Job Description

The Patient Care Coordinator in conjunction with the Nurse Manager, is responsible for the function of the department, ensures that those under his/her supervision adhere to the goals and objectives of the department in accordance with its policies and procedures. Contributes to and supports the mission, vision and values of the department and organizations. Assists with the development of policies and procedures for the department, maintains high visibility on the unit and is a clinical resource person who acts as a role model for the staff on the designated area. The PCC is responsible for maintaining the continuity of patient flow, provides a safe environment for patients, staff and visitors.

Additional Requirements

Graduate of an accredited nursing program. Master s Degree preferred, RN/BSN Degree required. BLS certification and all other required by the department and unit. Must complete a minimum of 8 hours per year of professional development focused hospital wide leadership issues.

Join the Coram/CVS team as a Lead, Intake Coordinator and you will support the branches with the initial admission of patients onto service with the Company. In this role, you will maintain communication and workflow between the branch, Sales staff, Insurance Verification team, and referral sources, helping to ensure new patients are receiving excellent care while on our services.As a Lead Intake Coordinator you will...* Process orders from referral sources and patients, responding to phone and fax inquiries, and resolving patient complaints.* Assess patient needs and document referral requests for coordination of care.* Provide information pertaining to equipment supplies and services to applicable departments in order to bring a patient on service with the Company.* Coordinate with the Insurance Verification Team to process patient details and collect benefit coverage information.* Relaying insurance coverage to referral sources and patients and discuss forms required for billing with patients.*Assist PCCs with training and questions This is a full-time, permanent, benefited position and is NOT through an agency. Required Qualifications

* Minimum two years experience in customer service.* Minimum two years experience in healthcare.* Minimum one year experience working in Microsoft Office, specifically Excel, Outlook and Word.

Preferred Qualifications

Experience working with healthcare insurance.* Experience with medical terminology.* Experience working in a pharmacy setting.* Ability to work in a team environment.* Ability to work with a sense of urgency.* Strong attention to detail.

Spiritual Care Coordinator Education Requirements:



Bachelor's degree from an accredited school of theology or equivalent, two units of Clinical Pastoral Education(CPE) preferred,



or Master's in Counseling or Master's in Social Work with two units of Clinical Pastoral Education (CPE) required;



or Master's degree in theological studies or equivalent and completion of two units of Clinical Pastoral Education (CPE) preferred. If affiliated or endorsed by a denomination or faith group must be in good standing with said group, as evidenced by a certificate of ordination or certification.



Responsible to provide spiritual support to clients, families, and the hospice staff via phone contact, home visits and referrals to the client/family, local clergy. Assists with memorial services and activities relating to spiritual and religious issues of client, family and interdisciplinary team.
