONSIBILITIES of the PATIENT CARE COORDINATOR
Register and schedule patients for a range of ENT, Sleep, Imaging, Cosmetic, Allergy, Audiology, Oral Appliance, and Post-Operative appointments.
Ensure appropriate and accurate data entry of patient information.
Ensure insurance verification accuracy through EMR system and/or directly with the patients' insurance.
Ensure copies of insurance, photo ID, outside medical records, and any required referrals are entered into the EMR system.
Courteously collect outstanding balances and copayments in advance of patient appointments.
Promptly respond to patient and provider inquiries via telephone, web, SMS, and EHR systems.
Provide knowledgeable responses to patients, patient family members, providers and Chicago ENT team members in a courteous and professional manner.
Assist with group administrative tasks including:
o Sorting faxes
o Verifying virtually scheduled appointments via ZocDoc, Healow, and PerfectServe
o Monitoring tasks assigned internally to “User, Scheduler” and “User, Front Desk” as assigned by the Scheduling Lead
o Responding to voicemail messages and Glacial inquiries.
o Reviewing provider schedules for accuracy and rescheduling as needed.
o Monitoring the ******************* general email box.
o Registering patients referred to the practice.
o Patient care coordination including outreach to patients, primary care providers, and/or other specialist offices.
Other duties as individually assigned.
REQUIRED QUALIFICATIONS of the PATIENT CARE COORDINATOR
High school diploma or equivalent required
Ability to work within an EMR database and navigate through a Windows environment required
Must be able to type a minimum of 30 words per minute with 90% or greater accuracy
PREFERRED QUALIFICATIONS of the PATIENT CARE COORDINATOR
1-5 years of experience as a healthcare call center representative, patient access representative, patient service representative, medical scheduler, medical assistant, or medical receptionist
Experience working in a Sleep or ENT Specialist Office
SUPERVISORY RESPONSIBILITY of the PATIENT CARE COORDINATOR
This position has no supervisory responsibility.

Responsibilities
Care Coordination:

Validates appropriate discharge education is provided to patient/ family / caregiver. Interacts with patient/ family/ caregiver to ensure discharge plan meets patient needs Collaborates with Discharge planner to updates patient/ family and interdisciplinary care team of changes in the discharge plan.
Quality Management:
Ensures care provided to assigned patients is consistent with national quality guidelines and appropriately documented in the patient's medical record. Identifies problems and opportunities for improvement in clinical outcomes, patient safety, and or resource utilization. Leads effort to resolve ongoing patient and or systems problems. Participates in unit goal setting, program development, clinical and system process improvement, and achievement of desired unit outcomes. Implements strategies to reduce resource utilization and length of stay for assigned patients. Assists with collection, analysis, and reporting of clinical outcome and resource utilization data.
Leadership:
Participates in local and national professional nursing organizations. We are seeking a dedicated and compassionate Patient Care Coordinator (PCC) to join our growing team. As the first point of contact for patients, the PCC plays a vital role in ensuring a seamless and positive experience through every stage of care. This full-time, onsite-only position blends administrative healthcare operations with high-touch, empathetic service.

Schedules: Candidates will rotate the following shift schedule(s). Must be available to work any of the following shifts (below).
Shift I - 7:30 am - 4:00 pm Shift II - 8:00 am - 4:30 pm or 9:00 am - 5:30 pm. Shift III - 12:00pm - 8:30 pm Saturday - 7:45am - 5:00 pm (rotating) Key Responsibilities: Patient Communication & Support
Provide exceptional customer service via phone, email, SMS, and patient portal.
Serve as the welcoming, professional first point of contact for new and returning patients.
Address inquiries, appointment scheduling, medication requests, and care coordination.
Assist with patient emergencies or admissions in a calm, supportive manner.
Electronic Medical Records (EMR) & Patient Management
Accurately maintain and update patient records.
Support communication of provider messages, appointment reminders, and follow-ups.
Generate and review reports to ensure smooth, efficient patient care.
Medication Coordination
Facilitate medication refill requests and coordinate with pharmacies and providers.
Support patients with clear communication regarding medication policies and timelines.
Insurance Navigation & Billing
Explain coverage, co-pays, deductibles, and prior authorizations in patient-friendly terms.
Verify insurance coverage and collect payments professionally and compassionately.
Ensure accurate billing and reconcile daily payment batches.
Clinical Operations
Greet, check in, and check out patients in a friendly and timely manner.
Support provider scheduling and maximize appointment slot utilization.
Assist with clinic operations and leadership initiatives as needed.
Qualifications
Associate degree (Bachelor's preferred).
Minimum of 1 year experience in a healthcare or customer service role.
Strong communication skills with a warm, empathetic approach to patient care.
Detail-oriented with strong organizational and problem-solving abilities.
Ability to work across multiple locations if needed.
Proficiency in healthcare technology and EMR systems.

The Care Coordinator is a highly visible customer service and patient-focused role. ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Coordinates and processes patient referrals to completion with precision, detail and accuracy.
Definition of completion:
Prioritizes HPP patients in Primary Care Physicians panel, stats, expedites and orders over 5 days.
Orders have been approved (when needed).
Schedules patient (Preferred Providers List of Specialist) and notifies them of appointment information, including, date, time, location, etc.
Uses Web IVR to generate authorizations (Availity, Careplus, Healthhelp NIA and any other approved web IVR for authorization processing).
Completes orders with proper documentation on where patient is scheduled and how patient was notified.
Referrals have been sent to specialist office & confirmed receipt.
Prepares and actively participates during physician/clinician daily huddles utilizing RITS Huddle Portal and huddle guide. Effectively communicates the physicians/clinicians needs or outstanding items regarding to patients.
Enters all Inpatient and Outpatient elective procedures in HITS tool.
Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.
Participates in Super Huddle and provides updates on high priority patients referrals.
Addresses referral based phone calls for Primary Care Physicians panel.
Completes and addresses phone messages within 24 hours of call.
Checks out patients based on their assigned physician/clinician. (Note: If assigned Care Coordinator is unavailable at the time of check out, a colleague shall assist patient. This process does not apply to Care Specialist)
Retrieves consultation notes from the consult tracking tool.
Follows up on all Home Health and DME orders to ensure patient receives services ordered.
Provide extraordinary customer service to all internal and external customers (including patients and other
ChenMed Medical team members) at all times. Utilization of patient messaging tools.
Performs other related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of medical terminology, CPT, HCPCS and ICD coding desired
Detail-oriented with the ability to multi-task. Must be open to cross-functionally training in referrals and back office duties
Able to exercise proper phone etiquette with the ability to navigate proficiently through computer software systems
Team-oriented with the ability to work extremely well with patients, colleagues, physicians and other personnel in a professional and courteous manner
Exceptional organizational skills with the ability to effectively prioritize and timely complete tasks
Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database, and presentation software
Ability and willingness to travel locally within the market up to 10% of the time
Spoken and written fluency in English; Bilingual a plus
