Key Responsibilities
Manage and respond to incoming client communications via phone, text message, and email
Provide clients with an educational and value-enriched consultative approach, to initiate, expand, and close sales opportunities

Responsibilities
Greet patients either in person or via telephone, and update their insurance/demographics in the practice management system.
Assure that physician accepts insurance that patients present with. If not already done prior to visit, verify insurance prior to patients arrival or at minimum at time of arrival before visit.
Communicate with appropriate staff regarding patients arrival, and ensure that patients medical record is available for physician.
Process/update HIPAA-related paperwork and other institutional forms as necessary.
Collect or retrieve referrals or insurance authorizations as required.
Review status of waiting room on a routine basis and ensure that patients are kept advised of wait times.
Ensures that patient has paid co-pay or collects co-pay under direction of billing staff, providing patient with receipt following established cash-control processes.
May schedule patient for follow-up appointment as needed. Provide patient with guidelines for requesting medical records, if necessary.
May perform simple charge entry tasks or enter payments collected from patients and prepare Cashiers deposit. Note: duties are mutually exclusive and may not be performed by the same employee.

Follow all HIPAA and any other governmental or state agency requirements regarding the appropriate handling of PHI documents.

Qualifications
High School graduate/GED.
1 year physician practice experience
Must have a minimum of one week training on-site which will be provided by practice.

We have an immediate opening for a Fertility CareCoordinator to join our team in New York, New York. The location will be determined post training but will be located at one of our four practices: Eastside, Westside, Downtown, or Brooklyn. The schedule is working full-time, On-site, Monday through Friday between the hours of 8:00 AM to 6:00 PM. The pay for this position is $23.08 per hour.
How You'll Contribute:
We always do whatever it takes, even if it isn't specifically our “job.” In general, the Fertility CareCoordinator is responsible for:
Coordinate and schedule all IVF/FET cycles.
Schedule patients for all appointments necessary for the completion of the IVF/EF checklist (labs, saline sonogram, cultures, uterine sounding, SA, etc.).
Ensure patient cycle medication pre-certification is initiated with patient services or financial coordinator
Confirm precertification completed for medications.
Maintain channel of communication with finance department regarding patient financial clearance for IVF cycle prior to GND start. Alert finance department of any changes in protocol or cycle status.
Schedule and review IVF checklist with patient to determine if completed; identify lab completion and ensure recent completion.
Adhere to lab review SOP.
Advise RN when chart is ready for final review.
Distribute all necessary consents (IVF, cryo, donor sperm, assisted hatching, ICSI, FET, etc.) and adhere to all policies for proper consent witnessing and processing.
Counsel patients that consents should be completed at IVF class or no later than by GND start.
Arrange IVF meeting with physicians and IVF nurse for case review: Prepare a list of patients to be discussed, both upcoming and in cycle, any problem or unusual case.
Enter data in EMR chart.
Responsible for EMR entries of any special cycle requirements and diagnosis as determined by physician and/or Nurse at case review.
Coordinate with urology team and embryology regarding any IVF cycles requiring TESE.
Transfer all relevant data regarding male patient onto EMR stimulation chart.
Attend TESE team meetings.
Meet regularly and work collaboratively with nursing staff to: Review upcoming IVF cycle, check charts for completion, confirm written protocol instructions for cycle have been prepared and sent to patient.
Assist patients with understanding of ordering process when using donor sperm for their IVF cycles: Ensure patient has been given appropriate donor sperm ordering information and consents, and that patient receives instructions to return them prior to the delivery of donor sperm.
Provide emotional support to patients when speaking with them.
Create and maintain IVF database (IVF Scheduler). Collaborate with Research Data Coordinator in the maintenance of SART data and statistics; function as back-up person to Research Data Coordinator when Research Data Coordinator is unavailable or on vacation.
Coordinate all aspects of PGT cycles: Schedule patient/partner for pre-screening, if required; Establish contacts with relevant labs; Schedule cycle according to embryology lab guidelines; Provide patient with appropriate consents; Provide appropriate paperwork to PGT lab.
Assist with the coordination of Egg Freezing Patients: Schedule patients for pre-screening, if required; Schedule cycle according to embryology lab guidelines; Provide patients with appropriate consents; Coordinate and schedule patients for Orientation class; Work collaboratively with internal and external Fertility partners, when necessary.
Participate in the orientation of new IVF coordinators.
Work collaboratively with Transfer In team to facilitate the transfer in of gametes to our embryology lab.

The skills and education we need are:
Bachelor's degree in biology, Human Biology, Chemistry, Anatomy & Physiology, Biochemistry or other related sciences is required, and 1-2 years of related experience preferred.

Must be able to work independently as well as in a team environment.

Essential Functions
* Monitor progress of each client on an ongoing basis through delivery of face to face, escort, written, electronic and telephonic outreach/monitoring/collaboration and planning activities, in accordance with Health Home, DOH, OMH, and departmental guidelines.
* Complete client-centered comprehensive functional assessments to identify the medical, behavioral health, and social needs/goals of each client.
* Develop, adhere to, and document daily schedule of appointments; inform supervisor of scheduling conflicts or changes and maintain accurate record of daily activities. Participate in individual and group supervision as scheduled by the appointed supervisor.
* Develop, review, and update written/electronic person-centered care plans that are driven by functional assessment outcomes. Shared and develop/update care plan in partnership with the
client and their Health Home network partners and collateral supports. Ensure that all Care Plans uphold the policy and procedure set forth by the department and Health Home.
* Maintain an accurate caseload panel through prompt identification and response to cases appropriate for level of care changes including but not limited to discharge or transfer activities.
* Maintain collaborative relationships with all service providers utilized in the care planning interventions, sharing/extracting regular status updates and participating in case conferences as needed (and as outlined in the policy and procedure of the department and lead Health Home providers) to monitor level of care and health status for all assigned members.
* Promptly review and address any crisis situations that arise for any client with supervisory staff, service network and any involved legal entities.
* Provide services to clients as needed to meet Care Plan objectives, including facilitating referrals to medical, behavioral health and social assistance entities; assisting with management of entitlements (Medicaid, SNAP benefits, SSI, etc.); assisting with securing stable housing; and arranging transportation and other services to support wellness and health care compliance.
* Utilize Electronic Health/Medical Record system(s) of assigned Health Home and NADAP database tools to maintain documentation and all relevant treatment records, entering contact notes within the timeframe outlined in the Program Manual guidelines.
* Performs other duties as assigned.
Qualifications:
* Bachelor's Degree in Social Work, Human Services or related field required
* Degree in social work, public health, or mental health counseling preferred with two years' experience or a master's degree with one year experience. Hiring Criteria may change depending on standards of governing entity of target population (i.e. DOH, OMH, etc.)
* Requires advanced knowledge of specialized or technical field or a thorough knowledge of the practices and techniques of a professional field. May require knowledge of policies and procedures, and the ability to determine a course of action based on these guidelines.
* Caseloads may flex based on need and acuity of targeted population. Flexibility in caseload management required.
* Bilingual Spanish Preferred
* Must provide personal vehicle and valid driver's license -- mileage reimbursement offered

The Intensive Case Managers operate within a multidisciplinary unit and include Client CareCoordinators. All Intensive Case Managers have at least 2 years clinical experience, which includes client direct contact experience. function as an advocate, facilitator, outreach coach, educator, carecoordinator, and motivational counselor for members and their families for members who have complex behavioral health and or medical conditions.
The role of the Intensive Care Manager includes, but is not limited to the following tasks:
Position Responsibilities:
Providing a timely outreach to new referrals
Engaging members into the program by providing compelling rationale on the benefits of the program to fit the unique member's needs.
Completing members needs assessment to determine appropriate services and inform the care plan.
Developing an individualized member centric comprehensive care plan with input from the member, provider, and family. The individual goals include recovery and resiliency, decreasing symptomatology and/or increasing functional ability in areas such as self-care, work/school, and family/interpersonal relations to reduce barriers to treatment.
Providing monitoring and reviewing of cases through planned outreach, incoming contacts, carecoordination and utilizing rounds, weekly reports, and individual supervision.
Rounding or staffing with a supervisor takes place once per month at a minimum for difficult or challenging cases.
Providing consultation and coordination with the behavioral health or medical providers, facility or family members, community agencies, or involved medical practitioners regarding treatment and/or treatment planning issues.
Providing motivational counseling and encourage self-advocacy to help sustain members' commitment to their care plans and treatment adherence.
Coordinating and consulting with the Care Manager as necessary. Attending regularly scheduled rounds to consult with a psychiatrist or health plan staff and discuss cases and the need for continued intensive care management and outreach. Sending outreach letters to members who are not telephonically accessible or who do not res pond to multiple telephonic outreach attempts.
Frequency of outreach to the member, supports and provider(s) occur at a minimum one time per month, but more may be scheduled according to the member's clinical needs.

Qualifying education: includes degrees featuring a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or another human services field.

Job Summary

Performs essential transitional carecoordination services, including pre-release contacts, day-of-release warm handoffs, assessments and service planning, and assists with entitlements, housing, vocational rehabilitation, life skills, and reintegration services.
Connects members to community support services and outpatient health services, including mental health, substance use, behavioral health, harm reduction and medical services.
Leads and advocates for the member during crisis response, case conference and IDT meetings, when applicable.
Documents all encounters and interventions timely and completes initial assessments, reassessments, service care plans, progress notes (using DAP format), and discharge plans.
Completes all mandated reports in the Health Home Reporting System (FCM) and the Assisted Outpatient Treatment (AOT) portal.
Attends compulsory training, related to prison re-entry, harm reduction, overdose prevention and behavioral health/criminal justice.
Maintains ongoing communication and partnership with DOCCS/Parole, the Department of Homeless Services (DHS), and the Office of Mental Health (OMH).
Provides carecoordination services from strength-based, recovery-oriented, trauma-informed, and culturally appropriate approaches.
Performs other duties as requested by immediate supervisor.

Qualifications
Bachelor's degree in social services, Human services and Social Sciences or, master's degree in social work with license to practice in New York State. At least six years in the provision of community-based social and case management services.
At least two years of experience in a professional environment providing carecoordination or clinically based interventions to individuals involved in the criminal justice systems.
At least two years in providing direct services to people who are seriously mentally ill, intellectually disabled or chemically dependent.
Knowledge of community resources for individuals with serious mental illness, developmental disabilities, or alcoholism or substance abuse.
Professional experience in navigating services for homeless and substance use populations with medically and psychiatrically complex needs.
